[Head of psychiatric service--a position or a vocation?].

An attempt to create a social and psychological image of contemporary psychiatric department leader has been made. Professional and social activity of a modern head physician, his personal, business and other qualities are described. Taking into account these characteristics, the author suggests a system of competitive selection for this position and a broad discussion of the problem in psychiatric society.

[Terrorism and mental health (problem's scale, population tolerance, management of care)].

The consequences of terrorist threat and terrorist acts for mental health of the individual, groups of individuals and community in general are analyzed. Mental disorders emerging in the victims of terrorism is described. The problem of terrorist threats use as a psychic weapon is discussed. Tolerance of population to terrorism can be divided into two types--psychophysiological and socio-psychological. The ways for elevating tolerability to terrorist threat and terrorist acts are suggested. Help in the centers of terrorist act must be of the complex character, being provided by different specialists including psychologists and psychiatrists. The importance of state structures and community support in this work is emphasized.

[Methodological bases for problem solution of monitoring and estimation of mental health services activity: national and international aspects].

General principles and questions, concerning the problem of monitoring and evaluation of psychiatric service's work from the points of systemic and mathematic modeling, are considered. An analysis of the structure and parameters values of national psychiatric service is presented. Suggested, are recommendations on using this information and improvement as well its collection, processing and presentation of the results in the national and international aspects. Taking Russia as an example, a selective correlation analysis of the regional rows of the values of the indices for psychiatric service's work was conducted to reveal their informational excessiveness. It is shown that the sets of the indices existed and being elaborated for solution of the monitoring problem and estimation of psychiatric service's work are excessively informative and can not be used without an additional analysis in corresponding mathematic modeling. In the view of the condition of the discussing questions, a use of hierarchical modeling apparatus seems to be the most natural and adequate for the solution of the problems posited.

[Basic points of conception of quality assurance in mental health care].

The article presents conceptual approaches to the problem of quality assurance in mental health care, which accumulate a previous national and international experience in the field of medical and psychiatric care quality, taking into account the results obtained by the authors. The article addresses juridical, ethic, economic and organizational aspects of mental health care improvement. Attention is drawn to a need in controlling system and quality evaluation based on adequate and informative criteria and quality standards and in making scientifically founded administrative decisions. Specific criteria for evaluation of mental health care quality, taking into account its main components--structure, process, results--were suggested.

[Modern psychiatric service and criteria for the estimation of its activity].

During the last decade the efforts of psychiatrists and managers of psychiatric service were directed to solving the problem of modern system of psychiatric care and its quality. In Russia the problem has to be solved under conditions of both social instability and economic crisis. The paper presents the data concerning investigation of the problem in terms of epidemiological, social, economic, ethical-juridical and organization criteria. The author concluded that the significant criteria for efficiency of the psychiatric service activity may be the following: satisfaction of the patients with this service; quality of their life; the degree of social integration; the level of their stigmatization in the society; the data about consequences of mental diseases for the family and the society; scientific-founded standards of medical-diagnostic and medical social aid. Social support and care of mentally ill patients and members of their families are essential in modern psychiatry.

[Contemporary system of mental health care: epidemiologic aspects].

The authors suppose that adequate mental health care management should be based on the knowledge about the number of its potential patients. In order to determine this number the authors formulated a hypothesis that the necessary period of the observation of patients depends on the course of the disease and on the risk of the relapse. From the authors' point of view all mental disorders comprise 3 groups. Patients who belong to the 1-st group need observation during all their life; patients from the 2-nd group need observation during an active period of the illness; patients from the 3-d group need observation during the acute period only. On the basis of estimation of the full number of mentally ill people in Russia the authors calculate a size of each group and the total number of the potential patients--about 21 million of people (14% of the Russian population). This rate has been defined as "the actual number of mental patients".

[Protection of mental health: a priority task of the state and the society].

The paper is devoted to the World Health Day 2001--Mental Health Day. It gives a history of World Health Day. The state of the psychiatric science in Russia, the reforms carried out in psychiatry during the last decade and some measures of the perspective development of Russian psychiatry, proposed by the scientists and by the heads of some regional psychiatric services during the last years are considered.

[The modern model of medical and social care for the elderly].

From forty to seventy per cent of the elderly are reported to suffer from psychiatric disorders. Being prone to somatic and psychiatric comorbidity, aged people are also need for social care. This reason stipulates psychiatric care optimization and organizing a specialized service, providing care for the elderly. Service structure may be organized as following: (1) psychogeriatric units in psychiatric institutions; (2) psychogeriatric units in geriatric or general health care institutions; (3) psychiatric services and psychiatric facilities in social institutions. Each unit should comprise such kinds of care as day-hospital, in-patient clinic and out-patient clinic. Federal- and regional interdepartmental and interdisciplinary psychogeriatric centers may be established for elaboration of methodological and practical principles and coordination of geriatric, psychogeriatric and social care service's activity for providing care to the elderly.

[Organizational and economic aspects of psychiatry].

The main trends in the organization and economic transformations in psychiatric services of Russia in 1989-1996 are analyzed: financial support of the institutions of psychiatric care, changes in the organizational structure of services, new forms and types of care. The opinion of the authorities of regional services concerning the organization and economic changes in institutions of psychiatric care are presented.
