Epidurual pneumatosis: a benign complication of benign pneumomediastinum.

Subcutaneous emphysema and pneumomediastinum are well-known complications of barotrauma. There are no fascial barriers between the posterior mediastinum and the retropharyngeal and epidural spaces; thus, air can diffuse freely to the epidural space and produce an epidural pneumatosis. We report a case of epidural pneumatosis caused by a pneumomediastinum following nasotracheal intubation for an attempted suicide. This benign complication of air leakage was easily recognized on computed tomography (CT) scan.

[Audiometric results of 81 ossiculoplasties after tympanoplasty with closed technique in chronic cholesteatomatous otitis].

We report our functional results about 81 ossiculoplastys during tympanoplasty with closed technique in chronic cholesteatomatous otitis. We discuss our functional results and the post-operative assessment of the audiometric gain. Patients presented either cholesteatomatous otitis sequelea or any preliminary status (retraction pockets) with attical impairment.

X-linked progressive mixed deafness: a new microdeletion that involves a more proximal region in Xq21.

We report a large two-generation pedigree with seven affected males segregating for an X-linked mixed conductive sensorineural deafness. The patients present with atypical Mondini-like dysplasia, dilated petrous facial canal, dilatation of the internal auditory meatus fully connected with enlarged cochlear canals, and, in one patient, a wide bulbous posterior labyrinth. Obligatory carrier females are mildly affected. Molecular characterization of this family revealed a deletion of locus DXS169, in Xq21.1. Loci DXS72 and DXS26, which, respectively, flank DXS169 proximally and distally, were intact. Since a gene responsible for X-linked progressive mixed deafness with perilymphatic gusher (DFN3) has previously been assigned by deletion mapping to a slightly more distal interval between DXS26 and DXS121, this study indicates either two different deafness genes or the involvement of a very large region in Xq21.

[The attic. Anatomoclinical correlation].

Among the hidden parts of the middle ear, the posterior part of the atrium undoubtedly raises the most interest of otologists. The attic is a frequent localization of cholesteatoma and retraction pouches. Its anatomy is not well known, especially the anterior and internal walls. This anatomic study, based on the dissection of 41 petrus bones was designed to determine the anatomic limits and describe the "noble structures" of this region in order to "guide" future surgical procedures.

[Recurrence of nasosinusal polyposis after ethmoidectomy by endonasal approach. Functional, endoscopic, x-ray tomographic aspects and surgical implications].

Recurrent polyposis after 116 endonasal ethmoidectomies performed in 61 patients were investigated on the basis of functional, endoscopic and tomodensitometric data. The results of the endoscopic examinations revealed that the anterior ethmoid was involved most often (41%) with either a single localization or in combination with other sites in the sinuses. Functional rhinosinus symptomatology was satisfactory in most cases after a mean follow-up of 22 months, especially for nasal obstruction which was initially predominant (91%). Headaches, especially fronto-orbial localizations, clearly decreased after the operation but there was no correlation between the presence of headache after the operation and the recurrence of the polyposis. Computed tomography gave results similar to those obtained by endoscopy. However, a distinction could not be made between radio-opaque images of polyposis and certain cicatricial or inflammatory reactions. Unlike the functional outcome, ethmoidectomy had little effect on these images. Recurrent polyps appeared most often on the anterior ethmoid and the role of the initial infundibulotomy can be debated. It would appear that the prognosis of polyposis is not modified by extended anterior ethmoidectomy, suggesting that a more conservative surgical approach may be appropriate for frontal ethomoidal polyps.

[Primary carcinoid of the larynx. Apropos of a case. Review of the literature].

We report a new case of primary atypical carcinoid of the larynx. This rare neuro-endocrine neoplasm of the larynx was initially mis diagnosed in 50% of the cases published. Since, characteristically these are lesions of the epilarynx causing dysphonia at early stages of development, the diagnosis should be kept in mind when the work-up for dysphonia reveals a small tumor of the epiglottis or aryepiglottic folds. The histological diagnosis should be oriented by Grimelius staining and confirmed immunoperoxidase marker studies for chromogranin and cytokeratin. Although, atypical carcinoids of the larynx are aggressive malignant tumors, the prognosis for early lesions of less than 1 cm is excellent. However, unlike early squamous cell carcinomas, these tumors do not respond well to radiation therapy. Primary surgery is the treatment of choice.

[Amyloidosis of the nasopharynx. Apropos of a case].

Rhinopharyngeal localization of amyloidosis is particularly rare. The authors report a case of isolated amyloidosis of the cavum, and provide a brief summary of the amyloid substances and other localizations of this pathology.

[Dysosmia and dysgeusia in the elderly].

A study of dysosmia and dysgeusia in the elderly was undertaken in 50 patients, according to a simplified protocol based on an interview, a complete ENT examination, chemical gustrometry and recognition olfactometry. As far as olfaction was concerned, 11% of the patients interviewed said they were hyposmic, whereas following the olfactometric examination, hyposmia was found in 25% of the patients. As far as gustation was concerned, 5% of the patients complained of parageusia. The chemical gustrometric examination revealed 4% of cases of hypogeusia. As a conclusion to this study, it appears that olfactory and gustatory disorders are well tolerated in the elderly, and are not a cause for referral. The recognition olfactometric examination permits the detection of hyposmia with a relatively remarkable frequency.

