[Roentgeno-endovascular occlusion as a method of hemostasis in abdominal trauma].

Under analysis is the authors' experience with roentgen-endovascular occlusions (REO) in 18 patients with closed injuries of the abdomen with massive hemorrhage. The embolization of spleen vessels was made in 8 patients, hepatic vessels--in 5 patients, renal vessels--in 3 patients and iliac artery branches--in 2 patients. In 10 cases REO was used as an independent method for arrest of hemorrhage and in 8 patients--for the preoperative preparation of the patients. The home hemostatic sponge was used in combination with superselective administration of the aminocaproic acid as the embolizing material.

[Surgery of nonparasitic liver cysts].

Under observation there were 24 patients with non-parasitic cysts of the liver. The cysts of the liver are frequently localized along the major or right portal fissures. According to indications, different surgical methods were used (resection of the liver, enucleation of the cysts, partial dissection of the cyst with tamponage of the cyst cavity by the omentum etc.). 22 patients operated upon recovered. Remote results were good.

[Emergency angiography in the diagnosis of nonpenetrating injuries of the abdominal organs].

On the basis of 87 urgent angiographies in 65 patients the diagnosis of injuries of parenchymatous organs is described. The patients are divided into 4 groups: 1-contusion of the organ, 2-subcapsular and central hematomas, 3-transcapsular ruptures and fragmentation of the organ, 4-multiple injuries. Urgent angiography helps to make the exact diagnosis, to establish the character of injuries and to choose the method of treatment of the patients and the operation technique.

[Anatomical and surgical basis for liver resection in cancer].

On the basis of the results of a study on the interaction of the venous and lymphatic systems of liver and gall bladder, the authors offer an original method of resection of portal lobe of the liver for primary cancer of this organ. Application of oncologically-adequate surgical procedures ensures good results and a favorable prognosis.

[Methods of excluding the liver from the general circulation for its subsequent perfusion].

Methods of exclusion of the liver from the general blood circulation for its further perfusion were studied in 316 experiments in corpses of humans and dogs. Original caval and portal canules constructed by the authors were used in 180 experimental operations in dogs weighing from 10 to 35 kg to prove the possibility of occlusion of the vena cava inferior above and below the liver to take blood from the hepatic veins and to return it into the liver after saturation with oxygen and drugs through the portal vein tributaries and hepatic artery.

[Artificial circulation method in operations on the liver].

The method of parallel and isolated artificial circulation in the liver is described on the basis of 97 experimental operations on dogs of various weights. The paper gives a brief description of the apparatus for the regional perfusion of the liver and portable oxygenator. The composition of the perfusate and main criteria of the liver perfusion adequacy are considered, including the hemodynamic shifts and disorders in the hepatic functions under the artificial circulation in the liver. The perfusion is estimated in different temperature regimens, the advantages of the hyperthermal perfusion over normal and hypothermal ones being also shown. The method of artificial circulation is recommended in vast resections of the liver, in transplantation and in the treatment of hepatic malignancies.
