The in vitro biological activity of Lepidium meyenii extracts.

The biological activity of methanolic and aqueous extracts from dehydrated hypocotyls of Lepidium meyenii (Brassicaceae, vernacular name "maca"), was studied on rat hepatocytes and human breast cancer MCF-7 cells. The extracts did not exhibit cytotoxicity in hepatocyte primary cultures up to 10 mg/ml as measured by the MTT viability test, and lactate dehydrogenase (LDH) and aspartate aminotransferase (AST) leakage. Moreover, after 72 h, extracts inhibited LDH and AST leakage from the hepatocytes. When hepatocytes were intoxicated by t-butyl hydroperoxide, neither extract prevented oxidative damage. Both extracts showed weak antioxidant activity in the DPPH radical scavenging test with IC(50) values of 3.46 +/- 0.16 and 0.71 +/- 0.10 mg/ml, for aqueous and methanolic extracts, respectively. Thus, the observed effect on spontaneous enzyme leakage is probably mediated through mechanisms other than antioxidant activity. Both methanolic and aqueous extracts have shown estrogenic activity comparable with that of silymarin in MCF-7 cell line. Maca estrogenicity was exhibited in the range from 100 to 200 mug of extract per ml. The findings in the present study show that maca does not display in vitro hepatotoxicity. In contrast, a slight cytoprotective effect, probably not mediated by antioxidant capacity, was noted. Maca extracts exhibited estrogenic activity comparably to the effect of silymarin in MCF-7 cells.

[Diagnosis and treatment of liver injuries].

AIM: The aim is to specify contemporary diagnostic and therapeutic procedures of liver injuries. METHODOLOGY: The authors present a trial group of 117 patients with various stages of liver injuries hospitalized at the Traumacentrum Surgical Department of the Faculty Hospital in Plzen from 1.1.2000 to 31.9.2005. 61 injured (52.1%) were treated conservatively, 56 (47.9%) surgically, who underwent 87 procedures, in total. RESULTS: 10 polytrauma patients exited (8.5%) from a haemorrhagic shock (group Moore IV-VI). Six subjects (5.1%) had complications on hospitalization. Average hospitalization lasted 18 days (1-69 days). CONCLUSION: The basic diagnostic tools in liver injuries include "bed side" ultrasonography, spiral computer tomography, in more complicated traumas also MRI, angiography or endoscopic retrograde cholangiography. As a treatment strategy, conservative methods prevail in haemodynamically stable patients, regardless of the liver injury stage. The objective of the surgical management is to provide "damage control surgery", i.e. the first procedure is aimed at life-saving, no time-demanding surgical procedures (e.g. liver tamponade), management of hypothermia, haemocoagulation and metabolic acidosis.

Cytoprotectivity of Prunella vulgaris on doxorubicin-treated rat cardiomyocytes.

The cardioprotective effect of Prunella vulgaris ethylacetate fraction (PVEF) and its constituent rosmarinic acid (RA) was evaluated on isolated rat cardiomyocytes subjected to doxorubicin-induced oxidative stress. The cytoprotectivities of PVEF and RA were concentration-dependent in the range of 0.005 to 0.05 mg/ml and the effect of PVEF correlated with the RA content. Dexrazoxan (DE), used as positive control, was less effective than PVEF or RA.

[Open inguinal hernioplasty, comparison of various techniques--a pilot study].

The authors compare inguinal hemioplasties using a classical method, Lichtenstein method and PHS method from the point of view of a surgeon, as well as using subjective assessments by patients. Each group consists of 11 patients. The authors assess the following parameters: use of analgesics, duration of hospitalization, daily routine restriction and pain.

[Role of videomediastinoscopy in diagnostic procedures of pathological conditions in the mediastinum].

