[An epidemiological study of ischemic heart disease, vascular brain diseases and their risk factors in an open population of 25- to 64-year olds in Novosibirsk].

To study the incidence of coronary heart disease (CHD), cerebrovascular diseases (CVD), combined pathology (CHD and CVD), and their risk factors such as arterial hypertension (AH), overweight (OW), hypercholesterolemia (HC), and tobacco smoking in the same population, a random representative sample of male and female populations were examined in three districts of Novosibirsk. AH and tobacco smoking were found predominant among men, whereas OW and HC among women. It turned out that almost every fourth person out of the men and almost every second woman suffered from cardiovascular diseases. In the men, the CHD/CVD ratio was 1:1, that in the women, was 1:7. In the men and women, suffering from combined pathology, AH occurred more frequently as compared to those with CHD or CVD alone. OW promoted the development of CVD and combined pathology in women to a greater degree, whereas HC favoured the development of CHD and CVD in men.

[Mortality, morbidity, lethality and the cardiovascular risk profile in the population of Novosibirsk].

The authors analyze the results of investigations in accordance with the WHO program "MONICA" in one of the districts of Novosibirsk. The bases of the objective information system pertaining to the tendencies in mortality, morbidity and lethality due to cardiovascular diseases, the prevalence of risk factors in the population, the possibilities of their monitoring have been established.

[Long-term observation of the trends in morbidity, lethality and mortality from myocardial infarct in the population aged 25 to 64].

The data are provided on long-term observation (for 10 years) of the myocardial infarction (MI) death rate, and incidence among the community using the standard WHO program. The Register of Acute Myocardial Infarction. It is shown that over the first two years of observation, the death rate and mortality tended towards decrease and then got stabilized. The diseases incidence tended towards growth and then got stabilized as well. It is noted that the prehospital death rate and lethality because of MI considerably exceeded the hospital ones and underwent almost no changes over 10 years. It is emphasized that perfection of the assistance rendering to MI patients does not solve the problem of the high death rate because of MI. It is probable that only primary care may solve the problem of reducing the cardiovascular disease incidence and death rate.

[Ten-year trends in acute cardiovascular morbidity in an open population].

The paper treats the data concerning the acute cardiovascular pathology occurrence examined during ten years in an open population of one of the districts of Novosibirsk. It is shown that at first there was a tendency towards the incidence increase followed by its stabilization and then by a tendency towards reduction over the last year. It is proved that the district selected for prolonged monitoring is a typical district of the city and that the tendencies in the incidence among its community should be classified as objective. Comparison of the WHO programs: The Register of Acute Myocardial Infarction (1969) and the Monitoring of the Trends in Cardiovascular Diseases and Determining Factors (1983) has shown no substantial differences in the incidence and death rates obtained in accordance with both programs. It is stressed that this enables one to compare the data of both programs applied to different populations.

[Health education and motivation of the population with regard to health-related problems].

A random representative population sample of 1493 individuals (753 males and 740 females), aged 25 to 64 years, was investigated, using standard epidemiologic procedures as part of a program, studying, trends of development and control of cardiovascular diseases. Population awareness of health status and attitudes to preventive efforts and harmful habits were assessed with reference to age and sex. Preventive screenings were considered to be useful by 83% of the males and 88% of the females; these rates were however smaller for young individuals. In groups of older individuals, health self-assessments were less adequate, while willingness to abandon bad habits increased, particularly so in the females, as compared to the males.

[Relation between behavioral risk factors of ischemic heart disease and various somatic indicators in men aged 25-64 years living in Novosibirsk].

In every second male aged 25-64 years from a representative random sample of a Novosibirsk district, relationships were examined between types A and B behavior, occupation, education, and some somatic parameters. Type A behavior was found to be associated with higher occupation and education levels. The individuals with type A behavior more frequently showed elevated levels of cholesterol, low-density lipoprotein cholesterol and increased indices of body weight and atherogenicity as compared to those with type B behavior.

[The prevalence of ischemic heart disease among an organized population of Novosibirsk].

In order to study CHD prevalence in an organized population (aged 20 to 59) in Novosibirsk a simultaneous cardiological study of workers and employees at one of the factories was conducted in 1985. CHD prevalence rate among all examinees was 12.2%. The growing number of risk factors brought about an increase in the number of detected CHD cases. Particular attention should be drawn to patients with risk factors and to prophylaxis among them.

[Prevalence of ischemic heart disease and various ECG changes according to the results of a population study].

A study of a random representative sample has furnished data on the prevalence of ischemic heart disease, its varieties and various ECG changes among an open Novosibirsk population of 40- to 59-year-old males. The results are compared to data obtained in other cities and regions of the USSR. It is demonstrated that the prevalence of ischemic heart disease is virtually similar in Novosibirsk and other big cities and differs basically from the situation observed in a small town.

[Prevalence of ECG changes in an unorganized population of Novosibirsk].

The paper is concerned with the data on the prevalence of various ECG changes in an open male and female population (aged 25 to 64) in Novosibirsk obtained on examination of a random representative sample. The analysis permitted defining groups of persons with prognostically unfavorable ECG changes serving as a basis for further study of regularities of their appearance.

[8-year prospective study of acute cardiovascular pathology in an open population].

Changes in the incidence of myocardial infarction, and associated mortality and lethality rates are reviewed over a 8-year period in an Novosibirsk district. Data on late postinfarction outcomes, obtained in a WHO-sponsored study, "Acute myocardial infarction register", are also presented. The incidence, mortality and lethality rates are showing a stabilization trend at present; in late outcomes, the greatest mortality and lethality rates fall to the first postinfarction year.

[Acute myocardial infarction and cerebral stroke among the population of Novosibirsk].

An epidemiological study of the morbidity and mortality rates of acute myocardial infarction and cerebral stroke based on the standard WHO programs "The Register of Acute Myocardial Infarction" and "The Register of Cerebral Stroke" was conducted for 12 months in one of the districts of Novosibirsk. The morbidity and mortality rates of males due to cardio- and cerebrovascular accidents were almost twice as high as those in females. The study of morbidity and mortality rates over time on the basis of the standard programs will make it possible to evaluate comprehensively the effectiveness of preventive programs.

[Morbidity, lethality and the effectiveness of rendering aid to acute myocardial infarct patients in Novosibirsk].

Epidemiological study of myocardial infarction morbidity rate was conducted according to the WHO standard program "Acute Myocardial Infarction Registers" for 12 months in one of the Novosibirsk regions. A relatively high disease rate was registered among the 20 to 65-year-old population group, 2.56 cases per 1,000 population. Work carried out according to this program makes it possible to improve considerably the organization of medical aid to patients with myocardial infarction, particularly in the prehospital stage.
