[Etiology, pathogenesis, clinical aspects and prevention of acquired immunodeficiency syndrome].

It is suggested that etiology, pathogenesis and clinical pattern of HIV-infection advanced stages (AIDS) may be closely associated with acute accidental involution of the thymus and activity of the hypothalamic-pituitary-adrenal axis in 10-35-year-olds. Feasibility of preventing the disease progression in HIV-infected individuals by stimulation of microcirculation in the thymic cortex needs further investigations.

Acquired Immunodeficiency Syndrome

[Characteristics of acute infectious endocarditis in various types of hospitals].

The study has revealed that active forms of infectious endocarditis constitute 30-50% of all septic diseases in hospitals. It is the normal cusps of the valvular apparatus of the left cardiac portion that are predominantly damaged, deformed or destroyed. Endocardial involvement of the right portion of the heart and the pulmonary artery occurs very rarely. Infectious patients develop sepsis more frequently than patients with non-infectious diseases but it takes the form of acute infectious endocarditis less commonly. Currently the clinical course of acute infectious endocarditis is characterized by predominantly ulcerous thrombotic damage of the normal valves with the formation of heart disease, the development of congestive insufficiency of the circulation and thromboembolic complications and the lethal outcome in the first weeks of hospitalization in 95.6% of patients with acute endocarditis in infectious and 56.6% in non-infectious hospitals.

Acute Disease

["Sports heart"].

Examination of 165 athletes showed that their heart was enlarged as compared to the heart of 140 healthy persons who did not take part in sports. Indirect signs of regular ventricular dilatation, moderate myocardial hypertrophy and increased mass of the heart were encountered in athletes. A 30-minute exercise on a bicycle ergometer of 1 500--1 800 kgm/min induced signs of pulmonary hypertension and overexertion of the right heart cavities in athletes. Signs of short-term deposit of blood in the lungs attended by a fall in arterial pressures in the systemic circulation and increase in gas exchange in the lungs were revealed at the beginning and immediately after the exercise. Compensatory hypofunction of the heart and hypotension as compared to the initial condition developed during rest after the physical exercise. The size of the heart was restored due to an increase in residual blood in the ventricles.

[Disorders of microcirculation in food poisoning].

Thirty-seven patients with alimentary toxicoinfections were examined. The values of central and peripheral circulation and the fluid spaces were determined by one-stage dilution of two indicators, T-1824 and sodium thiosulfate. The capillary blood flow was studied by means of bulbar microscopy in reflected light. A deficit of interstitial fluid prior to treatment and disorder of microhemo- and lymphocirculation were revealed. Infusion of a crystalloid blood substitute with an optimum volume and rate increased capillary filtration and corrected the transinterstitial metabolism.

[Pericardial lesion in meningococcal infection].

The study was carried out in 52 patients with a severe course of meningococcal infection. In two of them having meningococcemia and in three with meningococcal meningitis and meningococcemia the disease was complicated by concomitant symptoms of allergic pericarditis. Complex therapy with steroid hormones, antibiotics, oxygen, cardiac glycosides and vitamins was effective in meningococcal infections complicated by myo- and pericarditis but failed to prevent their development.
