The potentialities of cerebral oximetry in the para-infrared band as a means of neuromonitoring are assessed. The method is highly informative for selecting the treatment strategy during the acute period of craniocerebral injury and cerebrovascular diseases. Simultaneous analysis of intracranial pressure, central hemodynamics, and cerebral oximetry permits differentiation of mechanisms of intracranial hypertension and its compensation. Increase of rSO2 in increased intracranial pressure corresponds to brain hyperemia, its decrease to cerebral ischemia. A long trend of rSO2 changes is informative for disease prediction.
Electron microscopic study of arteries in spasm and ischemia of the brain revealed substantial ultrastructural changes in the vascular wall. The changes are characterized by varying thickness, edema, dissociation of fibrillar and amorphous components of internal elastic membrane of the arterial wall, by lesion in the network of collagen fibers on its inner surface, by the presence of defibrotic and uncoiled collagenous fibers of the adventitia. The changes are followed by the impairment of the arterial skeleton and hence they lead to their altered elastic properties.
The contralateral pterional approach was used in the surgical treatment of 17 patients with cerebral aneurysms. Six of them had solitary aneurysms in the ophthalmological segment and fork of the carotid artery, the aneurysms were clipped by the contralateral approach. Eleven patients were found to have multiple aneurysms in different carotid beds and the main artery, the aneurysms were clipped from blood flow by applying the pterional approach, including the contralateral one. The contralateral pterional approach may be the method of choice in surgery for cerebral aneurysms.
