Coagulation, platelets, and acute pancreatitis.

Acute pancreatitis generates a complex cascade of immunological events that affect the pathogenesis and the progression of this disease. Several inflammatory mediators seem to play a critical role in the pathogenesis of pancreatitis and the subsequent inflammatory response. In turn, these mediators can influence hemostasis. Coagulation abnormalities occur in acute pancreatitis and are related to its severity. The contribution of blood platelets in the disturbed hemostasis in acute pancreatitis, although extensively studied, remains obscure. This article reviews the local and systemic implications of hemostatic abnormalities during acute pancreatitis. Furthermore, we discuss the prognostic value and the potential therapeutic implications of platelet activation and other hemostatic variables.

Comparison of stroke among Christians and Muslims in Thrace, Greece.

INTRODUCTION BACKGROUND AND PURPOSE: Previous studies have demonstrated significant racial differences in incidence, age of onset, outcome and subsequent risk factors of stroke. However, there is scarce evidence available for Muslim populations worldwide. Xanthi region, a mainly rural and remote area in northern Greece, which is characterized by a mixed Christian/Muslim population, is considered appropriate for further studies in this area. METHODS: A prospective clinical study, based on a cohort of 530 consecutive unselected patients, 366 Christians and 164 Muslims, who had their first documented episode of stroke between 1998 and 2002. Chi-square and Fisher's exact test for comparison between discrete parameters, Student's t-test for comparison between quantitative parameters and Pearson's r for correlations (along with Bonferroni's adjustment) were used. A multiple logistic regression model was performed for evaluation of religion and gender as independent predictors of type of stroke. RESULTS: Muslims, when compared with Christians: (1) present with ischemic stroke less often (p = 0.049); (2) suffer from stroke at a younger age (67.26 +/- 9.77 vs 71.43 +/- 10.88 years; p < 0.001); (3) show a significantly lower incidence rate (87.2 vs 173.9 cases/100,000 person*years; RR 1.994; p < 0.001); (4) have lower rates of diabetes (p = 0.019) and atrial fibrillation (p < 0.001) and (5) present similar mortality (p = 0.347). CONCLUSIONS: Lower diabetes and atrial fibrillation rates among the Muslims of Thrace, Greece, might account for their low incidence of stroke. The contribution of culture-related habits (diet, occupation) remains to be investigated in further studies.

Desmoid tumor in Gardner's Syndrome presented as acute abdomen.

BACKGROUND: Gardner's syndrome can occasionally be complicated with intra-abdominal desmoid tumor. These tumors usually remain asymptomatic but can exhibit symptoms due to intestinal, vascular and ureteral compression and obstruction. CASE PRESENTATION: A rare case of a 41-year-old male patient with Gardner's syndrome complicated with intra-abdominal desmoid tumor, which first presented as acute abdomen, is presented. CONCLUSION: Extra-abdominal manifestations of Gardner's syndrome along with a palpable abdominal mass would raise suspicion for the presence of a desmoid tumor in the majority of cases. In life-threatening cases, surgical treatment should be considered as a palliative approach, though the extent of excision remains debatable.

Anorectal incontinence. pathogenesis and choice of treatment.

Fecal incontinence represent a complex and multifactorial disorder. Although the condition is widely accepted as belonging to the elderly, it is now becoming apparent that younger adults are also frequently affected. Its incidence is estimated at 2% of the general population, while in the elderly it may increase up to 60%. Despite the considerable advances that have been made in the evaluation of anorectal incontinence during the past decades, the cause of this entity still remains obscure. The patient's history, the physical examination, and specialized investigations are essential for the diagnosis and the selection of the appropriate treatment. In clinical practice the most useful tests are the anorectal manometry, the anal endosonography and the pudendal nerve latency. Complete functional and anatomical assessment of the anorectum, the anal sphincters, and the pelvic floor is mandatory in all patients with fecal incontinence for the appropriate diagnosis and identification of the cause, the type of incontinence and the selection of the appropriate treatment.

Optic nerve cyst associated with optic disk pits.

PURPOSE: To report a case with two optic disk pits which were associated with an optic nerve cyst in the same eye. METHODS: Observational case report. RESULTS: A 47-year-old patient noted visual impairment in the right eye. On examination the best corrected visual acuity in the right eye was 20/80 and in the left eye was 20/20. Biomicroscopy revealed, in the right eye, a very pale optic disk with two optic disk pits without macular elevation. Magnetic resonance imaging (MRI) revealed a well circumscribed 6 x 6-mm(2) round cystic lesion within the right optic nerve sheath adjacent to the temporal aspect of the right optic nerve at its retrobulbar segment, which compressed and displaced the nerve. CONCLUSIONS: In the case of an extremely pale optic disk with congenital pits and visual impairment without macular detachment, radiological examination is indicated in order to exclude the possibility of coexisting optic nerve anomalies.

Successful pregnancy outcome in a patient with primary Addison's disease.

A 32-year-old primigravida with primary Addison's disease was followed up from the early stages of pregnancy to delivery and puerperium by a multidisciplinary medical board. Besides fetal surveillance, great attention was given to steroid replacement therapy in order to avoid complications. Vaginal labor resulted in the birth of a healthy neonate that performed well.
