[Indications for the surgical treatment in bronchiectasis and postoperative results].

During Jan. 1990 and Dec. 2002 a total of 14 patients with bronchiectasis were operated on. They were 5 female and 9 male with mean age of 35.7 years. In two of them (14.3%) the disease was with bilateral localization. Patients were typically presented with cough and purulent sputum production 12(85.7%), recurrent pulmonary infections 8(57.1%), halitosis 6(42.9%) and hemoptysis 4(28.6%). The mean duration of clinical manifestations was 8.4 years. The indications for surgery were failure of medical treatment in 10(71.4%) patients, hemoptysis in 2(14.3%) and lung abscesus, as a complication in 2(14.3%) patients. The operative methods included 9(64.3%) lobectomies, 1(7.1%) pleuropneumonectomy, 1(7.1%) pneumonectomy, 1(7.1%) bilobectomy with broncho-esophageal fistula resection, 1(7.1%) middle lobectomy with polysegmental resection S7-10 and 1(7.1%) polysegmental resection S7-10. No 30-day postoperative death was faced. Major complications, followed by rethoracotomy, were found in 2(14.3%) of the cases. The postoperative results were excellent in 8(57.1%) of the patient, very good in 5(35.75) of them and in only 1 patient there was no clinical improvement. In conclusion, surgery for bronchiectasis is followed by acceptable postoperative complications and better long-term result after radical operation.

[Lung metastases from tumors at different sites].

A comparative study of necropsy material is carried out for the period of 1974-1983. For these ten years 11603 autopsies are performed. In 222 cases neoplastic processes with different lung metastases are obligatory, which makes 1.8% of total number of cases. The primary sites of these metastases are as follows: tumor of the stomach (26), breast (33), bile duct (10), bile ways (3), liver (13), large intestine (30), incretory glands (31), etc. The morphological evolution shows that the histologic type of the primary tumors varies (carcinomas, malignant lymphomas, teratocarcinomas, sarcomas, neuroblastomas, etc.). In some cases (carcinomas of the kidneys and liver, sarcomas of the bones) the histologic type of the primary tumor is identical to that of the Metastatic processes in the lungs. In some of the lung metastases however there are differences in the histological structure compared to the primary tumors. A clinical morphological comparative study is performed.

[The morphological diagnosis of alveolar lipoproteinosis].

The authors report on diagnosis of two cases with alveolar lipoproteinosis, using materials from transbronchial lung biopsy and bronchoalveolar lavage. Light microscopic examination of the materials showed accumulation of fine-grained substance in the alveolar cavity which was stained intensively in red after PAS reaction and toluidin blue. Heterogeneous material was established in the alveoli by the electro microscope. Noncellular components were represented by single round membranes, not large parts of tubular myelin or osmiophilic strata bodies. The cellular component was presented mainly by alveolar macrophages with not well developed lysosomal apparatus (primary lysosomes), slightly manifested elements of Golgi complex, moderately developed cytoplasmatic reticulum. Many of the macrophages had preserved phagocytic activity-presence of pino- and phagocytic vacuoles.

[The pathomorphological changes in the pleura of rabbits with artificial pneumothorax and uropolin pleurography].

The results from artificial pneumothorax in 25 rabbits with intrapleural administration of Uropolin (10 rabbits) as well as of 10% solution of NACl (7 rabbits) were described. Thoracotomy was made on 7th, 14th and 12 21st day and the parietal, diaphragmatic and visceral pleura with underlaying tissues were examined. Proliferation of the mesothelium [correction of mesother] and growth of the connective tissue, which were more manifested in rabbits treated with Uropolin, were established histologically. The authors think that pathohistological examinations confirm the clinical observed favourable effect of intrapleural administration of Uropolin for obliteration of the pleural cavity.

[Determination of the optimal conditions for explantation of portions of the liver].

The authors explanted liver organ cultures from mice, 1 to 3 months old, of the BALB/cY strain in three types of nutritive medium-pure nutritive medium 199; combined nutritive medium "A" (80% nutritive medium 199 and 20% of bovine serum as 400 ml of sol. glucose 40%, 7 mg of vit "C" and 20 mg of glutamine were added to 100 ml of the medium) and combined nutritive medium "B" (400 mg of sol. glucose 40% was added to 100 ml of the nutritive medium 199). The usage of the combined nutritive medium "B" was the most suitable for continuous culture (42-48 days) of liver explantants with preservation of the morphological intactness of the tissue. They examined also the blastomogenic action of 0.200 mg/ml od diethylnitrozoamine on liver organ cultures. It was established that 0.200 ml/ml of diethylnitrozoamine was nontoxic dose on liver organ cultures, but without manifested blastomogenic effect.
