[Hepatobiliogastroscintigraphy in assessment of the hepatobiliary system in primary cancer of the liver].

Primary cancer of the liver is characterized not only by laboratory evidence for cytolytic (87.5% cases), cholestatic (100% cases) syndromes, high levels of oncomarkers (CA-19-9, 50%; CEA, 68.75%; alpha FP, 43.75%) and activation of hemostasis system but, as shown at hepatobiligastroscintigraphy with 99m-Tc-brommeside, by at least 2-fold decline in absorptive and excretory function of hepatic polygonal cells (in 75% cases), at least a 5-fold decline in secretory function of the gallbladder (in 100% cases), cholestasis (in 100% cases) manifesting as a 3.8-fold reduction, on the average, of the ratio of secretory function of the gallbladder to that of the intrahepatic bile ducts, dilation of the latter (in 50% cases), Oddi's sphincter hypotonicity (in 50% cases). None of the patients showed duodenogastric reflux.

[Hemostasis and antithrombotic therapy in massive thromboembolism of pulmonary artery].

The results of treatment are reviewed for 114 patients with massive thromboembolism of the pulmonary artery in respect to antithrombolytic measures and initial condition of hemostasis. Prognostic hemostasiological criteria of tolerance and relative resistance of the patients to streptokinase preparations and optimal thrombolytic methods in massive thromboembolism of the pulmonary artery are determined.

[New principles in the prognosis, diagnosis, prevention and treatment of the DIC syndrome].

In accordance with the proposed methodics the state of Hageman factor (HF) system was studied, i.e. intimate connection of callicrein-kinine, coagulation and fibrinolytic blood systems in various clinic and hemostasiological forms of DIC-syndrome, acute and chronic arterial thromboses, thrombembolisms, post-operative and thrombolytic hemorrhages in 1118 patients. The author describes the principles of diagnosis, prophylaxis and correction of the disorders in blood coagulation, fibrinolysis and kinogenesis in this pathology. The article contains data concerning the structure of researches, index and HF coefficients, as well as reference hemostasiological criteria, classification and medical tactics in various forms of DIC-syndrome.

[The radionuclide diagnosis of post-gastric resection disorders].

The article studies the possibilities of radionuclide method in disclosing the basic postgastroresectional syndromes: dumping syndrome, duodenogastric reflux and afferent loop syndrome. After resection of stomach 92 patients were examined throughout a period from 1 to 10 years. During dynamic scintigraphy of liver characteristic changes were disclosed in biliary secretory system which could have diagnostic significance. This method gives the possibility to determine the frequency, duration and volume of duodenogastric reflux. It was found out that Roux's [correction of Rue's] method of resection have resulted in minimal number of duodenogastric refluxes. Dynamic scintigraphy of stomach is useful for studying evacuative function of the resected stomach and for afferent loop syndrome diagnosis, which sometimes is more informative than a wide-spread barium sulfate roentgenological method.

[Comparative study of hemostasis in the aorta and peripheral veins of patients with atherosclerosis of lower limb arteries].

A total of 127 male patients with obliterating atherosclerosis of lower limb arteries, stages I-IV, aged 43-76 years, were investigated. Thrombogenic potential in the blood from the upper, and lower limbs of patients was higher than in normal subjects, that was evidenced by elevation of blood platelet aggregation capacity, increased content of thrombocytic factor 4, fibrinogen concentration, and a rise in the level of soluble complexes of fibrin monomers. These changes were attended by the lowering of anticoagulant activity (a decrease in antithrombin III level) and fibrinolysis. The thrombogenic potential in the blood from the aorta was significantly higher than in the venous blood: increased intravascular spontaneous platelet aggregation, fibrinogen concentration, and significant rise in antithrombin III activity). Antithrombogenic potential of the vascular wall in patients is substantially lower as compared to that in normal subjects.

[Ultraviolet spectrophotometry of the cerebrospinal fluid in the differential diagnosis of strokes].

The authors describe the possibility of using the results of ultraviolet spectrophotometry of the cerebrospinal fluid (CSF) for the differential diagnosis of ischemic, hemorrhagic and mixed cerebral strokes. It was found that repeated examinations may be of a definite prognostic value. A correlational analysis showed that the greatest effect on the optical density of the CSF in cases of hemorrhagic strokes is exercised by protein and bilirubin and in cases of ischemic strokes by protein and aspartate aminotransferase (AST).

[Correlation between serum lipid level and coronary bed lesions in patients with ischemic heart disease].

The blood lipid levels were collated with the atherosclerotic damage to the coronary bed in 124 patients with coronary heart disease. An increase in the content of total cholesterol and low density lipoprotein cholesterol directly correlated with the degree of the total lesion of the coronary bed. The concentrations of high density cholesterol and very low density cholesterol and the atherogenicity index showed no correlation with the severity of atherosclerotic changes in the coronary arteries. The level of triglycerides in these patients displayed a tendency to increase. In evaluating the severity of coronary heart disease, it is recommended to determine the levels of total cholesterol and low density lipoprotein cholesterol. Changes in the concentration of high density lipoprotein cholesterol are unreliable for assessing the degree of coronary atherosclerosis.
