[A method of stable osteosynthesis in diaphyseal fractures of the femur].

The known methods of operative treatment of femur by means of intraosseous osteosynthesis, based on jamming of intraosseous structure in the bone, don't ensure sufficient level of the osteosynthesis stability that results in the secondary displacements. According to the method, proposed by the authors, a rod is introduced through the intercondylar fossa of the distal fractured fragment of femur (Imamaliyev, 1987) and is pierced through into the proximal fractured fragment of femur so that it perforates cortical layer of the bone in the region of greater trochanter with strong fixation in it. The structure is removed through the small incision in the area of the greater trochanter of femur. 35 patients have been operated according to this method. The remote results have been studied with 22 of them. Consolidation terms equal to 2.5-5 months. The secondary displacement has been noted with 4 patients with comminuted fractures.

[Treatment-diagnostic tactics for conservative treatment of uncomplicated compression fractures of the spine in the lower chest and lumbar regions].

Fractures of the vertebra bodies were divided by the authors into 5 groups depending on the character of destruction. Reposition is not required in a deficiency of the height of the injured body to 15%. Reposition is necessary if the deficiency is more than 15%. The aim of the reposition is to correct the axis of the vertebra by means of reestablishment of the shape and mass of the injured vertebra body by unbending and simultaneous stretching the vertebral column. The proposed method was tested in treatment of 241 patients. The necessary reposition was achieved in 84.4% of the patients. Primary invalidism was determined in 8.1% for 1-2 years. The working ability was completely recovered in 83.1% of the patients and partly in 16.9%.

[Treatment of diaphyseal fractures of long tubular bones by functional intraosseous osteosynthesis].

A clinical analysis of treatment of 328 diaphyseal fractures of long bones in 304 patients by functional intraosseous osteosynthesis which is not to be followed by plaster immobilization has shown that the function was reestablished long before the consolidation. The functional intraosseous synthesis was performed without drilling of the bone marrow cavity, without special appliances or devices. Terms of invalidism are shorter than traditional ones. Consolidation of the fractures takes place within 5-10 months. The nails are removed at the same terms. The method is of low cost, easy in use, reliable, is followed by few complications.

[Use of the vascularized iliac crest transplant in disorders of the processes of consolidation of diaphyseal fractures of the femur].

The authors describe a method of plasty of proximal parts of the femoral diaphysis with a nonfree vascularized autotransplant from the iliac crest on the muscular-vascular connections including the strainer of the wide fascia, portion of the musculus gluteus medius and the feeding ascending branch of the lateral artery surrounding the femoral bone with the accompanying veins. The operation has anatomical and experimental grounds. Results of treatment of 6 patients have shown its high effectiveness in disturbed processes of consolidation of fractures of the upper and medial thirds of the femur diaphysis.

[Experience with using autologous transfusion of ultraviolet- irradiated blood].

Autotransfusion of ultraviolet irradiated blood (TUVIB) has been included into the complex of treatment of 96 elderly and senile patients with fractures of the proximal end of the proximal end of the femur. The MD-73M apparatus has been used. The values of central hemodynamics, the threshold of tactile and pain sensitivity, the immune status and the coagulative system of blood have been studied. A high efficacy of TUVIB in this group of the patients has been pointed out, and a conclusion has been made about the necessity of individual choice of the doses of ultraviolet irradiation of autologous blood on the basis of adaptive reaction of calm activation. The side effects and the complications in performing TUVIB, the ways of their prevention and treatment have been considered.
