AIM: To study content and correlation of lipids in the serum and bile from diabetic patients with biliary diseases (BD). MATERIAL AND METHODS: Serum and bile lipids were studied biochemically in 125 patients with BD and diabetes mellitus and 87 healthy control subjects aged 20-59 years. Serum lipids were studied biochemically as well as content and correlation of duodenal bile lipids. Correlation was investigated between blood and bile lipids. RESULTS: Compared to healthy controls, diabetic patients with chronic cholecystitis and cholelithiasis had higher serum total cholesterol, triglycerides (by 28-34% and 63%, respectively) and a 18-22% decrease in HDLP cholesterol. Bile of such patients is oversaturated with cholesterol. Patients with diabetes type 2 had a close direct correlation between levels of total cholesterol, LDLP in the serum and Thomas-Hofmann index. CONCLUSION: BD patients with diabetes mellitus had high levels of serum lipids and high bile concentrations of cholesterol. Serum and bile lipids concentrations direcly correlate.
The aim of the study was to investigate manifestations of biliferous tract diseases in patients with diabetes mellitus (DM) and to measure concentrations and the ratio of lipids in gall-bladder and liver bile in these patients. 125patients with DM and biliferous tract diseases were examined. Patients with chronic cholecystitis (CC), cholelithiasis, and biliary dyskinesia underwent clinical examination as well as biochemical and instrumental tests. The control group included 87 healthy individuals of comparable age and gender. CC was diagnosed in 35.2% of DM patients, cholelithiasis - in 17.6% patients. In liver and gall-bladder bile samples taken from patients with CC and cholelithiasis, fatty acids concentrations was not significantly different, cholesterol level was significantly higher, and phospholipids level was lower than in the control group. Thus, indices of cholesterol saturation in liver and gall-bladder bile portions were higher in patients with cholelithiasis than in the control group. The study revealed high biliferous tract diseases morbidity in patients with DM. Thomas-Hofmann index, which characterizes bile saturation with cholesterol, is significantly higher in patients with cholelithiasis and DM, then in healthy individuals.

AIM: To study morphofunctional peculiarities of gastric mucosa in native and migrant rural population of North Siberia. MATERIAL AND METHODS: A comprehensive clinicomorphological examination including physical examination, ultrasonic scanning, cholecystography, duodenal intubation, intragastric pH-metry, esophagogastroduodenoscopy, quantitative and qualitative investigations of gastric mucosa was conducted in 148 migrants (Russians, Ukrainians) and 165 evenks living in Evenkia. RESULTS: In both population groups, more often in the migrants, in cholecystitis vs dyskinesia patients frequency of atrophic changes in gastric mucosa was higher while acid production in the stomach was lower, antral gastritis more active. CONCLUSION: Morphofunctional condition of the stomach influences the condition of the bile tract in native and migrant population of North Siberia.

THE PURPOSE: To reveal the structural basis of protective influence of the natural ceolite enterosorbents in fluorine intoxication. METHODS: Fluorine effect was realized by exposure of 15 rats of Vistar's line in electrolysis shops of an aluminium plant. 15 animals were in normal ecological conditions and consisted the control group. The both groups received the natural ceolite. In the all rats the hysto-stereo-metric structure of intestinal mucosa has studied. RESULTS: The use of ceolite promoted to decrease of necrosis volume and to increase of immune and extrasecretory structure activity in experimental animals after fluorine effect. THE CONCLUSION: The possibility of ceolite enterosorbent use is substantiated as protective means lowering the toxic fluorine effects.

