Assessment of rotation in proximal humeral osteotomy.

The correlation between the shift measured on bone surface in rotatory osteotomy and the radiologic change of the retroversion angle was analyzed. The humeral head retroversion was measured radiographically in 7 isolated humeri by means of a semiaxial projection. All bones were then osteotomized and the humeral head rotated in order to increase and decrease the retroversion. The retroversion was measured radiographically for each new rotational position of the head. The relationship between the measured shift in rotatory osteotomy and the change of retroversion was found to be linear. Twelve patients with recurrent anterior shoulder joint dislocations and a small humeral head retroversion were operated on in order to increase the retroversion. In all patients the increase of retroversion angle was smaller (57%) than expected on the basis of the shift in osteotomy and humeral diameter. The mean increase of retroversion was 2 degrees/mm shift (range 1.3-3.0 degrees/mm).

Recombinant leukocyte interferon alpha-2a and medroxyprogesterone in advanced renal cell carcinoma. A randomized trial.

In a randomized study of advanced renal cell carcinoma 60 patients were allocated to treatment with either recombinant interferon alpha-2a or medroxyprogesterone acetate. Correlation between the dose of interferon alpha-2a and plasma-concentration indicated linear kinetics. Survival was similar in the two treatment groups. Only one complete and one partial response were seen in the interferon group and only one complete response in the medroxyprogesterone group, indicating a low therapeutic potential of both interferon and medroxyprogesterone. Interferon influenced the serum liver enzyme levels; increased transaminases were seen in 17 patients treated with interferon but in only four patients in the medroxyprogesterone group. Two patients had very high serum liver-enzyme levels concomitant with intolerable tiredness, in both patients the symptoms disappeared and the enzymes normalized after discontinuation of the interferon treatment. Antibodies to interferon developed frequently in patients receiving high dose oligomeric interferon therapy but rarely in patients receiving low dose monomeric interferon treatment.

Retroversion of the humeral head in the normal shoulder and its relationship to the normal range of motion.

Retroversion of the humeral head and the range of motion of the shoulder joint in both the frontal and the scapular plane have been studied in 100 shoulder joints in 50 healthy subjects, 25 men and 25 women. The average angle for humeral head retroversion was 33 degrees on the dominant side and 29 degrees for the nondominant side. There was no difference between genders. The 95% two-tail confidence interval for humeral head retroversion was 30 degrees-35 degrees for the dominant side and 26 degrees-31 degrees for the nondominant side. A larger retroversion angle was consistent with an increased range of external rotation. Mean external rotation in 90 degrees abduction was 105 degrees in the frontal plane and 120 degrees in the scapular plane. External rotation in 90 degrees abduction was greater in women than in men, but there was no difference between dominant and nondominant shoulder joints. The range of motion of rotation was found to be greater than the accepted normal values.

Boston brace in the treatment of idiopathic scoliosis.

In a retrospective follow-up study of 64 patients with adolescent idiopathic scoliosis, the initial correction of Boston brace with straight lumbar profile strongly predicted good treatment results at follow-up (mean, 4.8 years). If the initial correction was > 50%, the curve was permanently reduced with an average of 7.2 degrees. The average initial correction was 62%. This initial brace effect was preserved in another comparable group of 60 patients treated with a Boston brace with 15 degrees lumbar lordosis. It is concluded that the Boston brace is effective in the conservative treatment of idiopathic scoliosis, and the corrective ability seemingly does not deteriorate with a change in the brace design from 0 to 15 degrees lordosis.

Radiologic assessment of humeral head retroversion. Description of a new method.

A radiologic method for assessment of the humeral head retroversion angle has been developed using one radiograph obtained in the semi-axial view. Validity and reliability of the method has been tested. In five healthy volunteers both shoulders were examined both with CT and with the new radiographic method. The average difference in angle determinations between the methods was 1.5 degrees and the maximum difference was 2 degrees. Angle determination on radiographs from 22 healthy shoulders was performed by two independent radiologists. The coefficient of variation for intraobserver measurements was 2.8 per cent and for interobserver measurements it was 4.6 per cent. Three isolated humerus bones were examined in multiple semi-axial projections and the humeral head retroversion was measured. The effect of humeral position (flexion, extension, abduction) on angle assessments was analyzed. A method error exceeding 2 degrees was only seen when the specimens were in an extended or extremely abducted position. It is concluded that with the arm in the correct position measurements of humeral head retroversion can be performed with this method with high accuracy.

Influence of acetabular component position in non-cemented total hip arthroplasty. A radiologic and clinical study.

Radiologic grading of non-cemented acetabular components in HP Garches total hip replacements was used to evaluate the relationship between a lack of supero-lateral cover of the acetabular component and clinical outcome. In 63 hips, three to five years after total hip arthroplasty, we observed that the acetabular component was partially uncovered in 56 per cent of cases. The fact that several components were partially uncovered did not influence the clinical outcome. Although the follow-up time is short the present study indicates that non-cemented HP Garches total hip arthroplasty is a technique which allows for stable fixation of the acetabular component.
