The in vivo effects of cyanide and its antidotes on rat brain cytochrome oxidase activity.

The in vivo effects of sodium cyanide and its antidotes, sodium nitrite, sodium thiosulfate and 4-dimethylaminophenol (DMAP), as well as the alpha-adrenergic blocking agent phentolamine, on rat brain cytochrome oxidase were studied. The course of inhibition was time-dependent and a peak of 40% was attained between 15 and 20 min after the s.c. injection of 1.3 LD50 (12 mg/kg) of cyanide. Pronounced dose-dependence was observed in the inhibition of the enzyme, at this relatively low, but lethal dose. Further observation was impossible because of rapidly lethal effects of cyanide. In animals artificially ventilated with room air, observation was possible up to 60 min. However, maximum inhibition was also 40%. When antidotes were applied 30 min after 20 mg/kg of cyanide, marked reactivation of cytochrome oxidase activity was observed with all antidotes (particularly with thiosulfate) except for phentolamine which had no effect. Prevention of methemoglobin forming with toluidine blue did not affect the reactivating ability of nitrite or DMAP, thus suggesting more complex protective mechanisms then simple methemoglobin formation. The high efficacy of thiosulfate may be attributed to its rhodanese catalyzed, direct binding to free blood cyanide, leading thus to its dissociation from cytochrome oxidase. The theory that cytochrome oxidase inhibition is a basic mechanism of cyanide toxicity could not be disproved.

[Pregnancy and labor associated with encephalopathy in neonates during the early neonatal period].

Pregnancy complications, drugs and surgical interventions during pregnancy, fetal growth, medications and interventions during labor, labor complications as well as fetal heart activity during labor in a group of 114 term infants without malformations, but with signs of central nervous system (CNS) damage throughout early neonatal period are compared with paired group of term healthy infants born in the same presentation and mode of delivery. Among prelabor factors only maternal hypertension (found in 16.7% of encephalopathy children versus 0.8% in a control group) was significantly correlated with CNS damage. Fetal growth retardation and long term ritodrine administration were found more frequent in encephalopathy than in healthy group of infants, although statistical significance between the groups could not be demonstrated. A prolonged second stage of labor, high oxytocin dosage, too frequent uterine contractions and vacuum extractions were found significantly correlated with neonatal encephalopathy. CTG pattern during labor was normal in only 28.9% of children, with encepalopathy prepathologic in 46.4% and pathologic in 24.7%. The respective percentages for healthy newborns were: 82.5%, 16.25% and 1.2%. All differences between the groups were statistically significant. Mean duration of prepathologic CTG score in the group of infants with encephalopathy (78.8 minutes) as well as of pathologic score (51.7 minutes) was significantly longer than in healthy infants (23.7 minutes prepathologic and 7 minutes pathologic).(ABSTRACT TRUNCATED AT 250 WORDS)

[Efficacy of alcoholates and clay in percutaneous contamination of rats with highly toxic organophosphorus compounds].

Efficacy of decontamination by alcoholate and clay has been examined in animals percutaneously poisoned by nerve war gases (VX, soman) and organophosphorous insecticides (parathion, dichlorvos, terbufos) at exposure of 2, 5 and 60 minutes. High efficacy was achieved after two minutes using alcoholates which, except for VX, was lower for 50% after 5 minutes. The similar results were also achieved with clay in nerve war gas poisoning, however, the efficacy of this material was somewhat lower. By delayed decontamination (after 60 minutes) in animals poisoned by parathion and terbufos somewhat lower but still satisfying efficacy was achieved (ZI = 3.3, that is, 11.5). The results obtained have shown that efficacy of decontamination, except for the used materials, depends also on duration of the procedure.

[Pregnancy outcome after early amniocentesis].

The indications, karyotype, and outcome of 1812 pregnancies following midtrimester amniocentesis (eACZ/as) performed from 1977 to 1989 are presented, especially regarding the risk of pathologic karyotypes and spontaneous abortion. There were 37 pathologic karyotypes (trisomy 21 in 24 pregnancies). The spontaneous abortion rate following amniocentesis was 2.8% although it was almost halved after direct ultrasound guidance had been introduced (1.87% versus 3.66%). In gravidas with previous spontaneous abortion, the preterm delivery rate and spontaneous abortion rates following rACZ were significantly increased. There is a progressive increase of the spontaneous abortion rate after eACZ with the number of previous spontaneous abortions; 5.83% when there was only 1 previous spontaneous abortion, 7.83% when there were 2, and 8.75% when there were 3 or more spontaneous abortions. Previous spontaneous abortions do not increase the risk of chromosomal aberration. In women aged 37 years or more the risk of trisomy 21 is 0.69%, aged 38 years 1.55% and over 39 years 0.44%. The risk is rapidly increasing after 40 years of age (2.25%). The proportion of pathologic karyotypes in these age groups was significantly higher in comparison to younger women. The age limit of 37-38 years as the indication of a possible appearance of mid-trimester ACZ is discussed.

[The effect of cyanides and antidotes on the heart rate in rat heart preparations in situ].