Due to recent advances in videotechnology and a worldwide comback of invasive staging methods of the lung cancer, mediastinoscopy has been revived. In the Surgical Clinic of the Faculty Hospital in Plzen, the authors carried out 54 videomagnetoscopic examinations of 53 patients aged 58.1 years on average, in total, during the period from 2000 till 2003. Mostly, in 41 cases (76%) the examination was performed as a part of the lung cancer staging procedure. None of the patients exited in direct correlation with the surgical procedure. Complications, not requiring reoperation, were reported in 2 cases. The lung carcinoma staging findings agreed with the postoperative N disease classification in 86% of the examined cases. Five cases (14%) were underestimated and always the superior lobe carcinomas were concerned, out of which four of them were on the left. At the same time, the non-invasive staging examination using computer tomography (CT) in the mediastinal lung cancer dissemination examination, proved to show a high rate of false-positive findings, when compared to that with videomediastinoscopy. Mediastinoscopy remains the supreme method in determining other pathological affections of the middle mediastinum, if properly indicated. The authors carry out the above miniinvasive examination as a part of the lung cancer staging method in all patients with the CT-confirmed mediastinal lymphonodes enlarged over 1 cm. They consider this method a necessary part of this disease staging protocol, in cases of the left superior lung lobe tumors combined with videothoracoscopy. At the same time the authors recommend it as the method of choice in diagnosing other pathological processes of the middle mediastinum, mainly for its high efficiency rate and its low rates of complications.

Antioxidant status, lipoprotein profile and liver lipids in rats fed on high-cholesterol diet containing currant oil rich in n-3 and n-6 polyunsaturated fatty acids.

Plant-based n-3 polyunsaturated fatty acids (PUFA) possess a prospective antiatherogenic potential. Currant oil from Ribes nigrum L. is one of the few plant oils containing PUFAn-3 (15.3 mol%) in addition to PUFAn-6 (60.5 mol%). This study was aimed at comparing the effects of currant oil with those of lard fat, rich in saturated (43.8 mol%) and monounsaturated (47.0 mol%) fatty acids, on antioxidant parameters, the lipoprotein profile and liver lipids in rats fed on 1 % (w/w) cholesterol diets containing either 10 % of currant oil (COD) or lard fat (LFD). After 3 weeks of feeding, the COD induced a significant decrease in blood glutathione (GSH) and an increase in Cu(2+) induced oxidizability of serum lipids, but did not affect liver GSH and t-butyl hydroperoxide-induced lipoperoxidation of liver microsomes. Although the COD did not cause accumulation of liver triacylglycerols as LFD, the lipoprotein profile (VLDL, LDL, HDL) was not significantly improved after COD. The consumption of PUFAn-3 was reflected in LDL as an increase in eicosapentaenoic and docosahexaenoic acid. These results suggest that currant oil affects positively the lipid metabolism in the liver, above all it does not cause the development of a fatty liver. However, adverse effects of currant oil on the antioxidant status in the blood still remain of concern.

Quaternary benzo[c]phenanthridine alkaloids as inhibitors of dipeptidyl peptidase IV-like activity baring enzymes in human blood plasma and glioma cell lines.

Quaternary benzo[c]phenanthridine alkaloids (QBA), fagaronine (FA), sanguinarine (SA), chelerythrine (CHE) and the QBA extract from Macleya cordata (EX) exerted differential inhibitory effect on the hydrolytic activity of particular dipeptidyl peptidase (DPP)-like enzyme isolated from human blood plasma and from human and rat glioma cell lines. The low-MW form of DPP-IV-like enzyme activity, corresponding most probably to DPP-8, observed only in glioma cells but not in human plasma, was inhibited preferentially by SA, CHE and EX, and only slightly by FA. The alkaloid inhibitory effect was concentration-dependent in the range 25-150 mM and directly pH-related. In addition, a subtle but consistent inhibition of the intermediate-MW form of DPP-IV-like enzyme activity, ascribed to DPP-IV/CD26, observed only in human plasma and of the attractin (high-MW form of DPP-IV-like enzyme activity, expressed in U87 glioma cells) by the studied alkaloids was observed. We conclude that some of the QBA biological effects could be determined by tissue and cell type specific dipeptidyl peptidase IV-like molecules expression pattern.

[Tracheobronchial injuries--experience with diagnosis and therapy].

