[Reconstruction of a crushed chest with HI-TEX PARP NT implant].

The authors present the case of a 57-year-old man with polytrauma, who was injured in a train accident. Together with other injuries, he also sustained multiple comminuted fractures on the left half of the rib cage. These subsequently required surgical intervention because of thoracic instability and impending lung injury. Extensive damage to the thoracic skeleton was treated by removal of the damaged tissues and replacement by the HI-TEX PARP NT implant. The presented procedure is being discussed as an alternative to metallic fixation in thoracic instability in cases of devastating injury or injury resulting in bone and tissue loss.

[Hartmann's resection of the large intestine--a method still useful].

UNLABELLED: Hartmann's procedure is an operation indicated especially in emergency cases for left-sided colonic pathology where primary anastomosis is considered unsafe. METHODS: Retrospective study 95 patients who underwent Hartmann's procedure over a 5 year period in the Surgical Clinic of the University Hospital in Pilsen. RESULTS: 81 (85%) operations were emergency procedures. 45 (47%) operations were done for the malignant disease, the remaining 50 (53%) for benign disease (33 for complications of diverticular disease). Median age of patients was 69.4 years. The 30-day mortality rate was 16.8%. 37 patients had colostomy reversed, after a mean of 5.6 months with no mortality. CONCLUSIONS: Hartmann's procedure is a safe method. Our results are similar to results collected by foreign clinics.

[Various types of pelvic lymphadenectomy--surgical options--comparison of their significance].

Based on recent literature data, the authors summarize contemporary trends in the surgical therapy for carcinomas in the pelvic region, in particular a significance of the pelvic lymphadenectomy, which has become a standard part of the treatment of the above mentioned disorders. Furthermore, they bring forward new trends in research on lymphatic mapping and the sentinel lymphonode detection.

[Esophageal stents].

The method of oesophageal stents application using a surgical procedure via gastrotomy for otherwise surgically unmanageable malignant stenoses of the oesophagus and the gastric cardia, has been used for several decades. Once the self-expandible endoprostheses, applied under either the endoscopical or radiological (RTG) control, or both, and following a change in the oesophageal malignities treatment strategy, the spectrum of indication groups of the patients, who would undergo the stent implantation, have enlarged. Recently, thank to the endoscopically removable endoprostheses, the authors broaden the spectrum of use to benign findings of the oesophageus and the gastric cardia (strictures, perforations, insuficiency of anastomoses), as well. In their work, the authors present a group of patients who have had the oesophageal stent, using the endoscopic method, implanted at the Surgical Clinic of the Faculty Hospital in Plzen since 1992.

INAA and PIXE of atmospheric and combustion aerosols.

Using instrumental neutron activation analyses and photon-induced x-ray emission techniques for analysis of size-fractionated atmospheric and combustion aerosols and other emission samples arising from fluidized-bed combustion of North Bohemian lignites up to 42 elements were determined in all samples types. This allowed the evaluation of element enrichment, time trends, and inter-element correlations and the performance of factor analysis of various fractions of atmospheric aerosols. The data obtained on mass and element size distributions of aerosols and emission samples obtained upon lignite combustion in an experimental scale atmospheric fluidized-bed combustor without and with added hydrated lime and limestone were used to elucidate the mechanism of abatement of toxic trace and matrix elements from flue gas.

[Severe injury of the tracheal bifurcation].

The authors demonstrate a rare case of serious rupture in the area of the tracheal bifurcation with incomplete rupture of both bronchi which occurred after a severe multiple injury. The authors describe the difficult surgical solution, using temporary selective intubation of both main bronchi across the surgical field. The imperfect success of jet ventilation is ascribed to severe pulmonary contusion and major haemorrhage from the trachea which increased the risk of barotrauma. The authors emphasize the importance of peroperative and postoperative bronchoscopy.

The nosocomial and occupational hepatitis type B infection in a chest disease hospital. A prospective study.

The results of a prospective study of hepatitis B infections among the patients of a Chest Disease Hospital are reported. A thousand and sixteen patients admitted during the twelve months study period were screened for the Hepatitis B(Surface)-Antigen(HBsAg) on admission and during the stay and were kept under epidemiological surveillance for six months following the discharge. The HBsAg prevalence was 3.54% on admission and the cumulative infection rate during the follow up period was 12.64 per 100 patients. The HBsAg prevalence on admission was significantly higher in males than in females, the susceptibility to infection found in the course of the study was in both sexes similar. The majority of infections was inapparent with the ratio of 8.6:1 of inapparent to clinically manifested forms. The incidence of viral hepatitis among the personnel was comparable to the incidence among the personnel of other hospital departments.

Hepatitis B (surface) antigen and viral hepatitis in family contacts of acute hepatitis B patients.

The results of a follow up study among family contacts of acute hepatitis B patients are reported. The family members were tested for HBsAg in the time of hospitalization of the index case and after three months again, the families were kept under epidemiological surveillance for six months. 19 (4,54%) out of the 418 persons screened for the first time were HBsAg-positive. The highest prevalence of HBsAg-positive persons was found in the families of patients without history of parenteral exposure. 5 (1%) of family contacts were hospitalized from 2 to 4 months following admission of the index case. All were the spouses of index case patients and none of them had other history of parenteral exposures than blood sampling for purpose of this study. 2 (0,58%) cases of silent antigenaemia were found among the 344 persons examined at the second screening.
