[Assessment of the state of implanted extracardiac conduits by NMR-tomography].

The object of the work was to reveal the unique possibilities of a highly effective noninvasive method NMR-tomography in evaluating the functional condition of extracardiac conduits in experiments. Conduit models with a stationary flow of blood and 10 dogs with implanted left ventricular-aortic conduit were studied. The studies were conducted on a Soviet MT-1000 NMR tomograph. In all studies with diverse cross sections a distinct location of the heart and the large blood vessels and a "new" left ventricular outlet tract was achieved. Minute features of the inner architectonics of the pumping chamber and the lining of the conduit were detected which could not be found by the routine methods. The study allowed certain conclusions to be made concerning the promising application of NMR tomography in the cardiosurgical clinic. With the use of the method some recommendations may be given for optimization of the designs of extracardiac conduits.

Left ventricular-to-aortic conduit as a variant of surgical treatment of left ventricular outflow tract obstructions. An experimental study.

In 76 experiments on mongrel dogs the authors tried to solve principal problems interfering with a broader clinical application of extracardiac conduit for left ventricular-aortic shunting. They present an original conduit design whose prolonged trials in chronic experiments on dogs yielded favourable results. A new, relatively atraumatic and easy-to-perform method was used for conduit implantation. Functional condition of conduits and systemic and intraventricular haemodynamics were investigated shortly after operation and in the late postoperative period. At various intervals after surgery the conduits corresponded haemodynamically and functionally to systemic blood flow. Use of conduit to construct a new left ventricular outflow tract proved to be an effective alternative in surgical treatment of many hitherto inoperable aortic root defects.

[The first clinical experience in extended myoventriculoplasty with programmed cardiosynchronized electroneurostimulation].

The article deals with the description and analysis of the first two operations undertaken for the development of the method and the first clinical use of extended myoventriculoplasty with programmed cardiosynchronized electroneurostimulation. The authors elaborated the items of stage-by-stage training of the skeletal muscle for its adaptation to work in a regimen of the contracting heart, and the techniques of myoventriculoplasty in patients with a complicated form of ischemic heart disease and dilatation cardiomyopathy. Apart from that, the authors approved for the first time in the clinic an original Soviet-made programmed cardiosynchronized electroneurostimulator intended for implantation. The immediate postoperative results were satisfactory.

[What should be the shape of extracardiac conduits?].

Studies of the hemodynamic features of the blood flow in extracardiac conduits allow for a grounded and aimed approach to the prosthesis design. The lack of interrelationship between the duct geometry, the flow pattern, and the heart anatomy is one of the reasons for the existing conduits inadequacy. Here is proposed a confuser conduit model providing a stable uninterrupted flow. Development of a hemodynamically optimal type-size series of conduits would allow one to reduce the incidence of complications resulting from inadequate prosthesis design.

A method of extended myoventriculoplasty with programmable cardiosynchronized electroneurostimulation: description and initial clinical results.

Description and results of the first clinical use of a new method of extended myoventriculoplasty with the simultaneous implantation of a cardiosynchronized programmable electroneurostimulator are presented. Stepwise training of skeletal muscle in order to adapt it to fulfilling the function of the contracting heart was studied. The technique of myoventriculoplasty was used in patients with severe ischaemic heart disease and dilated cardiomyopathy. During the study, an original Soviet-made implanted programmable and cardiosynchronized electroneurostimulator was tested for the first time in clinical practice. Early postoperative results are satisfactory.

[Study of hemodynamics after creation of a new experimental outflow tract for the left heart ventricle].

Experimental analysis was made of hemodynamic changes occurring in the left heart and systemic circulation after making an aorto-left-ventricular shunt while simulating the complete obstruction of an outflow segment in the left ventricle. The hemodynamics of the left cardiac cavities in a "new operation" was studied. Analysis of the recording of pressure curves in the aorta, large arteries, and left ventricle, of electromagnetic flowmetric, echo-, and Doppler cardiographic findings suggested good results of the operations in the periods up to 12 months. The proposed model of an experimentally induced artificial aorto-left-ventricular shunt with conduit allows one to test and study the functional state of various types of prosthetic valves (biological and artificial) in long periods after surgery.

Scientific prognosis of trends in the development of cardiovascular surgery in the socialist countries.

The growing incidence and importance of cardiovascular diseases in a number of countries calls for systematic quest for most efficient methods of treatment, which besides prevention and drug therapy include also surgical treatment. A group of experts from member countries of the Council for Mutual Economic Assistance (CMEA) has worked out a scientific prognosis of trends of the development of cardiovascular surgery, which include surgical treatment of ischaemic heart disease, congenital and acquired heart defects, renovascular hypertension and affection of the aortic arch. The prognosis includes also an estimate of the number of operations which will have to be performed in the mentioned diseases per 1 million inhabitants of the CMEA member countries.
