In order to determine the diagnostic value of alpha 1-antitrypsin (AAT) globules as a morphological marker of AAT-deficiency of the Pi-Z type, liver needle biopsies from a prospective series of 600 patients were stained with PAS after pretreatment with diastase and by indirect immunoperoxidase staining for AAT deposits. Serum AAT phenotypes of the patients were determined by means of isoelectric focusing. Thirty-two biopsies were from patients with the Pi-Z allele (31 MZ, 1 Z), and 568 biopsies from patients without the Pi-Z allele. AAT globules larger than 3 micron were found in 16 biopsies of which 15 were from patients with the Pi-Z allele (diagnostic specificity 0.94), whereas 20 of 26 biopsies with AAT globules larger than 1 micron were from Pi-Z patients (diagnostic specificity 0.77). Only 47% of the biopsies from patients with the Pi-Z allele contained AAT globules larger than 3 micron. Thus, although AAT globules larger than 3 micron are highly specific as a morphological marker of the Pi-Z allele, their rather infrequent occurrence in carriers of the Pi-Z allele indicates that all investigations concerning the correlation between AAT deficiency of the Pi-Z type and liver disease should be based on phenotyping of sera from all the patients.
Sucralfate, which has cytoprotective, pepsinostatic, antacid and bile acid-binding properties, was studied in a therapeutic trial against a placebo in patients with duodenal and prepyloric ulcer. The investigation was designed as a double-blind controlled study. Thirty-five patients with endoscopically verified ulcers were observed during 8 weeks of treatment. In 14 out of 17 (82%) patients treated with sucralfate the ulcers healed, as compared with 7 out of 18 (39%) in the placebo group, as compared with 7 out of 18 (39%) in the placebo group. In approximately 60% of sucralfate-treated patients all symptoms disappeared, as compared with approximately 30% in the placebo group. No significant side effects were detected, and compliance was good. Consequently, sucralfate represents as effective and safe treatment for duodenal ulcer.

Some purine metabolizing enzymes of lymphocytes and granulocytes were determined in 13 patients with cirrhosis of the liver and in a control group consisting of 18 healthy blood donors. Furthermore cytidine deaminase (EC 3, 5, 4, 5) (CRD) activity was determined in the granulocytes of these patients and in 16 controls. An increase of adenosine deaminase (EC 3, 5, 4, 4) (ADA) activity was found in granulocytes (P less than 0.01) as well as in lymphocytes (P less than 0.01) of the cirrhotic patients as compared to controls. Purine nucleoside phosphorylase (EC 2, 4, 2, 1) (PNP) activity in granulocytes and lymphocytes was identical in the two groups. In lymphocytes of cirrhotic patients decreased hypoxanthine guanine phosphoribosyltransferase (EC 2, 4, 2, 8) (HGPRT) (P less than 0.01), adenine phosphoribosyltransferase (EC 2, 4, 2, 7) (APRT) (P less than 0.02) and adenosine kinase activities (EC 2, 7, 1, 20) (AK) (P less than 0.05) were demonstrated. 5'-nucleotidase (5'-N (EC 3, 1, 3, 5) activity in lymphocytes of cirrhotic patients was slightly increased, the increase being correlated to the level of serum gamma globulin. Granulocytes from cirrhotic patients showed a decrease of CRD (P less than 0.05). The finding that ADA activity is increased in mature lymphocytes and granulocytes from cirrhotic patients argues against the possibility that increase of lymphocytes ADA activity is a consequence of malignant transformation or immaturity.

In a consecutive study of chronic alcoholics admitted for X-ray-negative dyspepsia, endoscopic retrograde pancreatography (ERP) showed pancreatographic changes in 47% of the patients, whereas exocrine pancreatic dysfunction was found in 19%. Only patients with severe parenchymatous damage on ERP had subnormal output of pancreas lipase. In patients with less severe pancreatographic changes the pancreas function test was normal. It was suggested that ERP is a more sensitive method for detection of minor and moderate duct abnormalities in pancreatic inflammatory disease. A clinical diagnosis of pancreatitis had been made in only 22% of the patients before the present investigation. Thus subclinical pancreatitis may be more frequent among alcoholics than previously recognized and should be suspected in cases of unclarified dyspepsia in patients with long-standing abuse of alcohol.

A lethal case of septicemia caused by Yershia enterocolitica serotype 3 is described. A 59-year-old male, previously healthy, presented with 6 weeks of fever, abdominal pains and gradual prostration ending in overhelming septicemia and death before a conclusive diagnosis was established. Necropsy showed cirrhosis, haemochromatosis and numerous abscesses in the liver.

Viral hepatitis has been known to occur among the Greenland population endemically as well as in smaller and larger epidemics. A large epidemic of acute hepatitis comprising around 9% of the entire population, viz. more than 4000 notified cases, swept through Greenland between October 1970 and December 1972. 996 verified cases were seen in the Godthaab district and subjected to more detailed studies. Most of the Godthaab cases were seen among children and adolescents, and no disease was observed in children less than one year of age. Out of 996 diagnosed cases 9 showed acute hepatic failure with coma. Two further cases of hepatic coma were referred for treatment from outside the district. Three of these 11 patients recovered spontaneously. Of the residual 8 cases 6 were treated with exchange transfusions and steroids. Four of these survived and recovered completely. No lasting sequelae had been registered in any of the surviving cases of the epidemic up to June 1975 (2 1/2 years after cessation of the epidemic). Prophylaxis with gamma-globulin was undertaken in a medium-sized settlement in which practically the entire population received gamma-globulin when the first case of hepatitis was diagnosed. In this settlement only 7 out of 297 inhabitants contracted hepatitis. By contrast, in a similar settlement where no gamma-globulin was given, more than 30% of the population developed icteric hepatitis. The clinical features and the prophylactic effect of gamma-globulin seem to indicate that the epidemic was caused by the hepatitis A virus. In accordance with this, transitory Australia-antigenaemia was demonstrated in the acute phase in only 2.6% of the cases, possibly inidicating a small admixture of acute hepatitis type B to the epidemic predominantly caused by hepatitis A virus.
