[Organization and work experience of the endoscopy office in an emergency abdominal surgery clinic].

Within a five-year period in a special room intended for endoscopy 1934 investigations were performed, including 1318-on urgent indications. 77 esophago-gastroduodenoscopies were performed elsewhere-in the operating room, in the intensive care unit; 35 investigations were carried out under general anesthesia with the use of myorelaxants of a short effect. In 256 cases the neccesity of active infusion therapy and reanimation arose. In the paper, the authors' considerations concerning the need of organization, in large cities, of a mobile endoscopic team attached to the emergency medical aid are given with special reference to some features of the organization of endoscopy room in the clinic of emergency abdominal surgery.

Abdomen, Acute↗

[Functional state and structure of gastric mucosa in patients with ulcer disease after vagotomy and pyloroplasty].

107 patients were examined in different terms after vagotomy (up to 6 years) performed for ulcerous disease of the duodenum, 43 of them--prior to and after the operation. It is found that the suppression of the acid- and enzyme secretory function of the stomach is not related to the development of the mucosa atrophy, but it is conditioned by eliminating the resolving role of vagus nerves and by a reduced sensitivity of fundic glands to humoral stimuli.

[Emergency esophagogastroduodenoscopy in the diagnosis of acute gastrointestinal hemorrhage].

It is reported that among 1383 endoscopies, performed in the clinic, 618--were defined as urgent ones for acute gastrointestinal hemorrhages. There were 470 male and 148 female patients, their age ranging from 14 to 93 years. 408 endoscopies were performed during the first 6 hours since patients' admission to the clinic, and 210--during the following 48 hours. Contraindications to an urgent exploration of patients in hemorrhage from upper portions of the digestive tract are determined.

[Diagnosis and treatment of the Mallory-Weiss syndrome].

Twenty-six patients with the Mallory-Weiss syndrome were treated in the clinic, in 23 of them the diagnosis was established at the height of hemorrhage by means of esophagofibroscopy. Nine patients were operated upon, since conservative therapy was a failure. Postoperatively, 2 patients died. Suturing of linear ruptures of the cardiac mucous membrane, ligation of the left gastric artery, vagotomy and pyloroplasty are considered to the operation of choice. The recommended operative procedures are being grounded. The immediate and late results of operative treatment are described.

[Central hemodynamics in man during decompression of the lower extremities].

Experiments were performed to study the effect of leg decompression in the head-down position at -15 degrees. The method of chronic catheterization was used, pressure was measured in different areas of the cardiovascular system, blood was withdrawn for biochemical analysis. The effect of leg decompression was compared with that of lower body negative pressure. Decompression produced changes in PAP and CVP that were similar in sign but different in magnitude. The decompression-induced changes in PAP and CVP were primarily determined by the area of exposure. Using previous data, a monograph was constructed to evaluate PAP and CVP variations as a function of the decompression mode and site.
