[A transition of a functional disease to an organic psychosomatic disorder].

Twenty patients with ulcer (83 female and 117 male) have been studied. It was found out that many patients had functional disorders of the gastrointestinal tract in the form of gastralgia or neurogenic vomiting ("organic neurosis of stomach") long before the disease manifestation. At that time the patients underwent gastroscopy and X-ray examination of the stomach and the duodenum but no pathological organic changes were revealed. The study showed that 13% of men and 28% of women with "organic neurosis of the stomach" had signs of ulcer disease 12-13 years later. Therefore, a transition of functional to organic within unified psychosomatic process is in principle possible.

[Clinical features and diagnostic criteria of bulimia nervosa].

Based on the results of the clinical follow-up study of 41 female patients, diagnostic criteria of bulimia nervosa that should be used in clinical studies are suggested as follows: (1) presence of anorexia nervosa or transitory amenorrhea in the premorbid period; (2) eating attacks with losing of the control over food consumption not less than twice a week during 3 months; (3) compensatory behavior in the form of spontaneous vomiting, abuse of purgative and diuretic medications etc; (4) fear of obesity; (5) cycloid affective changes with higher impulsivity, reduction of the control over primitive drives and/or expressed anxiety disorders; inclination to alcohol and drug abuse and nicotine dependence; (6) changes of the body mass index; (7) absence of amenorrhea. The disease dynamics is characterized by formation of the pathological cycle "diet--overeating--compensatory behavior" on the background of cyclothymic affective disorders. Two types of bulimia nervosa--with and without other drive disorders--have been singled out.

[Premorbid period of anorexia nervosa].

Premorbid period of anorexia nervosa was studied in 96 female patients. In 64 cases (group 1) anorexia nervosa was as a separate disease and in 32 cases (group 2)--as a syndrome of endogenous mental disorder. In group 1 premorbid period was characterized by the features of neuropathy and residual organic disorders in combination with accumulation of subjects with personality disorders in the patient's family that promoted development of prolonged neurotic disorders. In group 2 there was hereditary psychosis load, high frequency of obstetrical and other perinatal complications, overvalued ideas and predisposition to autistic games and fantasying. In both groups, psychopathological peculiarities included typical affective, obsessive-phobic, hypochondriac, cenesthopathic and behavioral disorders, of a stable character often provoked by psychotraumatic events.

[Borderline mental disorders in patients with prostate cancer].

Sixty five patients, aged 48-87, with prostate cancer at different stages were studied. Borderline mental disorders were found in all the patients. In the majority of them, affective disorders in the form of anxiety-depressive (52.3%) and astheno-depressive (24.6%) syndromes were observed. Astheno-hypochondriacal and dysphoric syndromes were detected less frequently (in 15.4% and 7.7% of the cases, respectively).

[Functional gastralgia].

Psychiatric and psychological examinations of 100 patients (80 females and 20 males) with functional gastralgia (ICD-10 diagnosis F45.31) have been conducted. In all the cases, the disease developed in chronic stress in persons with higher anxiety inclined to mild anxious and depressive reactions. Four stages of the disease development were described: reactive (neurotic) depression, psychosomatic reactions, psychosomatic cycles (mono- and polysystemic) and psychopathization. Development of psychosomatic personality was expressed as hypochondriac type.

[Psychological aspect of psychosomatic pathology of the colon].

Taking into consideration the age of individuals, 100 patients (25 men and 75 women) aged 21-60 years were examined by using clinical, psychological and statistic (correlation) analysis to determine whether there are relations between clinical manifestations of the irritable bowel syndrome and personality. They were all referred to a psychiatrist from a gastroenterology hospital. The formation and dynamics of this syndrome, as well as its the clinical variations (diarrhea, pain, constipation) are largely determined by personality traits (increased anxiety; high inclination to somatization of emotions with inability to adequately verbalize them, pronounced intrapunitivity; direction of hostility to themselves; permanent inclination to depressive reactivity) and a passive-defensive behavior under the conditions of frustration.

[Personal behavior and mental state in anorexia nervosa].

Personality of the patients with anorexia nervosa is characterised by high neurotization and anxiety that resulted in diffuse anxiety in stress situation. Personal peculiarities included also high total hostility and intrapunitivity, inclination to obsessive-phobic and dysthymic reactions and tendency to somatization of anxiety. Such peculiarities promoted the choice of intrapunitivity type of reaction in situation of frustration. Its manifestation increases with an increase in the disease duration. The range of psychotic disorders in anorexia nervosa is restricted to anxious-depressive, obsessive and asthenic sphere.

[Aspects of the development in dysphagia and its functional dynamics].

100 patients (88 women, 12 men) of 16-64 years of age with functional dysphagia were examined. In all the cases the disease develops under the influence of stress in individuals with high anxiety, easy formation of dysthymic, obsessive-phobic reactions in the situation of frustration. Functional dysphagia arises most frequently at the age of 20-40 years. 4 stages pass in the course of the disease: a stage of reactive (neurotic) depression; psychosomatic reactions; psychosomatic cycles (mono- and polysomic); psychopathization. Psychopathization manifests pathologic (psychosomatic) development of personality observed usually in long-term course of dysphagia.

[Mental disorders in basaloma].

There were observed 70 patients (21 men, 49 women) with basaliomas (cancer of the skin). Mental disorders were observed in form of affective disorders (depression, hypothymic colour of the surrounding), syndrome of dismorphophobia-dismorphomania, relatively short cancerophobic feelings, somatogenic asthenia and a development of personality disorders. A forming of the mental disorders were conditioned at basaliomas by a complex influence of both somatogenic and psychogenic factors. An influence of the psychogenic factors was relatively transitory and psychologically comprehending (dysmorphophobia, cancerophobia, depression). An influence of the somatogenic factors predetermined a stability, a duration and a small reversibility of mental disorders (asthenia, irritative weakness, pathological development of the personality).
