Pregnenolone sulfate modulates NMDA receptors, inducing and potentiating acute excitotoxicity in isolated retina.

Pregnenolone sulfate (PS) acts as a positive allosteric modulator of N-methyl-D-aspartate (NMDA) receptor-mediated responses. In the retina, we previously observed that the synthesis of pregnenolone and PS increases after stimulation of NMDA receptors and blockade of the synthesis reduces retinal cell death. This study was carried out to explore in the isolated and intact retina the possible role of PS in NMDA-induced excitotoxicity. Lactate dehydrogenase (LDH) measurements and morphological analysis revealed that a 90-min exogenous application of PS at 0.1-500 microM concentrations potentiated NMDA-induced cell death and at 50-500 microM concentrations caused cytotoxicity. After 45 min, either NMDA or PS caused no significant LDH release; but their co-application resulted in a high degree of toxicity. In addition, we found that a mild NMDA insult developed into serious damage when even low PS concentrations (0.1-10 microM) were used. Toxicity-inducing and -potentiating effects were specific to PS modulatory action on NMDA receptors, in that they were blocked by 4-(3-phosphonopropyl)2-piperazinecarboxylic acid (CPP) and MK-801 but not by 6-cyano-7-nitroquinoxaline-2,3-dione (CNQX), and neither dehydroepiandrosterone sulfate nor pregnenolone caused LDH release. Prevention of degenerative signs was seen in retinae pretreated with 4,4'-diisothiocyanatostilbene-2,2'-disulfonic acid (DIDS), a Cl- channel blocker, thus indicating a Na+/Cl--dependent acute mode of excitotoxic cell death responsible for PS toxicity. The positive interaction between the neurosteroid and NMDA receptors was further proved by a PS dose-dependent increase in NMDA-induced stimulation of [3H] MK-801 binding to retinal membranes. The results suggest a crucial role of PS in retinal vulnerability and propose the toxicity-potentiating effects as an important key in linking NMDA-induced endogenous synthesis to acute excitotoxicity.

Respiratory function and liver cirrhosis.

Respiratory function was studied in 27 patients with liver cirrhosis and in a control group of 36 subjects. A significant difference in the overall comparison of the cirrhotic patients vs the control group was found only for average values of PEFR and FEF 25% (p less than 0.05); the significant reduction of PEFR was still present in ascitic subgroup. Considering other subgroups created vs control group, a restrictive alteration was observed in patients in Child's class C, with lower albumin levels and in presence of hyperbilirubinemia. An obstructive dysfunction was present in alcoholic cirrhotic patients. A significant decrease (p less than 0.05) of FEF 25-75% and FEF 75% (a specific small airways function index) was observed in patients with esophageal varices vs patients without varices, not related to ascites. The small airway functional alteration seems related to hemodynamic features of portal hypertension.

[Correlative evaluation of electrocardiographic changes and spirometric parameters in pulmonary cardiopathy secondary to chronic obstructive bronchopneumopathy].

EKG is not considered a sensitive procedure for diagnosis of chronic pulmonary heart disease (CPHD). We performed a correlative study between EKG signs and spirometry in chronic obstructive pulmonary disease (COPD). Fifty-six consecutive in-patients (M 52, F 4, age 59.1 +/- 13.1 yrs) with COPD and echocardiographic/roentgenologic signs of right ventricular enlargement were studied. An EKG score system (0-6) was developed considering: 1) AQRS greater than or equal to 90 degrees; 2) P waves greater than or equal to 2.5 mm; 3) R wave V1 greater than or equal to 7 mm or R/S V1 greater than or equal to 1; 4) R/S V5 less than or equal to 1; 5) RV strain pattern; 6) RBBB. Significant (p less than 0.01) linear correlations were observed between: a) AQRS vs FEV1 (r = -0.59); b) AQRS vs FEV1/FVC% (r = -0.61); c) R/S V5 vs FEV1, (r = +0.38); d) EKG score vs FEV1 (r = -0.42); e) EKG score vs FEV1/FVC% (r = -0.40). Significant relationship between EKG score and spirometry suggests that this criterium can be useful for a more reliable ecg diagnosis of CPHD.

Vasoactive effects of buflomedil assessed by fluorescein angiography.

The vascular effects of buflomedil were studied by retinal fluorescein angiography carried out before (control) and after the intravenous infusion of buflomedil (250 mg in 15 min) in patients with different pathological conditions of retinal circulation. Sequential photograms, taken every 36 sec from the 10th sec after the start of the fluorescein injection, showed differences in circulation times between the two sequences in 9 (75%) out of 12 patients. Arterial phase and early venous phase differences were evident, with a mean delay after buflomedil of 3.24 +/- 2.35 sec and 3.22 +/- 2.58 sec, respectively. Significant differences were observed in the time to venous filling completion, with a mean delay of 6.99 +/- 5.19 sec after buflomedil. It is considered that the delay in the retinal arterial phase is related to the arm-retina circulation time, and the slowing down in venous filling is related to a lengthened circulation in the capillary bed.
