Salvage treatment for sarcomas of the hand.

We report 6 patients (1 male and 5 females; age range, 13-77 years) with hand sarcoma. The lesions were located between the digits and the distal end of the radius. A painless mass was the common manifestation in these patients. An incisional biopsy was performed on every patient, and histology revealed two low-grade malignant sarcomas and four high-grade malignancies. A surgical salvage procedure was performed. The 4 patients with high-grade malignancy underwent adjuvant therapy such as radiotherapy and/or chemotherapy. Four patients are still alive and exhibit acceptable function in the affected hand. Our surgical procedure is presented.

Tuberculous infection of the hand and wrist.

The authors report on 23 patients treated for tuberculosis of the hand and wrist during the past 20 years. Duration of symptoms prior to diagnosis ranged from several months to years. Swelling and pain on motion were common symptoms, as was increased sedimentation rate. Granulomatous inflammation was seen on paraffin section in most of the patients, however tissue cultures were positive for Mycobacterium tuberculosis in only 6 patients in this study. Treatment of these patients were divided into medical and surgical treatments. The incisional biopsy and antituberculous drug were carried out and administered in the medical-treated group for 2 years. The surgical synovectomy as well as debridement and administration of antituberculous drugs were carried out on the surgical group. The results showed no recurrence in this study. Most of the patients showed improved symptoms and an increased range of motion. In conclusion, this treatment resulted in good recovery of function with low morbidity.

Replantation of the avulsion of the scalp: a case report.

We reported a case of successful replantation of the avulsion scalp after 11 hours of ischemic time. The surgical management and postoperative management were presented. The fourteenth month follow-up result was satisfactory.

Effect of aloe vera gel to healing of burn wound a clinical and histologic study.

In a study of twenty-seven patients with partial thickness burn wound, they were treated with aloe vera gel compared with vaseline gauze. It revealed the aloe vera gel treated lesion healed faster than the vaseline gauze area. The average time of healing in the aloe gel area was 11.89 days and 18.19 days for the vaseline gauze treated wound. Statistical analysis by using t-test and the value of P < 0.002 was statistically significant. In histologic study, it showed early epithelialization in the treated aloe vera gel area. Only some minor adverse effects, such as discomfort and pain were encountered in the 27 cases. This study showed the effectiveness of aloe vera gel on a partial thickness burn wound, and it might be beneficial to do further trials on burn wounds.

A study of simplified porcine skin preservation.

The study of preserved porcine skin was done by using split-thickness skin grafts of pigs weighing approximately 25 to 30 kg. The harvesting of split-thickness skin grafts was performed under aseptic and antiseptic conditions. The povidone-iodine solution and 0.25% acidified sodium hypochlorite solution were used before immersion of the skin in 90% glycerine solution. Prepared skin graft was stored in the freezer compartment of a home refrigerator. Serial studies of the bacteriology and the histology of specimens were performed monthly for 12 months. No bacterial organisms were found during the study. The integrity of the skin lasted up to 5 months, however, the property of the preserved skin was as good as the other biological dressing even after 6 months. This simple, low-cost procedure is useful for restoring allografts or even autografts in a hospital without conveniences or adequate facilities.

Immunological and immunopathological studies of patients with burn scar.

Sera obtained from 5 cases of burn scars (patients group 1) and cases of burn scars with squamous cell carcinoma (patients group 2) of the skin are analysed for anticollagen antibodies. Anticollagen type VI was demonstrable in the serum of one out of 5 cases in group 1 and 4 out of 6 cases in group 2. Collagen type VI was demonstrable in the scar without carcinoma of one patient whose serum contained anticollagen type VI. It is suggested that collagen type VI in the scar and their immunobiologic reaction may be related to the regulation of tumor development in scar tissue of the skin.

Tuberculous infection of the hand and the wrist.

Twenty cases of tuberculous infection of the hand and the wrist from the division of plastic reconstructive surgery over the past 17 years. There were fourteen males, and six females. The ages of the patients ranged from 10 years to 80 years, with an average age of 50.1 years. The follow-up time was not less than 3 years. The characteristic manifestation of this diseases was insidious onset, and run chronicity. Painful swelling of the wrist and fingers as well as increased sedimentation rate were frequent findings. However, confirmed histopathological diagnosis is essential. Surgical debridement together with triple anti-tuberculous drugs are recommended. Triple drugs therapy should be given for not less than two years to prevent drug resistance and recurrence of the disease.

The preparation of lyophylised of fetal membrane for biological dressing.

The preparation of lyophylised fetal membrane was performed between November 1982 and May 1984, and the membrane was applied to a variety of wounds. The result of the study was impressive, the biologic dressing property did fairly well, and the utilization of this fetal membrane was convenient in time and place.

The Krukenberg procedure in the bilateral amputee after electrical burn.

Nine Krukenberg procedures were performed in 5 bilateral amputee patients suffering severe electrical injuries of both hands and wrists. The surgical procedure yielded functioning pincers in 7 of 9 operations. The patients could perform their daily activities very well and showed pronounced improvement in their self-confidence, self-reliance, and quality of life. The postoperative rehabilitation of these patients was not difficult. The Krukenberg procedure should be considered a substitute for prosthesis, especially in developing countries.

Protothecosis following hand injury. A case report.

A case of protothecosis following an accidental cut on the dorsum of the hand by a fragment of glass from an exploding beer bottle is described. The area was widely excised and oral tetracycline given. No recurrence has been seen over a period of four years.

Constriction band syndrome.

Thirty cases of constriction band syndrome diagnosed and treated with surgery at Ramathibodi Hospital, Bangkok, during 1973 to 1986 are reported. The incidence of this congenital anomaly could not be verified in this series because the number of cases was limited and the cause could not be concluded. The less frequent site of constriction band was the thumb, but acrocephalosyndactyly was the most common associated anomaly. All 30 cases were treated with surgery. One-stage Z-plasty encircling the limb was performed for those with pure constriction band. Cases of acrocephalosyndactyly were treated with staged separation of the digits as well as local flap and skin grafts. Good results were achieved in 16 of 20 patients with constriction band alone and in 4 of 10 acrosyndactyly cases. No compromised circulation of the distal limb or total flap loss was encountered in this study. This type of surgery certainly gives encouraging results.

Squamous carcinoma in scars: clinicopathological correlations.

Twelve cases of squamous cell carcinoma arising in scars of the skin are analyzed. Correlation of tumor aggressiveness, tumor differentiation, intensity of small lymphocyte infiltration, and nature of adjacent collagen tissue to tumor stroma was investigated. All tumors were predominantly well differentiated. Nine patients had no metastasis or recurrence after surgical treatment of the primary tumor. Three patients developed widespread metastases and died less than 2 years after operation. The tumors of these latter patients contained areas of pleomorphic and spindle cells and had minimal infiltrates of small lymphocytes. In contrast, the 9 patients who remained free of tumor had moderate infiltrates of small lymphocytes, and in just 1 of 9 was an area of pleomorphic tumor cells found. Prognosis in scar carcinoma, therefore, may be related to tumor differentiation and the degree of host response with small lymphocytes to the tumor, rather than to the universally poor prognosis recorded in the past.
