[Morphological characteristics of reparative process in tuberculous inflammation].

The morphological analysis of the reparative process in the involvement foci and adjacent pulmonary tissue in patients with pulmonary tuberculosis and experimental animals during intensive focal resolution has revealed a change in the cellular composition: epithelioid cells are replaced by macrophages, T-lymphocytes SD8 (suppressors) by those CD4 (helpers); actively synthesizing fibroblasts and vascular endothelium appear in the area of specific inflammation instead of the cells that have a decreased collagen-forming function. The employment of various methods for stimulation of cellular reactions (lasers, electromagnetic irradiation of the mm range - MM waves, the complex of cytokines) leukinferon enables one to alter the qualitative and quantitative cellular composition of granulomas, the rate of metabolic processes and the differentiation of cells, as well as to examine the regularities of reparative reactions more effectively and more purposefully. The perfocally adjacent pulmonary tissue wherein the diseased parenchyma and stroma recover has been ascertained to be also involved into the reparative process.

[Tuberculous periodontitis].

Peridental foci of infection (PFI) in periodontitis in 113 patients with pulmonary tuberculosis were evaluated clinico-morphologically with regard to mycobacterial populations. Vegetation in PFI of bacterial, bacterial and L-form M. tuberculosis (37.2%) was associated with clinically inapparent periodontitis symptoms, formation of large and middle-size PFI with destruction and foci of productive tuberculous inflammation. Persistent L-forms of M. tuberculosis (59.3%) brought out a symptomless periodontitis with small and middle-size PFI, manifest immune or minor tuberculous inflammation, reparative processes. The findings suggest that PFI in tuberculous patients' periodontitis is a form of extrapulmonary tuberculosis.

[Functional morphology of the lungs in patients with tuberculosis and diabetes mellitus].

The lungs of 20 patients (between the ages of 14 and 46 years) with tuberculosis and type I diabetes mellitus were subjected to a morphological examination with the help of a light and electron microscope. Thirteen patients were operated on for fibrous-cavernous tuberculosis and 7 for pulmonary tuberculoma. A duration of the concurrent diseases was 3 to 10 years (5.5 years, on average). All patients had a serious form of diabetes mellitus. The combined affection was characterized by a progressive course of tuberculosis with involvement of bronchi in the process. Specific features of tissue reactions pertinent to this combined pathology were revealed which comprised defective defense mechanisms (alveolar macrophages, type II alveolocytes and fibroblasts in the form of their dystrophy) generalized affection of pulmonary vessels, intensive fibre formation and disorganization of the forming connective tissue, which has a bearing on the development of the pathological process.

[Intra-dental route of experimental tuberculosis infection].

A total of 13 dogs weighing 13-15 kg were infected by intra-dental route with the tuberculosis mycobacterium strain H37R in a dose of 0.5 ml in the 5th dilution according to a turbidity standard. Two dogs were given intravenously the same dose of tuberculosis mycobacteria and they formed a control group. The observation varied from 10 days to 5 months. It was found tuberculosis development in intradental contamination could be attended by generalized specific affection of the viscera and development of a "minor" tuberculosis inflammation in the peridental tissues.

[Clinical-morphological criteria of drug-induced hepatitis in pulmonary tuberculosis patients].

A thorough clinicomorphologic examination of 288 pulmonary tuberculosis patients with hepatic dysfunction revealed drug-induced hepatitis in 16.3% of the cases during their chemotherapy. Drug-induced hepatitis was found to be acute in 29.7%, chronic active in 27.6% and chronic persistent in 42.7% of these patients. Life-time morphologic investigation of the liver biopsy specimens is of primary importance for the evaluation of hepatitis etiology and its progress. The most characteristic morphologic features of drug-induced hepatitis are the presence of eosinophilic leukocytes in lymphomacrophagal inflammatory infiltrates, hyperplasia of smooth cytoplasmic network of hepatocytes and gigantic mitochondria with paracrystals in the matrix, fatty degeneration of hepatic cells as well as the presence of stellate reticuloendotheliocytes with abundant heterogeneous inclusions.

Acute Disease

[Pulmonary histiocytosis X].

Morphologic studies of the lungs of 61 patients aged 15-62 years with pulmonary histiocytosis X revealed a different morphologic picture in relation to the duration of the disease. The most prevalent were primary and chronic forms characterized by the presence of histiocytic granulomas of varying size with blue-green inclusions, eosinophilia and corpuscles X in the macrophage cytoplasm. As the process progressed, fibrobullous changes occurred in the lungs. An open biopsy of the lungs and testing of the lavage macrophages for corpuscles X appears to be more informative and safe for the morphologic diagnosis of histiocytosis X.
