[Suture and suture material in the surgery of finger tendons].

The authors describe two methods of suturing injured finger tendons and requirements to suture material. A polyamide monofilament thread with a conventional number 3/0 used by the authors for a removable suture of the tendon is the suture material of choice. For a buried suture it is expedient to use absorbable threads which begin degrading within three weeks after operation. The developed by the authors loop-shaped removable and buried sutures are strong, atraumatic and simple for fulfillment.

[Early diagnosis of orthostasis disturbances].

The observations were carried out in 51 healthy volunteers (26 men and 25 women) who underwent passive orthotest with the registration of thoracic impedance (TI). It was found that women had a higher index of TI both in rest and in dynamics of the performance of the orthostatic test than men. In 10 subjects the increase of TI in orthostasis was higher than 15% that can be supposed to be a criterion of the pathological drop of the venous return. The TI dynamics in performing the orthotest was quicker than changes in AP and the appearance of complains can be taken for a preclinical symptom of latent volume disturbances in the orthostatic regulation that has an important diagnostic value.

[Calcification of the meniscus].

Degenerative-dystrophic changes in resected damaged menisci are noted with frequency, their calcification is a rare event. The authors in their orthopedic practice observed three patients with meniscus calcification. There is given a communication of one observation. The authors believe that meniscus calcification is an indication for its removal.

[Outcomes of the therapy of old injury of finger flexor tendons].

193 fingers of 131 patients have been treated at the level of the I, II and III zones due to inveterate tendon damages. In case of isolated damage of deep flexor tendon of the II-V fingers at the level of the I zone there were made palliative operations of 12 fingers: tenodesis and arthrodesis of distal interphalangeal articulation in functionally advantageous position. They failed to achieve successful outcome with one finger. In 17 cases long flexor tendon of the I finger had been restored by different methods; in all cases there were achieved good and excellent outcomes. Three fingers (IV, V) with the tendon, damaged at the II zone level, were subjected to plasty by free autotransplant. The outcomes are as follows; satisfactory--2, bad--I. At the remaining 161 finger (II-V) deep flexor tendon has been restored by different methods with utilization of autotendon with its natural blood circulation or revascularized one. There was formed an artificial tendon sheath in some methods. Positive outcomes, as evaluated by the system of the American association of hand surgeons, were achieved in 138 (85.7%) cases, by Buch-Gramcko system--in 143 (88.8%) cases.

[Orthostatic hypotension in primary amyloidosis].

Proceeding from clinicoanatomical observation of a 50-year-old man, the authors discuss the pathogenesis of arterial hypotension developing in generalized amyloidosis with severe affection of the cardiovascular and nervous system, and the adrenal glands.

[2 cases of temporal arteritis].

Two cases of temporal arteritis are described. In patient K., aged 79 years, the disease ran an acute course and resulted in bilateral blindness. In patient T., aged 67 years, the disease ran a subacute course with a unilateral amaurosis and involvement into the disease of the cerebellar, humeral, renal, leg and, possibly, heart arteries. Elevation of the amount of circulating immune complexes in patient K. and no elevation in patient T, with the complement content being within normal, can be viewed as features of the disease progress.
