[The treatment of HIV infection].

The combination of zidovudine (AZT) + lamivudine (3TC) twice a day + tenofovir once a day, in HIV-infected, antiretroviral-naive patients leads to the rapid and prolonged reduction in viral load, with good immune response and reasonable tolerance profile. According to the results of the boosted FosAmprenavir solo trial, adherence was simplified by the reduced number of daily administration and tablets (once a day) and hence a decline in virological failure rates. Enfuvirtide, an antiretroviral inhibiting the fusion of the virus membrane and cell, combined with optimal antiviral therapy, leads to virological response in 78% of patients and prolonged for more than 6 months in 33%, with good tolerance. In patients treated with neviparine, the frequency of hepatotoxicity justifies early and rigorous monitoring of the hepatic enzymes, notably in the case of high transaminase levels before initiation of treatment.

[Administration of an antiretroviral therapy, advice from experts].

Before changing antiretroviral regimens, the treatment team must review the treatment history, establish the context, assay plasma concentrations of antiretrovirals, genotype viral resistance, and if necessary reinforce adherence. Changing treatment that is successful from an immunological and virological standpoint may be indicated in the case of severe adverse events, which may lead to poor adherence, or to reduce long-term, notably cardiovascular, complications, or to simplify treatment. Virological failure can lead to the adaptation of treatment to the genotype or to the prescription of multiple combinations (7 or 8 drugs) or to the recycling of some drugs. Tolerance affects adherence. Improvement in the latter can be obtained by simplification, which should be attempted only once one is sure of the virological efficacy.
