[Laparoscopic surgeries for achalasia of esophagus].

For achalasia of esophagus (AE) 23 patients were operated, in 14 of them surgical intervention was performed using thoracotomy access, in 9--laparoscopic. Standard laparoscopic method was improved by the authors, allowing to achieve better visualization of the operation field, reduction of operation duration, decrease its traumatism in comparison with this data during performance of operation using thoracotomy access. Patients are enduring laparoscopic operation better, this operation is technically performable, according to most of indexes its exceeds thoracotomy method. There was established, that operative treatment of AE is expedient to conduct in early periods of the disease.

[Assessment of risk of thromboembolism in patients undergoing surgery].

The risk factors of venous thromboembolism (VTE) in 1146 patients who were underwent operated for the diseases of digestive organs, for obliterating atherosclerosis of abdominal aorta and peripheral arteries, diseases of bones and joints were studied. The methods of VTE prevention in these patients were studied. To evaluate the risk of VTE we designed the special chart. Three levels of risk of VTE: low, moderate and high were distinguished. The specific prophylaxis of the VTE using low molecular weight heparin fraxiparine was given to the patients moderate (90.7%) and high (93.3%) risk degree.

[Surgical laser application in thoracic surgery. Potential and critical evaluation].

A 25-year experience of pulmonary and pleural surgery in 348 patients, reconstructive esophageal operations in 74 patients, bronchoscopic interventions using different laser techniques--in 925 patients, and thoracoscopic operations of traumatic and spontaneous pneumothorax--in 150 patients has been summarized. The methods of laser pulmonary resection, processing of bronchial stump, fibrous capsule, pleurodesis, stitching of esophagogastric, esophagointestinal and interintestinal anastomoses and of bronchoscopic photo destruction of tracheal and bronchial tumors have been described. Haemostatic, bactericidal and ablastic properties of laser have been described. Lasers were used in miniinvasive thoracic surgery for thoracoscopic and videothoracoscopic interventions.

[Diagnostics and treatment of varicocele as a symptom of renal venous hypertension].

From the year 1997, we examined 61 patients with left-sided varicocele aged 9-28 years. Nine (14.7%) of them had a recurrence of the disease after intervention (Ivanissevich operation). The renal venous hemodynamics was evaluated in order to choose the right method of surgery. All patients underwent surgery, the results were positive. The operation of testicular vein was conducted together with excision of varicous scrotal veins.

[Endoscopic hernioplasty for inguinal hernia as an alternative to open procedure].

Results of treatment of 124 patients with inguinal hernia, to whom endoscopic hernioplasty was performed, were analyzed. In 96 of them transabdominal hernioplasty was done and in 28--the preperitoneal one. In 2 patients the hernia recurrency occurred. The method is mostly effective for recurrent and bilateral hernia. High cost, the necessity to apply narcosis, the danger of peritoneal cavity organs damage are the method shortcomings.

[Morphological substantiation of choice of the laser processing of residual cavity wall after hydatidectomy for pulmonary hydatid cyst].

Morphological investigation of the pulmonary echinococcal cyst after applying the laser irradiation influence on it was done. The optimal way for performance of an adequate scolexo- and bactericidal treatment of fibrous capsule after the pulmonary hydatidectomy conduction is application of defocused to 5-10 mm irradiation of the CO2 and the Nd AIG-laser with 150-250 Wt/cm2 power density. In the cyst suppuration it is preferable to use Nd AIG-laser with increase of the exposition time in 3-4 times and of the energy absorbance up to 400-800 J/cm2. Local coagulation using irradiation of the Nd AIG-laser by the contact method with the 20 Wt power output (250-400 Wt/cm2 power density) is applied if bronchi, which are confided into residual cavity, have diameter up to 3 mm.

[Mini-invasive methods of treatment of choledocholithiasis].