The incidence of penetrating injuries of the respiratory pathways is permanently low and relatively stable, on the other hand, it may be assumed that there is an increasing rate of tracheobronchial injuries during blunt accidents of the chest in particular in conjunction with the increased rate of traffic injuries. Serious injuries are involved, in particular because of their high morbidity and mortality; frequently they are part of associated injuries and polytraumas. In 1999-2002 at the Surgical Clinic of the Plzen-Lochotín Faculty Hospital a penetrating stab wound of the cervical trachea was treated in an 11-year-old boy and injuries of the large bronchi of adult men resulting from blunt chest injuries during car accidents. All three injuries were associated with a typical symptomatology of pneumocolon and pneumomediastinum, or persistent pneumothorax and respiratory complaints resp. The definite diagnosis was established in two instances shortly after admission, based on results of CT and bronchoscopic examination, in the case of the injury of the middle bronchus only on the following day during surgical revision combined with preoperative bronchoscopy. Treatment was surgical in all instances. Twice the lesion was sutured, in one case pulmonary resection was performed. No significant postoperative complications were observed nor any deaths. The main factors which influence in a decisive way the prognosis of the casualty and the therapeutic result are in the authors' opinion, consistent with data in the literature, early assessment of the diagnosis of tracheobronchial injury and early adequate therapeutic action. In the diagnosis a dominant position is held in addition to CT in particular by treacheobronchoscopy. In case of surgical treatment reconstruction operations are preferred.

Cytotoxicity of natural compounds in hepatocyte cell culture models. The case of quaternary benzo[c]phenanthridine alkaloids.

The quaternary benzo[c]phenanthridine alkaloids (QBA) produce a plethora of species- and tissue-specific effects but the molecular basis of their biological activities remain mysterious. The objective of the present study was to investigate the cytotoxicity of QBA alkaloids, sanguinarine (SA), chelerythrine (CHE), fagaronine (FA), and the extract from Macleaya cordata in primary cultures of human and porcine hepatocytes. The cellular damage was assessed by the MTT assay, lactate dehydrogenase (LDH) leakage and the determination of intracellular glutathione (GSH) levels. The results are summarised as follows: (i) The alkaloids tested in doses 0.1 and 10 microM did not display statistically significant cytotoxicity for 0-3 h incubation; (ii) SA and CHE showed the dose- and time-dependent toxicity within the range 25-100 microM whereas FA was not toxic; (iii) the LDH leakage into the medium was higher for SA than for CHE, thus revealing a potent potential of SA to disturb cell-membrane integrity; (iv) after 3 h incubation with 100 microM SA/CHE, mitochondrial dehydrogenase activity (MTT assay) and the cellular GSH levels decreased to residual values of about 40% suggesting that mitochondria are unlikely to be a primary target for SA/CHE in the cell; (v) no differences were found in the response to QBA application in human vs porcine hepatocyte.

Determination of anthracycline antibiotics doxorubicin and daunorubicin by capillary electrophoresis with UV absorption detection.

Sweeping preconcentration and electrokinetic injection was used for the capillary electrophoretic analysis of trace amounts of biologically active anthracyclines with UV absorption detection. Phosphate buffer (100 mM), pH 2.5, with addition of 40% v/v methanol was used as background electrolyte (BGE). Sodium dodecyl sulfate (150 mM) was added to BGE in the inlet vial as the sweeping agent. The system enables effective separation of anthracyclines as well as cleanup from matrix impurities. Sweeping preconcentration of sample provides an excellent detection limit (1 x 10(-9) mol L(-1)). The method was applied for the determination of therapeutic levels of doxorubicin in real plasma samples.

Pharmacokinetic study of iodine-labeled silibinins in rat.

The very low bioavailability of silibinin (silybin, SB), the main antioxidant flavonolignan of silymarin from Silybum marianum L. (Asteraceae), requires sensitive methods to study the modulation of silibinin bioavailability. To evaluate the potential for use of radiolabeled silibinin, two silibinin derivatives, separated by HPLC after iodination ((125)I-SB(1) and (125)I-SB(2)) and their complexes 1 : 1 with phosphatidylcholine ((125)I-SPC(1) and (125)I-SPC(2)) were administered concurrently with a single intragastric dose of 5.0 mg or 50 mg of unlabeled silibinin (alone or as a constituent of the complex) per kg of body weight in a comparative study of bioavailability in the rat. Pharmacokinetic parameters as well as organ uptake of (125)I-SB(1)-derived radioactivity showed a dose-response pattern. The parameters of bioavailability after (125)I-SPC(1) intake were not influenced by unlabeled silibinin (complexed with phosphatidylcholine), since maximal levels were achieved by the lower dose of unlabeled compound. The superior bioavailability of (125)I-SPC(1) was obvious at the lower dose of unlabeled compound as elevated AUC and RA(max) (maximal percentage of administered radioactivity), and increased radioactivity in liver, kidney, spleen and heart. An absence of these characteristics with (125)I-SB(2) and (125)I-SPC(2) suggests the use of(125)I-SB(1) for studies of modulation of its bioavailability in vivo in rat.

