Diagnosis and treatment of cortical tumours of the suprarenal gland.

The authors report on their own groups of benign (20 patients) and malignant (14 patients) cortical tumours of the suprarenal gland. In both kinds of tumour the occurrence is higher in females. Adenoma occurred more often in the left adrenal gland and cancer in the right one. Some tumours manifested themselves by an increased production of suprarenal hormones, others were hormonally inactive and did not become manifest until the later stage of development. In the therapy of adenomas the classical lumbar approach through the 11th intercostal space is adequate, in the case of cancer and extended lumbotomy laparotomy or thoracotomy is necessary. The need of participation of other specialists in the treatment (endocrinologist, radiologist, oncologist, anaesthesiologist) is emphasized.

Adenoma

["Surveillance" of non-seminomatous testicular tumors. A prospective study of 100 patients in the first stage of illness].

In a prospective study, 100 patients with clinical stage I nonseminomatous testicular tumours were investigated by a surveillance policy, which consisted of regular follow-up after orchiectomy alone until disease progression was confirmed. Follow-up revealed that 65 of the 100 patients were free of disease at 25 to 90 months after orchiectomy. Disease progression was detected in 35 patients at 3 to 27 months following orchiectomy. Patients with progression were treated by PVB chemotherapy. Patients with non-seminomatous testicular tumours in clinical stage I represent a group which is not homogeneous and can therefore not be treated by means of a single modality. It is necessary to individually define optimal treatment based on prognostic factors. The surveillance policy is available in patients without risk factors and regular follow-up is mandatory. Multidisciplinary collaboration and cooperation with patients are essential.

[Surgical treatment of residual pulmonary metastasis of testicular germ cell tumors after chemotherapy].

Advances in chemotherapy achieved a high percentage of complete remission after primary cytostatic treatment in germinal testicular tumours. In 30-40% of patients in the IVth stage of the disease supplementary surgery is, however, necessary--retroperitoneal lymphadenectomy or metastasectomy of the lungs. The authors give an account of initial results of surgical removal of metastases in the lungs in seven patients of 48 treated with cytostatics on account of a germinal testicular tumour. The low risk of thoracotomy, if correctly indicated, justifies us to describe this operation not only as auxiliary but therapeutic.

[Antegrade pyeloureterography in the transplanted kidney].

Antegrade pyeloureterography performed via fine puncture needle yields detailed imaging of the anatomy of the renal pelvis, calices, and ureter of the transplanted kidney. It is a simple and safe examination method providing information which, as a rule, can not be obtained by urography, radioisotope gammagraphy or ultrasonography. Puncture of the collecting system of the transplanted kidney with subsequent pyeloureterography is indicated for determining the site and nature of the obstruction in the ureter in urinary stasis in the transplanted kidney, for diagnosing urinary fistulas, for functional examination of the dilated upper urinary tract, as well as for percutaneous treatment of urological complications after kidney transplantation.

[Factors affecting the recurrence of epithelial tumors of the urinary bladder].

Of the 178 patients with superficial tumors of the urinary bladder followed up over minimally three years from the onset of treatment, 137 (77%) developed relapse of the tumor. Comparison of anamnestic data and objective examination findings in patients with and without relapses yielded a statistically significant probability of relapses in patients with multiple tumors, in tumors with an increased number of cell layers, and in tumors with infiltration of the lamina propria. Patients presenting with any of these features are at risk of relapses and require prophylactic treatment.

Prognosis of patients after retroperitoneal lymphadenectomy for clinical stage I nonseminomatous germ cell tumour of the testis.

A retrospective analysis of patients with the clinical stage I nonseminomatous germ cell tumour of the testis was performed in order to test the reliability of clinical diagnostic methods in assessing the extent of the disease, morbidity, relapse rate, and survival after retroperitoneal lymphadenectomy, and in an effort to pinpoint the risk factors leading to relapse. A total of 69 patients with clinical stage I nonseminomatous germ cell tumours of the testis underwent retroperitoneal lymphadenectomy. Metastases in the retroperitoneal space were found in 8 [11.6%] patients. Immediate postoperative complications were observed in 7.2% of patients. Loss of ejaculation was found in 52.9% of patients after bilateral, and in 21.4% after unilateral retroperitoneal lymphadenectomy. Relapse was noted in 5 out of 61 [8.2%] patients, with the relapse invariably localized in the lungs. Of the 69 patients, 65 [94.2%] are alive, free of signs of the disease at an average 5.4 years since initiation of treatment. The study did not find a correlation between relapse and histologic findings and/or the extent of the germ cell tumour.

Urinary excretion of vitamin C in chronic renal failure and after renal transplantation.

