The management of unreduced traumatic dislocation of the hip in developing countries.

Traumatic dislocation of the hip often remains untreated in developing countries. This paper reports the outcome of twenty nine patients with neglected dislocations (3 anterior and 26 posterior). We have outlined the indications for five different methods of treatment. An attempt has been made to explain the causes of poor results.

Management of unreduced traumatic posterior dislocation of the hip: heavy traction and abduction method.

Eight cases of unreduced traumatic posterior dislocation of the hip were treated by heavy traction and abduction. In four out of eight cases a concentric reduction was obtained. In two patients where a concentric reduction was not obtained, open reduction was performed. In these patients, there was an excellent or good result. In the remaining two patients a non-concentric reduction was accepted, and they had poor clinical results.

Crossed fused renal ectopia.

An unusual form of crossed renal ectopia is presented. Both kidneys are at the same level on the same side with fusion throughout their entire lengths.

Atypical coronal or sagittal Z ruptures of the achilles tendon: a report of four cases.

A typical rupture of Achilles tendon occurs as a complete rupture about 4-6 cm proximal to its insertion to the calcaneus. The morphology of the tear is most often random without any distinct recognizable pattern. The authors describe four cases of atypical "Z" rupture of the Achilles tendon. In two, the orientation of the "Z" was in the coronal plane and other two in the sagittal plane. All four cases were treated by open repair.

Achilles Tendon↗

Rupture of the peroneus longus tendon.

The authors report a case of fracture of the os peroneum with diastasis in a 34-year-old woman. This represents one of the few cases of this type of fracture to be reported in the literature. This injury must be included in the differential diagnosis of a sprained ankle, and only careful examination of serial roentgenograms can prevent an incorrect diagnosis.

Rupture of the plantar fascia.

Rupture of the plantar fascia in athletes engaged in sports that require running and jumping has been reported. However, spontaneous degenerative rupture of the plantar fascia is not well documented in the literature. This paper reports a patient with degenerative rupture of the plantar fascia.

Aged↗

Medial malleolar fracture associated with deltoid ligament rupture.

The author reports a case of a fractured medial malleolus with a completely disrupted deltoid ligament following a pronation injury. To the author's knowledge, concurrent failure of both structures over the medial side of the ankle has not been previously documented in a pronatory injury.

Rotator cuff repair in a district hospital setting: outcomes and analysis of prognostic factors.

In a prospective study of 58 complete rotator cuff tears, 54 patients underwent surgery by a single surgeon for chronic, symptomatic, full-thickness rotator cuff defects. An independent observer performed the evaluation. Fifty-two of 58 patients reported marked relief of pain and rated themselves as having mild or no deficit in their ability to perform daily activities. The study evaluated preoperative and intraoperative factors that influenced postoperative outcome. Eighty-three percent of patients had good or excellent results according to the modified Constant scoring system and 76% with the UCLA system. There was no pain or minimal pain in 90%, and 92% reported overall improvement after the surgery. It is concluded that in a patient with a full-thickness rotator cuff tear, pain and shoulder function can be improved by acromioplasty and cuff repair, but a slight decrease in the range of motion and muscle strength will probably remain.
