[Foreign bodies of gastrointestinal tract].

153 patients with swallowed foreign bodies in gastrointestinal tract (GIT) were treated for the last 10 years (1988-1997). In 85 (55.5%) cases foreign bodies (FB) were swallowed by psychopathic persons, in 45 (29.4%)--with the aim of mutilation, in 23 (15.0%)--due to carelessness. In 105 (68.63%) patients the foreign bodies were single, in 48 (31.37%) there were multiple foreign bodies of GIT. In 73 (47.71%) cases FB were found in the stomach. Clinical picture in patients with swallowed FB was variable. Dynamic roentgenological examination together with thorough anamnesis is the most important diagnostic tool in this pathology. The indications for urgent surgical treatment were sticking of the FB, peritonitis, gastro-duodenal bleeding, gastrointestinal obstruction. Elective surgery was used in patients with sticked F.B., with multiple FB, which formed conglomerates, as well as in single FB. with the length over 12-15 cm. Active expectant policy is valid FB of GIT with tendency for passage, due to conservative treatment.

[The use of contact impulse-periodic YAG-ND laser in surgery of major duodenal papilla].

YAG-Nd impulse-periodic laser scalpel in elimination of major duodenal papilla stenosis was tested experimentally. The direct damaging influence of the laser was tested in acute experiments and the optimal power of irradiation (5 W) was chosen. The processes of regeneration of the wound inflicted by laser irradiation were tested in chronic experiments. It is shown, that regeneration was accompanied with minimal leucocyte infiltration and complete healing of the wound is achieved in 28 days.

[Use of polypropylene single thread as the suture material].

The authors studied the results of using polypropylene monothread as suture material in 254 patients who underwent planned and emergency operations for various diseases. It was found that polypropylene monothread used for suturing the skin, deeper located tissues, granulating wounds is sufficiently elastic, acapillary, strong, and biologically inert owing to which there is no need to remove it until complete healing of the wound even in considerable exposition. The monothread causes a less traumatizing effect than does a thread of natural silk when drawn through the tissues. A blue or green color of the monothread makes it visible during the operation.

[Experience with and the prospects of using a single polypropylene thread in surgery of suppurative wounds].

A polypropylene monofilament in placing secondary sutures causes minimal damages in the tissues. The absence in a monofilament of the capillary-filtrative properties prevents the development of infiltrate, suppuration, dehiscence of the wound edges. In using a modified suture of the granulating wound, suggested by the authors, a good cosmetic and functional effect was noted.

[Immobilized papain in the treatment of acute destructive lactation mastitis].

Experience with the surgical treatment of 123 patients has shown the positive effect of proteolytic enzyme Papain immobilized on fiber polymer materials. It shortened the period of purification of purulent wounds of the lactating mammary gland from devitalized tissues which facilitated putting early secondary sutures on the granulating wound without a dissection of wound margins and resulted in quicker healing (13,5 days).
