Microvascular anastomosis using carbon dioxide (CO2) laser: evaluation by SEM after corrosion cast.

In order to compare the carbon dioxide laser-assisted microvascular anastomosis (CO2 LAMA) with conventional microvascular sutured anastomosis (CMSA), 40 microarterial anastomoses were performed in Wistar albino rats. At different time intervals from zero to four weeks after the procedure, the anastomoses were examined under the scanning electron microscope (SEM) after resin corrosion cast (Mercox). CO2 LAMA was easier and less time-consuming than CMSA, with the same patency rate. Healing of the lumen surface was similar in both procedures, suggesting that CO2 LAMA can be reliably used in microvascular anastomosis.

Spherocylindric (Luck) cup arthroplasty for osteonecrosis of the hip.

Osteonecrosis (ON) in young adults is a serious condition causing pain and functional disability. Thirty-eight hips with ON were treated, beginning in 1972, with a spherocylindric cup (SCC) derived from the original Luck. The mean follow-up time was six years, 11 months (minimum, one year; maximum, 12 years). The overall results, using the Merle d'Aubigne grading system, were excellent in four, very good in nine, good in nine, fair in one, and failure in seven. No acetabular protrusio was observed. The seven failures were associated with deep infection in one case and unsatisfactory technique in three. One failure was unexplained, except that the ON was Grade IV. Two failures occurred in one patient with predisposing factors, such as hyperlipidemia, hyperuricemia, and exogenous hypercortisonism. Although the results are not as satisfactory as those of total hip arthroplasty (THA) with respect to relief of persistent pain, some patients remain stable with time even after more than ten years. With correct indications and good surgical technique, SCC arthroplasty is a justifiable alternative to THA in young adults.

[Total hip and knee arthroplasties in hemophilia. Apropos of 27 cases].

Thirteen total hip and 14 total knee prostheses were performed in 21 haemophilic patients. Nineteen had severe factor deficiency. Their average age was 43 years. Pain was the only indication for hip prostheses. All the hip arthropathies were of Arnold stage IV or V. For the knees, the indication was permanent pain with patello-femoral involvement in nine cases. In other cases, stiffness and instability was the indication. Seven knees, one out of two, had a flexion deformity of more than 30 degrees and only in four knees, one in three, was the range of flexion greater than 90 degrees. The quadriceps was extremely atrophied in eight knees and was always fibrotic. The arthropathies were of stage IV in three cases and stage V in eleven cases. There was pre-operative valgus deformity, greater than 20 degrees in three cases. At the hip, all 13 total prostheses were cemented. The 11 patients had factor replacement for an average of 21 days. No inhibitor was present pre-operatively or developed after operation. Early complications were rare: two moderate hematomata of the hip and one haematoma of the homolateral psoas. The late complications were not unusual: two aseptic bipolar loosenings after eight and nine years. At long-term follow-up, with a mean of four years and one month, 77 per cent of the hips were painless and stable with a good range of movement. Eight per cent were painless and stable but had some limitation of flexion. Fifteen per cent were failures. At the knee, a Guepar I knee was inserted in four cases, a Total Condylar knee in eight cases and a Kali knee in two cases. All the patients had factor replacement for 24 days. No post-operative inhibitor developed. The early complications were three superficial skin necroses, four deep hematomata and one transient common peroneal nerve palsy. The late complications were one permanent dislocation of the patella and one bipolar septic loosening with septic pyaemia which needed an arthrodesis and required 79,290 International Units of factor VIII, 72 units of packed cells and 28 bottles of fresh plasma. After a mean follow-up of two years and five months, five knee prostheses had a very good results, four a fair result and five a bad result.

[Modern technics for studying injuries of the lumbo-ischiatic plexus. Apropos of a new case].

A 22 year old patient was admitted with multiple lumbosacral root meningoceles following a motorbike accident. Because of the marked neurologic deficit in left lower limb a radiculographic exploration was completed by a CT scan, with improvement in analysis of images, post-traumatic pseudo-meningoceles being neither an absolute proof nor the unique lesion responsible for the clinical picture. They may be missing whereas the root is absent. Combining these two examinations provides for total screening of lesions and more selective choice of therapy.

[Appearance of the normal knee by MRI using a surface antenna].

NMR imaging of the knee using a surface aerial provides remarkable anatomical detail of this joint without the need for invasive procedures or stressful positioning. Results of the use of this technique on seven normal knees confirmed the precise anatomical details provided, with direct visualization of the femorotibial interline and particularly of the cruciate ligaments, menisci and cartilages. This method allows a new diagnostic approach to the knee, especially for traumatic or tumoral lesions, and greater efficacy of treatment.

[Treatment of acute pyogenic arthritis of major joints of the limbs].

The authors have treated 52 cases of septic arthritis of major joints. When aspiration contain an opaque fluid, conservative treatment with antibiotic therapy and immobilisation obtained healing in half of the patients. Surgical lavage of the joint was needed on 21 occasions with healing in 15. Synovectomy was performed in 8 cases and was indicated only at the stage of pyogenic synovitis before the development of pyogenic arthritis. When the latter stage was reached, arthrodesis was indicated. It was performed primarily on 6 occasions and secondarily once. Seven patients died. The average age was seventy years. The importance of early treatment is stressed.

[Treatment of acute pyogenic arthritis of large joints of the limbs. Apropos of 79 cases].

In reference to 79 cases of primary acute pyogenic arthritis involving large joints of the extremities, the authors analyze respectively the results of medical and surgical treatments. After tapping, the medical treatment combining antibiotics and cast immobilisation, has enabled recovery in more than one out of two cases with "restitutio ad integrum" of the joint. In the absence of rapid improvement, an arthrotomy, performed in 23 instances, has enabled, with irrigation and drainage of the joint, to control in a large number of cases, the infectious process. At the stage of purulent synovitis, a synovectomy was performed in 14 instances, at the price of a frequent decrease of the subsequent mobility. At the osteoarthritis stage, resection or arthrodesis were necessary (10 times) in order to obtain drying of the lesions. All things considered, 50 out of 79 joints recovered completely. We will stress the frequency of iatrogenic etiological factors (16 cases) and the immediate mortality (13 deaths). Finally, at the knee, arthroscopy (12 cases) enables to properly visualize the lesions and an effective irrigation-drainage, while decreasing immediate mortality and subsequent stiffening of the joint.
