[Thrombembolectomy in the treatment of patients with acute disorders of mesenteric circulation].

In acute impairement of the mesenterial circulation, the authors in 31 patient performed thrombembolectomy from the superior mesenteric artery, which in 22 was supplemented with resection of a necrotized area of the intestine. Five patients recovered. In these patients within the first 24 hours, the control relaparotomy was performed and the early postischemic disorders were cupped off with the use of tubal decompression, active aspiration of the intestinal contents, lavage of the intestine with the balanced electrolyte solution, hemosorption and intraintestinal administration of the oxygenated 10% glucose solution.

Acute Disease↗

[Clinical picture, diagnosis and surgical treatment of mesenteric vein thrombosis].

Thrombosis of mesenterial veins was found to develop in 30 women and 28 men of the average age of 51 +/- 2.1 years. It was established that case history of ischemic heart disease, diseases of the liver, spleen, thrombotic lesions of vessels of different localization and splenectomy should be taken for conditions predisposing to the development of mesenterial vein thrombosis. Treatment of occlusive lesions of mesenterial veins must include not only resection of the infarction portions of the intestine, but also correction of protein-electrolytic disorders, nitrogenous and carbohydrate metabolism as well as liver function.

Diagnostic Errors↗

[Acute disorders of the mesenteric circulation among heart defect patients].

The author analyzes the clinical picture, diagnostic methods, and results of the treatment of mesenteric artery thromboembolism in 30 patients with rheumatic heart disease. The early symptoms that permit the physician to suspect mesenteric thromboembolism include sudden convulsive pains in the abdomen, vomiting, diarrhea, leukocytosis, and the discrepancy between the clinical picture of the complication and scarce objective data from examining the abdominal cavity. When the diagnosis of the acute occlusion of the mesenteric vessels has been made, urgent surgical intervention is indicated which is to include the resection of the non-viable portion of the intestine and the surgical correction of the blood flow in the superior mesenteric artery.

Acute Disease↗

[Role of arteriosclerosis in the development of an acute disorder of the mesenteric circulation].

On the basis of observation of 227 patients with the acute disturbance of mesenteric circulation the author considers intestinal infarction to be one of serious complications of atherosclerosis. Early diagnosis and operative treatment (reestablishment of circulation in mesenterial vessels, resection of the gut), postoperative intensive therapy allow improving results of the treatment.

Abdomen, Acute↗

[Pathogenesis and the diagnosis of postoperative thromboses of the mesenteric vessels].

The examination of 36 patients with postoperative disorders of the circulation in the mesenterial vessels has shown postoperative infarctions of the intestine to be most frequently observed in elderly patients after surgery on the abdominal organs. Inflammatory-purulent diseases of the abdominal organs against the background of cardiovascular diseases are of special importance.