Our studies demonstrated that chronic opisthorchosis with a skin syndrome is accompanied by a severer clinical course. The course of rehabilitation measures with the use of resort recreation (mud-bad cure) combined with traditional treatment stages makes it possible to considerably enhance the efficiency of treatment of both opisthorchosis and its skin manifestations, and should be applied more often in practice.
AIM: To study association between Helicobacter Pylori infection in ulcer in native population and migrants in Siberia. MATERIALS AND METHODS: Epidemiological survey of ulcer has been performed in Northern (Evenkia, Yakutia) and Southern (Khakasia) regions of Siberia. Clinical examination covered 3908 Europeoids (Russians, Ukrainians) and 1999 Mongoloids (Evenks, Evens, Khakases). Endoscopic examination was made in 2144 patients. Light microscopy has detected Helicobacter pylori (HP) in 453 patients (215 migrants and 238 natives). RESULTS: Prevalence of ulcer in the South of Siberia was 6.1 and 1.5% for migrants and natives, respectively. In the North--8.9 and 0.6%, respectively. HP was diagnosed in 83.7% of Mongoloids and 69.2% of Europeoids in the South and 92.2 and 100%, respectively, in the North. HP index, contamination and adhesion in migrants were higher than in native population. CONCLUSION: Siberial Europeoids and Mongoloids have specific ethnic features of ulcer occurrence and its association with HP. Native population is protected better from the pathogenic bacteria. Further study of the causes of virulent bacterial properties and mechanisms maintaining long persistance of HP in the stomach is necessary.
AIM: To investigate the relationship between Helicobacter pylori infection and ulcer in population of Khakasiia. MATERIALS AND METHODS: An epidemiological study of dyspepsia and ulcer prevalence in Khakasiia population covered 1052 patients (564 residents and 488 immigrants). 339 patients were examined endoscopically. Helicobacter pylori (HP) was detected in gastric mucosa using light microscopy after Giemsa staining in 214 patients (104 immigrants and 110 residents). RESULTS: Prevalence of ulcer and HP were 6.1, 1.5% and 69.2 and 88.9% in immigrants and residents, respectively. HP index, degree of dissemination and adhesion in gastric mucosa of immigrants with duodenal ulcer were higher than in controls. The same but less manifest tendency was in the residents. CONCLUSION: The population of the Siberian South has ethnic features of ulcer prevalence and pathogenesis. It is thought more valid to study mechanisms of ulcerogenesis through quantitative rather than qualitative characteristics of HP infection.
Biochemical composition of the bile and blood serum was investigated in Evenkia and Yakutia residents and immigrants. Blood lipids in the immigrants were much higher while bile lipids lower than in the natives. It is suggested that normolipidemia in the residents may be determined by intensive synthesis of biliary acids and other lipids in the liver and their secretion into the bile. Later, high content of primary cholates (17%) was found in the bile of residents which was not confirmed in the immigrants.
AIM: The study of biochemical bile composition in Asian North migrants with biliary diseases (BD). MATERIALS AND METHODS: The diagnosis of BD was made upon clinical examination, ultrasound scanning, duodenal intubation in 329 examinees. The bile was examined for total bile acids, total cholesterol, total phospholipids and bile acid spectrum. RESULTS: Cholelithiasis patients were found to have high index of bile cholesterol saturation, high level of deoxycholic acid and low ratio primary/secondary bile acids. CONCLUSION: Changes in bile lipid spectrum are the leading cause of forming bile concrements in migrant population of the high altitudes.

Biliary tracts were investigated in 3420 migrants and 1445 aborigines (Evens, Evenkis) living in Evenkia and Yakutia. Acalculous chronic cholecystitis and cholelithiasis were found in 8.5 and 8.8% of the migrants, respectively. In aborigines these diseases occurred in 4.5 and 1.5%, respectively. The risk to develop biliary disease for migrants grows after 4-5 years of stay in the Far North. It is concluded that biliary pathology in the Far North population was related to ethnic and geographical factors.
Altogether 156 Evenks and 162 Evens were examined in the reindeer regions of Evenkia and the Yakut Republic located in the extreme north of Russia. Seasonal changes in the concentration and ratio of bile lipids were discovered. The conclusion is made that space and time coordinates are of importance for the diagnosis and treatment of biliary tract diseases in northern nations.
An epidemiological survey demonstrated that biliary disease detection in aborigines of the Far North is more frequent in subjects with nontraditional way of life. Chief biliary disease factors different in importance for Evens and Evenks are as follows: prior viral hepatitis, obesity, alcohol abuse. Incidence rate for different biliary diseases is not uniform in Far North native population. This may be explained by specificity of biliary functions.
Gallbladder function and lipid composition of bile were examined for the first time in the indigenous population of the extreme north of the USSR. Bile from Evens and Evenki was not found to be oversaturated with cholesterol, with motor function of the gallbladder being characterized by hypermotility. This is of interest, since there are data on the protein-lipid nature of nutrition in aborigines of the North.
