Isolation of Rickettsia akari from a patient in a region where Mediterranean spotted fever is endemic.

Rickettsia akari was isolated from blood collected from a patient in Croatia in 1991. We believe this is the first human isolate of R. akari to be reported in more than 40 years and the first ever from southern Europe. The Croatian isolate was antigenically and genetically indistinguishable from the prototype American strain and a Ukrainian strain. In all probability, rickettsialpox would be diagnosed more frequently and over a wider geographic area if physicians gave greater consideration to the diagnosis and if laboratory diagnostic methods were better able to distinguish among spotted fever group rickettsioses.

Absence of association between the angiotensin-converting enzyme gene polymorphism and borderline hypertension in men of St Petersburg, Russia.

We studied the insertion/deletion (I/D) polymorphism of the angiotensin-converting enzyme (ACE) gene in 131 young males with borderline hypertension (BHT) and in 87 normotensive (NT) young males as a control group using polymerase chain reaction (PCR) technique. The allele frequencies of insertion and deletion ACE alleles were not different in both groups. These results suggest that I/D polymorphism of the ACE gene is not associated with a BHT in males of St Petersburg.

Coxiella and Rickettsiella: comparison of ultrastructure with special reference to their envelope.

Ultrastructure of Rickettsiella phytoseiuli (R.p.) multiplying in female ticks Dermacentor reticulatus was compared with that of Coxiella burnetii (C.b.) in the same ticks and in mice. C.b. in ticks and mice were always represented by 2 main cell types: small dense round or rod-like cells (DC) and larger bacteria-like cells (BC). DC were surrounded with a characteristic five-layered 20 nm thick envelope. Under the envelope DC had a stack of parallel intracytoplasmic membranes with a periodicity 5-6 nm. R.p. in tick fat body and synganglion were also inside phagosomes and formed 6 sequentially developing cell forms: dense (elementary), intermediate, bacterial, giant, and crystal-forming in which small dark bodies (initial particles) condensed. Two of them--dense and bacterial--corresponded to DC and BC of C.b. The DC envelope structure of R.p. was strikingly similar to that of some C.b. DC in mouse. We confirmed the general morphologic similarities in the structure of C.b. and R.p. DC and that of C.b. BC and intermediate cells of R.p. The envelope structure of DC type was found in other gracilicute bacteria and is supposed to have no taxonomic value but to be a reflection of population heterogeneity.

[Malignant non-Hodgkin's lymphomas in abdominal surgery: apropos 3 cases of Burkitt's lymphoma].

On the occasion of three observations of the extremely rare and malignant Burkitt's lymphoma, the authors record their 7-year experience in the operative treatment of 22 patients with non-Hodgkin's malignant lymphomas. Proceeding also from data available in the current literature, the most frequently encountered syndrome-complex is presented, the knowledge of which is of major importance for the early diagnosis, resp. for the results of combined treatment. The possibilities of the operative treatment are evaluated. In the authors' series it was radical in 31.8 per cent of the cases with 13.6 per cent early postoperative lethal outcome and 5.2 months average postoperative survival rate. Proceeding for the rarity of the disease in everyday surgical practice and that it is poorly known, but has become a nosologic entity of increasing interest, the authors have devoted the first part of their article to a discussion on the basic characteristics of non-Hodgkin's malignant lymphomas and the modern achievements of their treatment.

[Our experience with the surgical treatment of acute gynecological diseases].

Experience is recorded in the treatment of acute gynecological diseases based on a series of 61 patients operated in the Clinic of Propedeutics of Surgical Diseases at the Research Institute of Surgery in Sofia for the period Jan. 1983-April 1990. They comprise 6.08 per cent of all emergency cases. The most common gynecological pathology was, as follows: 59 per cent ovarian cysts, 11.5 per cent adnexitis, 26.2 per cent adnexitis + pelvioperitonitis, 6.6 per cent extrauterine pregnancy, etc. The early postoperative results were very good--zero case fatality rate, absence of serious complications, 6.6 per cet suppurations. Taking into account also the very good remote results, the authors point to the trivial appraisal of the surgical activity and the need of thorough evaluation of the patient's fertility, all the more that 78.7 per cent of the women were younger than 39. In the light of the current trends in gynecological practice, the authors specify the indications and contraindications for surgical interventions in these diseases.

[The current aspects of treating residual choledocholithiasis].

Taking into consideration the seriousness of the problem residual calculosis of the common bile duct, the authors analyse the noninvasive methods of treatment during the past 20-30 years. Available data in the literature are adduced pointing to the importance of endoscopic papillotomy and extraction of the calculi. Indications and contraindications for performing this procedure are defined. The different methods of treatment are evaluated on the basis of a follow up of three patients with residual calculosis of the common bile duct, treated by noninvasive method and the experience of treatment of nine patients with this disease for a period of nine years at the Department of Propedeutics of Surgical Diseases, Medical Academy in Sofia, on the one hand, and the experience with 984 patients in whom endoscopic papillotomy has been performed at the Workers Hospital "Dr. R. Angelov" in Sofia, on the other. Preference is given to endoscopic papillotomy with extraction of the calculi in residual common bile duct calculosis over operative treatment, with observance of definite indications. Excellent results of this therapeutic approach are reported: total percentage of early complications 1.52, late complications 1, ineffectiveness of the method 0.4 and case fatality rate 0.6. Another advantage is the limited contraindications and possibility for operative correction.

Listeria isolation from foods of animal origin.

Microbiological examination of Listeria, isolated from foods of animal origin was carried out during the period 1986-1987. A total of 642 samples from chicken, beef and pork raw meats, fish, eggs and from the environment were investigated. Using the cold technique, Stuart's transport medium, tryptose broth and Ralovich's medium, 76 Listeria strains of serogroup 1 and 2 were isolated.

[Cardiac metastasis in primary cancer of the liver].

A rare case of a 77-year-old man with primary liver cancer and an isolated metastasis in the right atrial endocardium is presented. The metastasis was 30/40 mm large and occupied a considerable part of the atrial cavity. Clinically the disease was manifested by progressing chronic right ventricular failure, ending with a total heart failure. The diagnosis was made at the post mortem examination. The rarity of the case is pointed out. The possibility of a correct diagnosis while the patient was still alive is discussed.

Morphological and cytochemical study of Chlamydia with EDTA regressive technique and Gautier staining in ultrathin frozen sections of infected cell cultures: a comparison with embedded material.

The cryo-ultramicrotomy technique was applied to study the ultrastructure of Chlamydia using two strains: one of C. psittaci and one of C. trachomatis. It clearly appeared that in both strains reticulate bodies show a high degree of plasticity, contrasting with the rigid spherical appearance of elementary bodies. Ultrastructural cytochemical study shows DNA fibrils dispersed throughout the cytoplasm in reticulate bodies whereas DNA is condensed in a nucleoid in elementary and intermediate bodies. The EDTA regressive technique reveals ribonucleoproteins in reticulate and elementary bodies of both studied strains.