Effects of sodium-cyanide and some of its antidotes and antagonists such as sodium-nitrite, 4-dimethylaminophenol (DMAP), sodium-thiosulphate and alpha-adrenergic blocking agents on the heart rate of rats in situ under conditions of the artificial breathing have been studied. Also, effects of opioid receptor agonist, naloxane, on the heart of the poisoned animals have been studies. Sodium-cyanide causes strong and irreversible effects on the heart rate which are completely abolished by effects of sodium-nitrite, 4-DMAP and sodium-thiosulphate, and partly and timely limited by effects of alpha adrenergic blockers, phenotholamine and promethazine, and to a significantly slower degree by naloxane.

[The effect of sodium cyanide and its antidotes on the isolated perfused rat heart].

The experiments were performed to investigate the effects of cyanide and several antidotes (sodium nitrite, DMAP, sodium thiosulfate and phentolamine) on isolated perfused rat heart to further elucidate the mechanism of action of cyanide and its antidotes. Sodium cyanide (40, 50 and 60 mg/ml) produced maximum effects on amplitude, heart rate and coronary flow, after 3 min, regardless of the event recorded. Some dose-dependence was noted, especially on the heart rate, somewhat less on amplitude and almost none on the coronary flow up to 15 min, but notable afterwards. Some spontaneous recovery occurred in each event, the most significant being that of the heart rate. Antidotes per se did not differ significantly in their action on isolated rat heart. All values, with some variations, were maintained within 10-20% of control. Antidotes, injected 1 min after sodium cyanide produced the most evident effects on amplitude and heart rate -- all antidotes practically and almost instantly neutralized the depressant effect of cyanide. Nitrite and particularly DMAP, were the most effective on coronary flow while other antidotes, although effective up to 3 min, did not much differ from cyanide alone throughout the rest of observation.

[Decontamination with clay or alcoholate of pigs percutaneously poisoned with VX and soman].

The efficacy of clay or alcoholate as decontaminants in pigs percutaneously poisoned with 6 LD50 of O-ethyl S-2-diisopropylaminoethyl methylphosphonothioate (VX) and 3 LD50 of 1,2,2-trimethylpropyl methylphosphonofluoridate (soman) nerve gases was tested. It was assessed by the time of onset of the first signs of poisoning and death, as well as by the activity of blood cholinesterase (ChE). No toxic signs or fatalities were observed in decontaminated pigs, regardless of the decontaminant used. In VX poisoning up to 240 min. both decontaminants kept ChE values at normal level. Twenty four hours later, ChE activity in pigs decontaminated with clay was 71%, significantly higher than in pigs decontaminated with alcoholate (49%). In soman poisoning the activity in control group was maintained at almost normal level up to 60 min, followed by rapid fall to 58%. Further readings were impossible due to the death of all animals. No significant difference between decontaminants could be noticed throughout the observation of 24 hr. The values were kept between 80 and 100%, with the trend of rising after 120 min.

[Aspiration bronchopneumonia as a complication of acute poisoning with psychotropic drugs].

In the five year period, retrospectively and prospectively, the frequency, clinical, radiographic and bacteriological characteristics of the aspiration bronchopneumonia (ABPN) were followed in the acute poisoning by psychotropic drugs (PD). In 1769 patients with acute poisoning by PD, ABPN was determined in 44 (2.49%) patients, and most frequently in groups with polymedicamentous (5.99%) and neuroleptic (5.17%) poisoning, and rarely in the group poisoned by anxiolytics (0.77%). Severest poisonings by PD were complicated by ABPN in 16.84% of cases. High conformity of clinical and radiographic finding of bronchopneumonia was achieved. The diagnosis of bronchopneumonia was established on the day of admission at the Clinic in 81.9% of cases on the other day in 13.6% and on the third day in 4.5% of patients. Bacteriological examination of sputum revealed the pathogens in 63.6% of patients, but in no patients the anaerobic bacteria were isolated. The treatment was very complex, and beside the detoxification measures, it was necessary to remove the aspirated contents from the respiratory tract (usually during endotracheal intubation or therapeutic bronchoscopy) and immediately apply antibiotics. The examinees were hospitalized twice longer than the patients whose course of poisoning by PD was not complicated by ABPN. The poisoning ended lethally in two (4.5%) patients. In no cases the ABPN was the immediate cause of death, but it significantly contributed to the lethal outcome.

[Radioprotective agents in the decontamination of rats poisoned with percutaneously administered mustard gas].

The efficacy of rat skin decontamination by radioprotective agents AET and cystamine were tested. Results were compared to standard decontaminants-alcoholate, clay and fuller's earth. It was found that all the animals decontaminated with standard decontaminants had survived 4 LD50 of poison for 21 days. All the animals poisoned by 6 LD50 had survived for 1 day except for those decontaminated with alcoholate where 50% survived throughout the observation period. Alcoholate and clay protected all, and fuller's earth 70% of animals poisoned by 12 LD50, but no longer than 24 h. AET and cystamine protected all rats from 1.7 LD50 for 3 days, from 1 LD50 for 2 days, and from 2 LD50 for 1 day. When higher doses of poison were used (1.7-6.8 LD50) only partial protection was achieved. Our results support the conclusion that radioprotective agents could be convenient means for improvised decontamination of persons poisoned by sulfur mustard, so additional investigations are necessary.
