[The importance of electrogastrography in clinical practice].

Changes were analyzed in electrogastrogrammes (EGG) of 135 patients who had underwent a surgical treatment for gastric and duodenal ulcer. Time of restoration of bioelectrical activity of the stomach was determined, with diagnostic and prognostic value of EGG during the process of rehabilitation being shown. Changes in EGG were studied in 92 patients with acute appendicitis. Efficiency of the above procedure was established with respect to obtaining supplementary objective data of informative value on the course of the pathological process in the abdominal cavity, which fact permits the prognostication of the different complications development. Transcutaneous electrostimulation of the stomach was shown to be helpful in gastroenterological patients as was intracavitary pneumomassage.

Appendicitis↗

[Intensity of lipid peroxidation in the small intestinal wall in peritonitis and its correction].

Experiments were conducted on albino rats to study lipid peroxidation (LPO) in muscle homogenates of the small intestine in peritonitis of various duration. LPO increased most significantly in 24-hour peritonitis. There was no statistically significant difference between the LPO values two days later and those of 24-hour peritonitis. With the use of the antioxidant alpha-tocopherol with essential 12 hours after the development of peritonitis no significant changes occurred in the LPO values. In 24-hour peritonitis LPO was initiated but it was less pronounced. alpha-Tocopherol acetate and essential were included in the complex of therapeutic measures in 20 patients. They were administered before and 3-4 days after the operation. As a result the motor-evacuation function of the gastrointestinal tract was restored in a shorter time. The postoperative period took a favorable course, and the duration of in-hospital treatment reduced by 2-3 days on the average.

[The importance of x-ray and endoscopic studies of the stomach in the complex examination of patients following resections and vagotomies].

X-ray and endoscopic examinations were made in the course of follow-up of patients surgically treated for gastroduodenal ulcer (n = 134). X-ray investigations of postoperative stomach make it possible to assess it functionally, to determine its relations with other parts of gastrointestinal tract. Endoscopy is helpful in revealing changes in gastric mucosa, anastomosis capable of producing diverse motor and evacuatory disturbances of the stomach. The methods are recommended in postoperative follow-up as these can identify early abnormalities and give information for relevant correction. The correction made by the authors resulted in a decrease in temporary disability by 2-3 weeks and in unfavorable outcomes from 17.2 to 4.3%.

[The mechanisms of the evacuatory function of the gastric stump after a Hofmeister-Finsterer resection].

Attention is drawn to the fact that evacuation from the gastric stump is determined not only by the size of the anastomosis, the manner by which it is formed, whether transversely or longitudinally but also by the functional condition of the gastric stump and the efferent intestinal loop. Motor-evacuation disorders on the part of the stump and efferent intestinal loop precede the development of most postgastric resection syndromes. The authors examined 197 patients who underwent Hofmeister-Finsterer gastrectomy. Five types of evacuation from the gastric stump were distinguished, to which various combinations of motor function of the stump and efferent intestinal loop corresponded. After studying the type of combinations of the motor function of the stomach and efferent intestinal loop, the authors conducted purposeful correction which promoted normalization of the motor function of these organs. For instance, evacuation in portions, which is most efficient in respect of function, was encountered in 42% of patients without correction and in 75.9% of those in whom correction was undertaken. This allowed postoperative functional complications to be reduced from 38% to 12.08% and the adaptation period to be shortened by 2-3 weeks on the average.

[Functional state of the stomach after vagotomy and the correction of various post-vagotomy complications].

The author studied the secretory and motor-emptying function and the endoscopic picture of the stomach after various types of vagotomy conducted for peptic ulcer in 84 patients. The effect of the motor-evacuative function on the secretion values is shown. It is pointed out that secretion must be studied after restoration of adequate motor-emptying function, which is normalized 3-6 months after the operation. The endoscopic picture of the stomach depends on the type of the auxiliary operation on the stomach and the lapse of time after the operation, and the existence of reflux. The author suggests a differentiated complex of rehabilitation therapy including endogenous intracavitary pneumomassage of the stomach, its percutaneous electrostimulation, and the intake of mildly-mineralized water; early application of these measures raises the efficacy of the surgical treatment.

[Functional state of the stomach after gastrectomy and vagotomy and the ways of improving the results of surgical treatment of peptic ulcer].

A complex examination of 286 patients was performed after gastric resection by the method of Hofmeister-Finsterer, 64 patients after truncal vagotomy with antrum-resection, 14--after selective proximal vagotomy with draining operations and 26--without draining operation. Time for the recovery of motor-evacuatory function of the stomach after organ-preserving operations was considerably shorter than after resection of the stomach. In order to shorten the period of adaptation of the digestion organs after operative treatment of ulcer disease a complex of rehabilitating measures is proposed including percutaneous electrostimulation, diet, balneological and medicamentous therapy. It allowed the terms of temporary invalidity to become average 2 weeks shorter and to improve results of surgical treatment of ulcer disease.

[Study of regional blood flow in the prognosis of the process of healing of postoperative wounds].

The authors suggest using the regional blood flow in control over the course of the wound process after various types of laparotomy so as to prognosticate the possibility of suppuration of the postoperative wounds. Examination of 20 patients showed that the regional blood flow values depended on the operative trauma and the character of the inflammatory process in the abdominal cavity. Decrease of the regional blood flow in the wound to 24 ml/min/100 g and lower indicates that purulent inflammation will most probably develop in the postoperative wound.

[Patient rehabilitation after gastric resection for peptic ulcer].

The authors evaluated the results of gastric resection in the Hofmeister-Finsterer modification in 262 patients with ulcer disease. Treatment considered of drugs, balneotherapy and electric stimulation with consideration of the functional state of the resected stomach. The used rehabilitation complex allowed to improve the results of surgical treatment, to reduce time of temporary loss of working capacity by 2-3 weeks.

[The motor activity of the gastrointestinal tract in acute appendicitis].

From study of electrogastrograms in 92 patients with acute appendicitis before and in various periods after appendectomy conducted under local or halothane nitrous-oxide-oxygen anesthesia, the authors conclude that electrogastrography may be used in complex with other methods for prognosticating the possibility of the occurrence of postoperative paresis. They point to the preventive significance of general anesthesia in the development of postoperative pareses. Percutaneous electrostimulation had a favourable effect in 12 patients with paresis developing after appendectomy.

Acute Disease↗

[Evaluation of the surgical treatment of peptic ulcer patients].

The authors examined 134 patients after different methods of surgical treatment of gastric and duodenal ulcer. Examination included determination of blood erythrocytes and hemoglobin, serum plasma, changes of body weight, coprology, ECG and EEG. Normalization of above values was earlier in patients after selective proximal vagotomy. These data should be considered in the postoperative and outpatient treatment.
