[Why do tumor cells avoid immune surveillance?].

Why does cancer develop? What is the essence of premalignant lesions? My answer to these sacramental questions follows. I found out that hyperplastic and tumor nodules are immunologically privileged sites, like the anterior chamber of the eye or the hamster's retrobuccal sac. The antigens from the nodules do not provoke the body's immune reactions, unlike the same antigens from the extranodular tissue. On the other hand, both the autoantibodies and the killer-cells sensitized to this antigens are unable to react with that part of the cells possessing antigens, which are situated within the nodules and completely destroy the same cells in the extranodular tissue. Hyperplastic and tumor nodules are special sites where immunological surveillance is absent. It is because they have no lymphatic and so recirculation of immunocompetent cells through its territory is impossible. Thus, newborn malignant cells in these precancerous formations avoid the immunological control and survive, in contrast to the tissue with the normal structure. So, neoplastic processes will be free from immunological surveillance in the precancerous sites and will therefore be more likely to progress.

[Pathomorphism of endemic goiter].

Pathomorphism of goiter developing against the background of goiter prevention in the Bukovina endemic area was studied. Subdivision of goiter into endemic and sporadic lost its significance due to the stabilization and levelling of the goiter morbidity. The true (simple or benign) goiter is characterized by an euthyroid course, predominant affection of adult women and domination of the nodular variants. A goiter node is a hyperplastic structural-functional gland unit--the thyron. The leading role in the goiter etiology belongs to endogenous deficiency of thyroid hormones, local factors are significant in the goiter pathogenesis.

[Mathematical model of the morphogenesis of tumor nodules].

A mathematical model of morphogenesis of tumor nodes has been developed on the basis of the physical theory of elasticity. According to the model, one of the important morphogenetic factors of tumors consists of their internal elastic forces determining the structure of tumor nodes and intermittent phase-wise pattern of their development. The existence of a certain "critical point" in the morphogenesis of nodes (determined by the size of the tumor and its histological structure) was established the passing of which lead to a sharp decrease in the tissue pressure in node centers. The gradient of tissue pressure in tumor nodes is the leading pathogenetic factor of secondary changes and results in the appearance of a typical zonal histotopographic structure of tumors. On the basis of the rate of tumor growth alone, the model allows the intensity of their cell division to be determined and also shows the process of cell proliferation in tumor nodes to be subject to the regularities of Fibonacci number series.

[The orthogenesis of atherosclerosis].

It has been ascertained that myofibroblasts penetrating the aortal intima from the medium layer because of its compensatory rearrangements in sites of wear and pull of the vascular wall when a fibrous plaque becomes a lipid one, get transformed in stages. Ordinarily elongated orderly arranged cells become spider-like ones, polymorphous, disintegrated cells. Lipid drops get accumulated in their cytoplasma. The above myofibroblasts were identified as Langhans' cells. Changes are reported in aortal intima plaques identical to those occurring in cell culture near the Geĭflik's [correction of Geuflick's] line. It means that accumulation of lipids in aortal plaques and atheromatous degeneration of the latter may be related to aging and necrosis of myofibroblasts which reach the Geĭflik's [correction of Geuflick's] line too soon in such sites of their compensatory proliferation as enlargements of fibrous tissue in the intima. Thus, atherosclerosis is to be regarded as quite a natural process, as reaction of re-arrangements of the vascular wall in sites of its premature wear. Prophylaxis of atherosclerosis is a problem of major social concern, which may be solved only through providing adequate conditions of work and life, cultivating a healthy life style.
