[Age-related characteristics of the structure of renal arteries].

The investigation has been performed on 337 preparations of human kidneys and on 129 clinical angiograms of kidneys at different age without renal and cardiovascular system pathology. A complex of morphological techniques, mathematical method including, has been used. Peculiarities in architectonics of the intraorganic renal arterial vessels have been described during various age periods of postnatal ontogenesis; essential variations of the renal artery index have been revealed in newborns and adolescents. They reflect functional rearrangement of the vessels during these age periods. Six periods in formation of the renal arterial intraorganic vessels have been determined, when probability of decrease and failure of their compensatory potentialities is especially great.

[Mechanisms of the development of immunodependent inflammation in silicosis].

Patients with uncomplicated silicosis were found to develop chronic immunodependent inflammation caused by an adjuvant effect of silica and characterized by autoimmune, histamine- and complement-dependent components. Reduced specific and non-specific antimicrobial defence underlies frequent complication of silicosis by tuberculosis and nonspecific infectious diseases of the bronchopulmonary system. A diagram showing the development of immunodependent inflammation in silicosis is presented.

[Anatomy of the sinoatrial node and the supply of its vascularization in man].

Seventy heart preparations of persons belonging to different sex and age have been investigated, using a complex of anatomical and histological techniques. The dimensions of the sinoatrial node (SAN) vary with age and depend on various size and form of the heart. The large atrial branch of the right and left coronary arteries supplies mainly the SAN with blood. More seldom the atrial branches of both cardiac arteries, having anastomoses, realize the SAN blood supply. The character of the SAN vascularization depends on branching variations of the atrial vessels. At the right coronary variant the sources of the SAN blood supply are the SAN branch, the right intermediate or right posterior atrial branches, and at the left coronary variant--the anterior left, the posterior left and the intermediate left atrial branches. At the even variant the SAN blood supply sources are the right intermediate and the anterior left atrial or the right posterior and the left posterior atrial branches. The data obtained can be used for comparison with the results of coronography to make a skilled analysis of clinical-roentgenological observations.

[Hemodynamic evaluation of the effectiveness of preservation of subvalvular structures in mitral valve prosthesis].

The article deals with the clinical evaluation of the results of mitral valve prosthetics with preservation of the subvalvular structures in 40 patients and comparative evaluation of the hemodynamic indices after prosthetics with preservation of the subvalvular structures of the posterior cusp and after routine mitral valve prosthetics. The authors show good clinical results in patients after valvar-preserving prosthetics of the mitral valve and essential improvement of left-ventricular contractile activity after such an operation. Comparative analysis showed that the total ejection fraction increased by 10.8 +/- 4.5% in the main group and reduced by 11.4 +/- 2.6% (p less than 0.01) in the control group. Left-ventricular end diastolic pressure did not change in the main group but increased significantly (p less than 0.05) in the control group. A conclusion is drawn that preservation of the subvalvular structures in mitral valve prosthetics is expedient even in patients with an initially grave condition because the method makes it possible to normalize intracardiac hemodynamics more rapidly in the postoperative period and essentially improve the contractility of the left ventricle.

Heart Valve Prosthesis

[Retrograde cardioplegia through the right atrium].

The results of retrograde cardioplegia through the right atrium in 50 patients are analysed. The uniform cooling of the myocardium, frequent restoration of the sinus rhythm, and the low hospital mortality rate are evidence of the high efficacy of this method of myocardial protection. Electronmicroscopy showed marked intactness of the myocardial ultrastructure and moderately manifested and reparable reperfusion changes. It is concluded that the method must be employed in coronary disease, combination of ischemic heart disease with heart valvular disease, as well as in mild forms of aortic insufficiency which cannot be corrected.

[Organ-sparing surgery in cancer of the lung].

Indications for different variants of functionally sparing operations in carcinoma of the lung were determined from study of the long term results. Comparison of 5-year survival after pneumonectomy and lobectomy showed the adequacy of the latter in advancement of T1-2N0M0 tumor (39.7% and 37.0%). In T1-2N2M0, these values were 35.7% and 34.5%, respectively. The indications for the type of bronchial resection and lobectomy (wedge, sleeve) were specified depending on the level of the neoplastic lesion of the bronchi and the condition of the intrathoracic lymph nodes. The problem of sublobar lung resections (segmentectomy, wedge, marginal) in early forms of peripheral carcinoma is discussed. The high efficacy of endoscopic removal of early forms of central carcinoma of the lung is shown on the basis of analysis of 54 cases.

[Age-related characteristics of intraorganic vascular structures of the cerebral cortex, heart, kidneys and spleen in rats in the normal state and in arterial hypertension].

The work has been performed on 40 normotensive and spontaneously hypertensive rats at the age of 6 months-1 year. In all the organs studied a direct dependence between the degree of changes in the intraorganic arterial walls and a relative content of the arterial vessels in the organ tissues has been revealed. The functional changes of the arterial bed of the organs mentioned in spontaneously hypertensive rats during hypertensive phase at the stage of stable hypertension, as a result of a prolonged adaptation transform into organic ones.

Age Factors

[Use of emergency cytological studies in bronchoscopic diagnosis of pulmonary cancer].

The article deals with the indications for emergency cytological study of central and peripheral cancer of the lung which were determined on the basis of examination of 268 patients. The significance of this method in detecting pulmonary carcinoma and reducing the number of repeated bronchoscopies is shown. A method for quick staining of the specimens is suggested and the role of emergency cytological study in complex examination of patients with cancer of the lung is shown. Emergency cytological study of material taken during bronchoscopy made it possible to verify the diagnosis in 77.7% of patients.

[Age-related characteristics of the organic angioarchitectonics of normal rats and in arterial hypertension].

Architectonics of intraorganic arterial vessels in the cervical cortex, heart, kidneys and spleen have been investigated in 40 normotensive and spontaneously hypertensive rats, 6 months and 1 year old. In all the organs studied a direct dependence has been revealed between the degree of changes in the intraorganic arteries and relative content of the arterial vessels in the tissue organs. Functional changes of the arterial vessels in the organs studied observed during the hypertensive phase, transfer into the organic ones, as a result of prolonged adaptation, when the stage of a stable hypertension takes place.

[Development of blood vessels in the human eyeball].

Blood vessels of 167 eyeballs of human embryos and fetuses have been studied, using macro- microscopical, histological and morphometrical methods. Time of appearance and differentiation of blood vessels in the eye anlage has been determined. Topography and architecture of the vitreous body artery are described in detail, as well as the vascular tunic of the lens and the pupil membrane. Regularities and time of reversal development of the vascular formations mentioned are followed during the whole intrauterine development. The data obtained add to extend our knowledge on the role of disturbances of the normal morphogenesis of the eyeball blood bed in pathogenesis of its congenital diseases.
