[Cytogenetic study in multiple myeloma. II. Clinical significance of chromosomal changes].

The authors evaluated in a group of 56 patients with multiple myeloma the clinical impact of assessed chromosomal changes. They found a close relationship between the frequency of chromosomal deviations and the extent and activity of the disease. The authors did not detect a relationship between the frequency of karyotypic changes and the number of plasmocytes in bone marrow nor a significant difference in the incidence of hypodiploidy in different immunochemical types of multiple myeloma. Patients with chromosomal changes had a reduced response to cytostatic treatment. The authors revealed a prognostic importance of chromosomal changes, in particular of hyperdiploidy and the concurrent incidence of numerical and structural deviations. The authors discuss the relationship of monosomia of the X chromosome and the Y chromosome to the clinical picture of the disease and chromosomal findings in secondary myeloblastic leukaemia, myelodysplastic syndrome and secondary plasmocellular leukaemia.

Ultrasonography of primary liver carcinoma (PLC) and targetted cytologic examination.

A primary liver cell carcinoma was disclosed in 11 cases out of a total of 61 patients with a focal hepatic lesion. The authors investigate the diagnostic significance of abdominal ultrasonography (US) as a major diagnostic modality in early detection of PLC. It is concluded that abdominal US is an important contribution to diagnosis of hepatic lesions smaller than 1 cm in diameter. The echographic features of PLC are often unspecific thus requiring a complementary targetted cytologic examination. A short comparison of US computed tomography and liver scintigraphy is given.

Mediastinal teratoma and acute megakaryoblastic leukemia.

The association between mediastinal teratoma and acute megakaryoblastic leukemia (AMKL) in a 15-year-old boy is described. The clinical course is compared with 20 previously reported cases of AMKL in children. Chromosome studies at diagnosis of the leukemia showed multiple leukemic stem lines with numerical and structural abnormalities.

Bone marrow necrosis in malignant diseases. A report on seven intravitally recognized cases.

Bone marrow necrosis (BMN) is a necrosis of the hemopoietic tissue including the fibrovascular medullary stroma. Most frequently, it is caused by failure of bone marrow microcirculation. It is a complication in a wide spectrum of diseases, most frequently of malignancies, and is only rarely diagnosed ante mortem. In 6 of our 7 intravitally diagnosed cases, BMN was recognized already at the cytological examination of the bone marrow and was verified by the histological examination of the biopsy specimens as well as at necropsy. All our patients suffered from various malignant diseases. Three had generalized gastric carcinoma, the remaining hematological neoplasias: Acute lymphoblastic leukemia, acute monocytic leukemia, blastic transformation of chronic granulomegakaryocytic myelosis and primary medullary centrocytic lymphoma. The survival varied from 4 to 14 weeks after the BMN diagnosis. Clinical, hematological and autopsy findings as well as the etiopathogenetic views and prognostic implications of the diagnosis are discussed.

Significance of nucleo-cytoplasmic maturation asynchrony in multiple myeloma.

Bone marrow samples of 97 patients with multiple myeloma were examined ultrastructurally over a period of eight years, and the degree of nucleo-cytoplasmic maturation asynchrony (NCA) of myeloma plasmocytes was estimated according to the classification scale of Graham and Bernier. One half of the patients showed first degree, 40 percent second and 10 percent third degree of NCA. Clinical classification of the disease according to the staging system of Durie and Salmon and the quantitative staging system of Salmon and Wampler showed some relation to the degree of NCA. The degree of NCA was found to be related to the density of the bone marrow infiltration with myeloma plasmocytes but not to the response to chemotherapy. Some relation was seen to exist between the degree of NCA in myeloma plasmocytes at diagnosis and the prognosis of the disease. The role of electron microscopical analysis in MM diagnostics and its contribution to management of the disease are discussed.

[Primary hemangioendothelioma of the bone marrow].

A case of malignant haemangioendothelioma in a 61-year-old female is described. The tumour infiltrated extensively medullary spaces in a number of bones without their destruction. Extraskeletal neoplastic foci were found in the intestine and in a lymph node, liver and especially spleen were involved more extensively. Several diagnostic features of the tumour are listed together with its position in some recent classification schemes.

Prognostic significance of stratification systems in multiple myeloma. I. Risk categories (good and poor risk).

A group of 193 patients with multiple myeloma (MM) consisting of cases treated only symptomatically or by nonsystematic therapy with Cyclophosphamide or Melphalan (1959-76), and of patients given systematic polychemotherapy with intensive supportive treatment (1976-84) were subjected to prognostic analysis of the importance of MM stratification into two categories, poor and good risk. The evaluation included only hitherto untreated patients with MM of the IgG, IgA, and Bence-Jones' types. All the three evaluated stratification systems (ALGB, NCI-SECSG and CALGB) were found in the present study to show a good, equally significant relation to the prognosis of the disease. The survival median of poor risk patients in terms of the used classification system for the 1959-76 subgroup was 5-6 months, for the 1976-84 subgroup 20-22 months. In the good risk category it was 24-27 months in the 1959-76 subgroup and 47-50 months in the 1976-84 subgroup. Permanent validity of the initial prognosis and in the poor risk category safe coverage of patients with a high risk of early death was proved. Good agreement of the studied stratification systems with the clinical staging system of Durie and Salmon [14] (most of the Stage III patients consisted of poor risk patients) was recorded, which, however, was not the case with the staging system of Merlini, Waldenström and Jayakar [27]. The CALGB system is considered the most suited to the needs of clinical practice.

Prognostic significance of stratification systems in multiple myeloma. II. Clinical staging systems.

A group of 193 multiple myeloma (MM) patients consisting of cases treated only symptomatically (1959-63) or by nonsystematic monotherapy with Cyclophosphamide or Melphalan (1963-76) and of a subgroup given systematic polychemotherapy and intensive supportive treatment (1976-84) were evaluated for the practical applicability and prognostic relevance of three staging systems. The clinical staging system of Durie and Salmon and the quantitative system of Salmon and Wampler have proved in both subgroups of various periods and with different therapeutic approaches to be well applicable in the clinic allowing the patients to be divided into three prognostically different groups according to the size of the tumor mass. Merlini, Waldenström and Jayakar's staging system has likewise shown relationship to prognosis though the patients could be divided only into two prognostically different groups. It is evident that a deeper knowledge of MM requires nowadays a more comprehensive, complex and prognostically more relevant classification system.
