[A chronic hyperimmunocomplex process and its interrelationship with free radical-generating systems].

On chronic hyperimmunocomplex process (CHIP) model in rats of Wistar line (Cochrane C., Koffen D., 1973) we determined cAMP and cGMP concentration, nitrogen oxide (NO), urine, urine acid,--in muscle and clearance organs, and also in plasma. In result of carried out investigations there was determined the increase of large and middle CIC in blood plasma, their fixation on endothelium of aorta bifurcation and glomerula basal membrane, partially on liver cells. There was revealed considerable disbalance of cyclic nucleotides concentration in kidneys homogenates, spleen and plasma, with guanils fall (adenil index, what is interconnected with NO level lowering in all cases and urine increase in plasma, simultaneously with its decrease in number--in kidneys and spleen. Urinic acid increased in number in kidneys, spleen, plasma. These changes create favourable background for damaging of endothelium-neutrophil-thrombocyte cooperation through hemokinin, moleculo-adhesive, fermentative mechanism with further development of proliferative-remodulating processes in vessel wall.

[The participation of neutrophilic mechanisms in the pathogenesis of chronic elevated blood immune complexes].

For reproduction of chronic hyperimmunocomplexemia model the classic Cochrane C. G., 1973 elaboration was used. The circulating immune complexes (CIC) were identified by precipitation methods, and fixated--by immunofluorescent in endothelium reproduction of aorta, and their clearance organs--liver and spleen. The estimation of neutrophil status was carried out according to rosette-forming neutrophils ability--by latex, zimosane, mieloperoxidasa, NST tests. The growth of large and middle CIC in vessel bed was estimated, their fixation in aorta bifurcation, and also on liver and spleen calls, but their intensivity is less, comparing to aorta endothelium. The neutrophil status was characterised by O-neutrophils growth, strengthening of capturing macrophages function, considerable activation of fermentative polynuclear systems, although reserve possibilities in them grew in less degree. Morphologic investigations allow to speak mediatingly about neutrophils participation in endotheliocytes damage, and also about their ability to secure the mononuclear cells further participation in damaging of aorta walls and partially of liver and spleen.

[The use of immunotropic therapy in patients with systemic vasculitides].

148 patients with systemic vasculitis (SV) were observed. In 90 of them "in vitro" we evaluated the action of immunotropic mediators (levamisol, timalin, human plasma immunoglobulin, placental polybiolin)--to evaluate the expression of lymphocyte antigens and oxide-renewal phagocyte activity. It was estimated, that the major sensitivity patients--do have lymphocytes to placental polybiolin. We studied the influence of this preparation in SV patients, and its effectivity was compared with glucocorticoids. It was estimated, that placental polybiolin has selective action upon T-lymphocytes subpopulations, improves antigens expression on operating variations of T-lymphocytes, stabilises humorally-regulatory protein figures. Its application effectivity makes 76.7% comparing to glucocorticoids (52.6%) in patients with light and middle degrees of systemic vasculitides hardness--individually sensitive to placental polybiolin.

[Diagnostic criteria for systemic scleroderma].

Diagnostic informative value was studied of a set of N. G. Guseva's diagnostic criteria (1993) for systemic sclerosis (scleroderma). A total of 104 patients with systemic sclerosis (scleroderma) and 126 patients with other rheumatic diseases were examined. The above set of criteria was found out to be of high sensitivity and specificity--99% and 98.4% respectively. The authors present their own modification of the set of diagnostic criteria by N. G. Guseva (1993) which is not inferior in respect of sensitivity and specificity to the original but is more simple, and with a greater potential for further applications, due to which features it can come into widespread use.

[The immunological characteristics of systemic vasculitis].

A total of 142 patients with systemic vasculitis (SV) during the phase of activation of the process were evaluated. Diagnosis was based on the clinical as well as laboratory, functional and morphologic findings. Immunological investigation revealed increase in T-lymphocyte effector activity, marked sensibilization of lymphocytes to the intimal antigen, hyperimmunocomplexemia, decline in phagocytosis, hypocomplementemia. Musculocutaneous biopsies showed fixation of IgG and the complement C3-component. For different SV variants agreed specific markers were determined. The results obtained permit objectivizing different immunotherapeutic alternatives in SV treatment.