Last years in the clinic in diagnosis and treatment of choledocholithiasis choledochofibroscopy is applied widely while performing an open or laparoscopic intervention and while introduction of choledochoscope through external biliary fistula as well. Best results were achieved in those patients, in whom it was possible to evacuate all concrements through ductus cysticus. Introduction of biliary stent have permitted to perform clippage of ductus cysticus without making external drainage, securing rapid rehabilitation of patients.

[Contemporary approach to the treatment of complicated diabetic angiopathy of the lower extremities].

The experience of treatment of 278 patients with diabetes mellitus, complicated by the lower extremities diabetic angiopathy, was summarized. The authors had proposed the complex of treatment, including combined application of the high- and low-energy laser irradiation and catheterization of the shin arteries branches and the anterior abdominal wall superficial arteries for an adequate introduction of medicinal agents towards the affection focus. Application of the method proposed had promoted the results of treatment improvement, had permitted to proceed with economical amputation of extremity and to escape the reoperations performance.

[Complications after laparoscopic cholecystectomy].

In 1780 patients the results of laparoscopic cholecystectomy (LCH) conduction for 1992-1997 period were analysed. In 1123 (63%) patients the operation was conducted for chronical and in 657 (37%)--for an acute cholecystitis. In 162 (9.1%) patients the choledocholithiasis features were revealed before the operation. In 46 (2.6%) patients the conversion to open laparotomy was accomplished. The LCH operation duration was from 35 to 190 min, (50 +/- 12) min in average. The frequency of an early complications occurrence was 8.8%, and in 18 (1.4%) patients the complications were revealed in late follow-up period. All the patients survived.

[Endoscopic laser operations on the trachea and bronchi].

The result of 596 laser endoscopic operations in 368 patients with tracheobronchial tumor was analyzed. In 175 (47.5%) patients the complete recanalization of stenosis was noted, in 119 (32.4%)--partial and in 74 (20.1%)--treatment was uneffective. In 9 (3.4%) patients the complications occurred.

[Contact YAG laser scalpel].

Laser scalpel, designed on the base of Neodymium YAG Laser, is proposed for contact tissue dissection during the surgical procedure. The application of the sapphire monocrystal tips as a contact laser scalpel, has provided the opportunity for obtaining satisfactory hemostatic and dissecting effects with minimum energetic losses. Proposed by the authors, contact laser scalpel is light enough, rather handy in surgical procedures even in the regions difficult to access in thoracic and abdominal cavities and allows to perform precisional dissections of the tissues.

[Application of contact laser scalpel in thoracic surgery].

The authors have carried out 296 transthoracic and endoscopic YAG-laser operations in various diseases of bronchopulmonary system. Sapphire and quartz applicator-tips were used as a contact scalpel. The procedures of laser operations with the use of the contact mode are described in detail and comparative evaluation of both types of thermal applicator-tips is given. The combination of cutting and coagulation properties of the contact laser scalpel enables its use in various operations on the lung and pleura including thoracoplastic interventions as well as accomplishment of an endoscopic photoresection of endobronchial tumors.

[Laparoscopic cholecystectomy in acute cholecystitis].

Laparoscopic cholecystectomy was fulfilled in 108 patients admitted to the clinic with acute cholecystitis. Operations were made on 73% of them during the first four days from the beginning of the disease, 18.5% were operated upon within 5-7 days, 8.5% - 8 days later. Endoscopic papillotomy with removing the stones from the choledochus was performed in 10% of the patients before operation. Serious problems during taking the gallbladder from the inflammatory infiltration were observed in 29% of the patients. Technical problems took place more often if the patients were operated upon 5 days after the beginning of the disease. Change for open laparoscopy and standard cholecystectomy were necessary in 9 patients (8.3%). There were no lethal outcomes after laparoscopic cholecystectomy. Complications were observed in 12 patients (11.1%). The average period of staying at the hospital was (5.2 +/- 2.1) days. Laparoscopic cholecystectomy can be successfully performed in patients with acute cholecystitis by a sufficiently experienced surgeon.