[Use of the free forearm flaps in cervicofacial surgery. Apropos of 5 cases and review of the literature].

The free fasciocutaneous forearm flap has been described only recently. Five cases of pharyngeal or oral repair with a forearm flap (FAF) are reported and used as a basis to discuss anatomical features and the technique used for removal of the flap. The relevant literature is reviewed. The FAF appears to be the flap of choice for reparative surgery of pharynx or mouth regions in oncology, the principal advantages of this flap being: fineness, flexibility and plasticity of the skin, reliability and lack of morbidity, and function effectiveness with respect to phonation and feeding. Indications for forearm and myocutaneous flaps in oncology of upper respiratory and digestive tracts are assessed critically, other indications for the forearm flap being cutaneous repair of cervicofacial lesions due to tumor or injury. The forearm flap is of pronounced value for cervicofacial repair surgery and constitutes a new step in the plastic conception of these repair procedures.

[Critical study and role of partial vertical reconstructive laryngectomies with epiglottoplasty by the Tucker method. Apropos of 18 cases].

Two groups could be distinguished in a series of 18 patients with glottic lesions: one with pure glottic tumors (12 cases) and the other with tumors extending beyond the glottic region (6 cases). All patients but one were operated upon by first intention for epidermoid carcinoma, but also for associated pronounced dysplasia. The so-called Tucker technique was used with slight modifications (less extensive resection of thyroid cartilage, conservation of the two arytenoid cartilages). The postoperative course was satisfactory with decanulation and ablation on the 13th day, and a mean hospital stay of 22 days. Two postoperative deaths due to vascular complications occurred and one patient developed a swinging epiglottis after 2 years. The remaining 16 patients are all alive, including 8 after less than 3 years. Analogous results were noted in an analysis of a total of 93 case-reports from 5 different series of patients. This method provides an elegant and satisfactory repair of pure cordal surgery, but adds nothing from the oncologic point of view since other better adapted and currently well defined operations have to be envisaged for lesions extending beyond this region.

[Relations of sinusitis and primary glomerulonephritis].

Certain types of glomerulonephritis develop typically as the result of focal infection, and may recover following effective treatment of the infective focus at cause. Apart from the classical immunity mechanism consisting of the formation of immune complexes with the bacterial antigen, a major role for environmental allergens must also be evoked, as well as the probable existence of different immunity mechanisms at the origin of certain types of GN (mainly mesangial GN with IgA deposits and lipoid nephrosis). Invasion of the pulmonary allergen through a rhinosinusal route is responsible for a rhinosinusitis demonstrating a typical clinical features and course. Progression of the rhinosinusitis runs parallel with that of the GN. The course of the GN may be improved after treatment of the sinusitis and the allergic disposition. Ten particularly significant case-reports of GN associated with rhinosinusitis are studied from various aspects.

[Use of the pectoralis major myocutaneous flap in transmaxillary buccopharyngectomy].

Pectoralis major musculocutaneous flaps were used for reconstruction after transmaxillary buccopharyngectomy to treat 22 patients with large oropharyngeal tumors. Short and medium term results are discussed. Large excisions such as these raise many functional problems, even though the use of a musculocutaneous flap for reconstruction is a valid procedure from the anatomical point of view. The cancer prognosis must still be reserved as follow up is still limited for these cases.

[Use of pectoralis major myocutaneous flaps in cervicofacial surgery. Our experience with 46 cases].

Musculocutaneous flaps from pectoralis major were used during cervico-facial surgery for repair after cancer operations in 46 patients. Use of this technique in surgical practice is discussed as a function of the site of cancer, and results described in relation to the viability of the flap and the outcome. Removal of the flap is simple and possible by a one-stage operation, and the method provides functional improvement for these patients.

[Post-traumatic optochiasmatic syndrome].

A case of post-traumatic opto-chiasmatic syndrome is described, and the principal characteristics of the affection outlines. This rare syndrome complicates severe cranial injuries, and is simple to diagnose when confronted with a patient with bitemporal hemianopsia frequently associated with diabetes insipidus or paralysis of certain cranial nerves. Suggested mechanisms for the lesion include sagittal stretching of the chiasma or an ischemic focus.

[Results of several times daily treatment by cisdiamine-platinum and bleomycin. Therapeutical considerations (author's transl)].

Seventy two head and neck cancers have been treated with a combination of Cis-DDP (20 mg/m2/day) and bleomycin (6 mg/m2/day, 24 hours infusion) given for 5 days 3 courses every 3 weeks in preoperative cases and for 6 days (4 courses every 3 weeks) in non preoperative cases. Fifteen patients treated for local recurrence after irradiation experienced no significant response and their actual survival at 12 months is 22%. For the other patients the objective response rate was 78% with 10 complete responses and the projected survival at 12 months varies from 62% to 92% according to the subsequent treatment (irradiation or surgery followed by irradiation). These results are in agreement with a few reports using similar combinations; but it needs to answer new questions about possibility of surgical strategy variations in the cases of good or complete responses with such a chemotherapy.