Urinary excretion of vitamin C was investigated in 44 patients with chronic renal diseases and in 25 patients after renal transplantation with various mean glomerular filtration rates. In both groups a hyperbolic relationship was observed between FE vitamin C and CCr. In addition, direct relationships were found between FE vitamin C and FE Na, FEK and FE H2O in both groups. Various chronic renal diseases and treatments had no influence on the investigated relationships. In 16 healthy subjects urinary excretion of sodium during maximal water diuresis did not increase but urinary excretion of vitamin C significantly increased. In 10 patients in the polyuric stage of chronic renal failure without dialysis treatment during Giordano-Giovanetti-Maggiora diet with addition of sodium chloride the urinary excretion of sodium increased but that of vitamin C was not influenced. The results obtained for the biochemical parameters tested suggest that the urinary excretion of vitamin C depends on the urinary excretion of water.

The role of tumour markers and lymphography in determining clinical stage of non-seminomatous testis tumours.

Findings established by means of tumour markers examination and lymphography were compared with histological findings in retroperitoneal lymph nodes removed during lymphadenectomy. Overall accuracy of tumour markers was 70.1%, sensitivity was 71.4% and specificity was 67.9%. Overall accuracy of lymphography was 79.4%, sensitivity was 80.7% and specificity was 77.5%. Combined results of tumour markers and lymphography showed overall accuracy 73.1%, sensitivity 93% and specificity 37.5%. Since neither the results of tumour markers examination, nor lymphography are sufficiently sensitive in order to determine exactly the presence of metastases in regional lymph nodes, retroperitoneal lymphadenectomy must be considered in the present for a method which is not only of therapeutic, but also of diagnostic and prognostic importance.

Long-term results of treatment of urethral strictures by transpubic urethroplasty.

Intrapelvic ruptures of membraneous urethra connected with pelvis fractures lead, as a rule, to strictures. Relative inaccessibility of these strictures above diaphragma urogenitale and behind symphysis makes their surgical treatment difficult. Transpubic approach with removal of wedge of pubic bones enables a direct approach to stricture and its modification under sight control. Technique of transpubic approach is described and long-term results obtained in 10 patients being 29-54 months after operation are evaluated. In one patient urinary continence in a sense of stress incontinence was disturbed, in one patient impotence occurred and one patient had disturbed gait.

Treatment of retrocaval ureter.

Retrocaval ureter is a developmental anomaly of the course of the ureter in relation to the inferior vena cava. In spite of the fact the pathophysiological consequences of this anomaly are very serious, clinical manifestations are relatively poor. Experience with the management of this disorder in our 4 patients justifies the therapy we applied. It involves resection of a part of the ureter running behind the inferior vena cava, transposition of the ureter and subsequent end-to-end anastomosis of both parts of the ureter or of the ureter directly with the pelvis. The results verified at postoperative check-up examinations were very good, demonstrating the correctness of the tactics and procedure used.

Occurrence of adenocarcinoma in the colon sigmoideum following Maydl's operation for urinary bladder exstrophy.

Two cases of colonic adenocarcinoma appearing 44 and 25 years, respectively, following an operation for urinary bladder exstrophy done according to Maydl are reported. A need for regular controls of patients with inner urine derivations has been emphasized. In the case of a suspected tumour it is necessary to alter the inner derivation to an outer one and also a resection of the colon at the site of the urinary bladder trigone is required. The Czech surgeon Karel Maydl was the first to implant the trigone of a splitted urinary bladder into the colon sigmoideum in 1892. This type of operation is used even today by several European urologists with excellent long-term results with respect to the preservation of an intact ureterovasical passage which prevents the reflux of the intestinal contents into the ureter, and also prevents the formation of strictures in the terminal parts of the ureter. In two patients who had been living for 25 and 44 years since they were operated on for urinary bladder exstrophy according to Maydl, a colonic adenocarcinoma was diagnosed. The tumour directly affected the implanted trigone wall and also involved the surrounding parts of the colon wall.

Formation of calcium oxalate urolithiasis in experiment and its inhibition by pyridoxine and magnesium.

Possibility of application of experimental model of calcium oxalate urolithiasis to the study of some medicament actions useful in prevention of calcium oxalate urolithiasis in men has been examined in animal experiments. Inhibition effects of pyridoxine and magnesium in formation of calcium oxalate stone formation have been proved. Administration of pyridoxine and/or magnesium to experimental animals inhibited formation of incrustations on renal papillae. Administration of nothing but magnesium lowered considerably also frequency of nephrocalcinosis occurrence. Good reproducibility of applied model of calcium oxalate urolithiasis creates conditions for further experimental study of this kind of urolithiasis. In the previous series of experiments (4) we tiped out an experimental model of calcium oxalate urolithiasis in animals well reproducible in our conditions. In the following period we were interested, whether the created model can be applied to verification of efficiency and further study of some medicaments affecting this kind of urolithiasis. At present, pyridoxine and magnesium are mode widely used for prevention of calcium oxalate urolithiasis in the clinical practice. Therefore, we decided to use the possibilities of the evaluated experimental model for the study of their inhibition effects. The results of experiments are presented in this paper.
