Recent data on prognostic and predictive significance of breast cancer-associated molecular markers are presented and their clinical value in making individual prognosis and choosing adequate therapy is discussed. Markers of tumor proliferation, steroid hormone receptors, factors of epidermal cell growth, plasminogen activators and inhibitors, factors influencing angiogenesis and apoptosis and genome characteristics are considered. It is concluded that only Ki-67, ER, PR and Her-2/neu may be recommended for clinical use.

The results of organ-sparing treatment of patients with cancers of the breast, uterine cervix, endometrium and ovary are described in the paper. A prospective randomized clinical study launched in 1995 at Petrov's Research Institute of Oncology comprised cases of above 700 patients with breast cancer, around 300 women with cancer in situ and with microinvasive cancer of the uterine cervix and 83 patients with initial endometrial cancer. The results of the above treatment (segment resection + axillar dissection + radiotherapy) were shown to be similar to those obtained after Petey-Dyson mastectomy (5-year survival of 86.7% versus 88.8%, p = 0.81). The risk of local recurrence was increasing in patients with the tumors' diameter of more than 1 cm who were not treated by radiotherapy. The total regression of tumors was registered in 70% of patients with initial endometrial cancer after hormone therapy by progestagens and antiestrogens; 20% of them maintained the reproductive function.

A randomized study of two intravenous schedules of Clodronate showed that a single 4-hour infusion of 1500 mg Clodronate proved significantly more effective than the traditional 2 hours infusion of 300 mg during 5 days. The regimen may prove preferable in therapy of patients with painful bone metastases.
Wide excision of neoplasm is most frequently used in surgery for soft tissue tumors, on appropriate sites and whenever the "envelope" can be preserved. The most frequent site of pelvic girdle soft-tissue tumors was the glutal area (39.0 +/- 4.8%). Out of 100 patients with such sarcomas, wide excision of tumor was performed in 82, and tumor excision + bone resection--18. The former procedure was mostly used in the management of liposarcomas (26.5 +/- 4.2%), while the latter--in synovial sarcoma (33.3 +/- 4.9%).