["Silymarin"--an extract from the milk thistle (Silybum marianum)--is it a drug or nutritional supplement?].

Thanks to the new knowledge, there is an increase of interest in plant extracts in the developed countries, but their classification is, however, problematic. It is necessary to clearly define the borderline between a drug or a defined content of an individual active principle, and a nutritional supplement, which is freely on sale. The extract from the seeds of S. marianum of (ESM) shows biological effects corresponding to nutritional supplements, and it is therefore more logical to class ESM with this group and to investigate the pharmacological effects in its chemically defined components.

[Stromal tumor of the small intestine--a cause of massive gastrointestinal hemorrhage].

A 41-year-old man attended our clinic with massive gastrointestinal bleeding due to a tumor of the small intestine. The tumor was not diagnosed until during acute laparotomy. The histological examination revealed a gastrointestinal stromal tumor with uncertain biological behaviour. The authors discuss the rare gastrointestinal stromal tumors, where preoperative diagnosis is difficult and in symptomatic forms urgent laparotomy is usually the only diagnostic method. The differential diagnosis between leiomyoma, schwannoma, mesenterial fibromatosis or intraabdominal solitary fibrous tumor as well as metastatic melanoma is not simple and immunohistochemistry plays a leading role. Prediction of biological behaviour of these tumors is also problematic and depends on the histological picture, site, size and mitotic activity.

[Injury of the rectum with a porcelain cup].

The authors present the case-history of a 33-year-old man who injured his rectum by a foreign body. He attended treatment only after decubital necrosis of the intestinal wall and development of peritonitis. Extraction of the foreign body from the rectum was performed and treatment of the abdominal cavity, drainage with sigmoideostomy which was submerged in the second stage. The patient is 9 months after the second operation and does not suffer from any disorders as regards defaecation.

[Carcinoid of the ileocecal junction with symptoms of acute abdomen].

The authors describe a group of six patients (two men and four women) operated at the Surgical Clinic in Plzen during 1995-2000 on account of a carcinoid of the GIT. The mean age of the patients was 48.6 years, the range 14-77 years. In five instances a carcinoid of the appendix was detected. In a separate case-record they evaluate a carcinoid of the ileocaecal transition with symptoms of acute abdomen, early surgery, its extent incl. the postoperative procedure.

Determination of total antioxidant capacity in plasma by cyclic voltammetry. Two case reports.

The cyclic voltammetry (CV) was used for the measurement of the plasma total antioxidant capacity from two types of patients. The first one consisted of 29 volunteers (men aged 18-21 years) who were administered placebo or silymarin at a dose of 858 mg/day. After two months of silymarine administration, CV revealed a statistically significant increase in total antioxidant capacity compared to placebo. No statistically significant changes in TBARS, SH-groups, creatininin, urea, and uric acid concentrations were found. The second group under study comprised 49 patients with chronic renal disease during dialysis therapy. After dialysis, CV revealed a decrease of total antioxidant capacity in the plasma, which was equivalent to a decrease in creatinine, urea and uric acid. CV was performed using a system consisting of a working glassy carbon electrode, an auxiliary platinum electrode, and a reference saturated calomel electrode; a linear change of voltage of 200 mV/s was applied. CV is a simple and relatively reliable method for assessment of body antioxidant status. It is also time and cost effective.

Capillary electrophoretic determination of sanguinarine and chelerythrine in plant extracts and pharmaceutical preparations.

Capillary electrophoresis was employed to determine the principal quaternary benzo[c]phenanthridine alkaloids, sanguinarine and chelerythrine, in two plant extracts and one oral hygiene product. Phosphate-Tris buffer of pH 2.5 was used as a background electrolyte, limits of detection were 3 micromol/l(-1) (sanguinarine) and 2.4 micromol,l(-1) (chelerythrine) using UV detection at 270 nm. The method, which correlated well with HPLC, is suitable for serial determination of sanguinarine and chelerythrine in plant products and pharmaceuticals.
