Physician reimbursement by salary or fee-for-service: effect on physician practice behavior in a randomized prospective study.

We used a resident continuity clinic to compare prospectively the impact of salary v fee-for-service reimbursement on physician practice behavior. This model allowed randomization of physicians into salary and fee-for-service groups and separation of the effects of reimbursement from patient behavior. Physicians reimbursed by fee-for-services scheduled more visits per patient than did salaried physicians (3.69 visits v 2.83 visits, P less than .01) and saw their patients more often (2.70 visits v 2.21 visits, P less than .05) during the 9-month study. Almost all of this difference was because fee-for-service physicians saw more well patients than salaried physicians (1.42 visits and .99 visits per enrolled patient, respectively, P less than .01). Evaluating visits by American Academy of Pediatrics' guidelines indicated that fee-for-service physicians saw more patients for well-childcare than salaried physicians because they missed fewer recommended visits and scheduled visits in excess of those recommendations. Fee-for-service physicians also provided better continuity of care than salaried physicians by attending a larger percentage of all visits made by their patients (86.6% of visits v 78.3% of visits, P less than .05), and by encouraging fewer emergency visits per enrolled patient (0.12 visits v 0.22 visits, P less than .01). Physicians' interest in private practice, as determined by their career plans, correlated significantly with total number of patients enrolled (r = .48, P less than .05) and total clinic patients seen by each resident during the study (r = .40, P less than .05).(ABSTRACT TRUNCATED AT 250 WORDS)

Outcome of abuse during childhood among pregnant low income women.

The outcome of experiencing abuse as a child was studied by comparing pregnant women who did recall versus those who did not recall this type of maltreatment. Fourteen hundred low income women were interviewed in prenatal clinic; those who said they were both punished by abuse and beaten by caretakers as children were considered abused. Abuse was recalled by more white than black women but families were followed after delivery and protective service reports of abuse for their offspring were equal. Because of low numbers, black subjects were dropped and the 95 white women who recalled abuse during childhood were compared to the remaining 832 white subjects. The groups did not differ in attitude about current pregnancy, age or marital status, and no differences were found for their children at birth. Abused mothers were more likely to have felt unwanted and unloved as children and to have lower self-images and more isolation than controls. Abused mothers had greater stress, and many of their stresses reflected disturbances in interpersonal relationships. Thus, women abused as children had some characteristics similar to those of known child abusers. Although abused women had more aggressive tendencies, their children were reported to protective services for abuse at the same frequency as control children. Intergenerational transmission of abuse was therefore not demonstrated prospectively. Classic theories of child abuse suggest a special child, special parent and stress act as independent agents to cause abuse. The above data suggest, alternatively, that abuse during childhood may lead to other risk characteristics and to greater stress. These may act together to increase risk for abuse. The special child may have an independent influence on abuse.

Prospective study of antecedents for nonorganic failure to thrive.

The cause of nonorganic failure to thrive (NFT) was studied prospectively in 274 low-income pregnant mothers by interviewing them for characteristics that retrospective studies indicated were associated with child maltreatment. After delivery, medical charts of mothers and infants were reviewed for complications of pregnancy, delivery, and postdelivery hospitalization, and the infants were observed for growth failure. The characteristics of 15 families with infants who developed NFT were compared with those of 86 selected randomly from the remaining families. Interview results suggest that NFT correlates significantly with aberrant nurture during the mother's childhood and with conflicts between the parents of the infants. Perinatal events correlating significantly with NFT include complications of pregnancy, short gestation, and residual minor medical problems at discharge from the nursery. We speculate that these events predispose to NFT by disturbing the development of the mother-infant relationship.

Prediction of child abuse: a prospective study of feasibility.

Feasibility of identifying risk for child abuse prospectively was determined by interviewing 1400 expectant mothers and predicting that 273 were high risk. Nonaccidental child injuries were subsequently reported to authorities for 6% of these versus 1% of the remaining 1127 families. Prediction was only effective for 24 months following interview. Thus prenatal prediction was feasible although the rate of false positive high risk assignment would limit practical application of the interview we used. As a first step to improve prediction accuracy, a group of the 20 strongest predictors was selected from the interview by regression analysis. The correlation of these with abuse was .44 compared to .15 for the original interview. Important predictors included subjective impressions of interviewers, residency transience, untruthfulness, disturbed childhood nurture, unwanted pregnancy, and perhaps conditions that increased parent-child exposure.

Relative roles of burn injury, wound colonization, and wound infection in induction of alterations of complement function in a guinea pig model of burn injury.

Stimuli involved in induction of alterations of the complement system and production of circulating inhibitor(s) of phagocytic function of polymorphonuclear neutrophils following burn injury were investigated using a guinea pig model of scald burn injury. The activity of C1-C9, assessed by measurement of total hemolytic complement, was found to increase primarily in response to burn injury per se, whereas reduction in the activity of the alternative complement pathway was shown to develop in association with natural colonization and local burn wound infection with bacterial pathogens. Invasive burn wound infection induced experimentally with Staphylococcus aureus, Pseudomonas aeruginosa, or Candida albicans exacerbated this latter abnormality, caused consumption of C1-C9 activity, and was associated with appearance of serum factors that depressed phagocytosis of Escherichia coli 075 by peritoneal polymorphonuclear neutrophils. Thus injury and coexistent infection both play important roles in induction of humoral alterations of host defense associated with burn injury.

Child health and maltreatment.

Children who are abused have been said to have more illnesses than children who are not maltreated. The relationship between abuse and illness has been hypothesized to function in 2 ways: (1) that abuse precedes the illnesses and children from abusive homes become ill because of the damaging environment they endure, or, conversely, (2) that the illnesses precede the abuse, with the fussy behavior of ill children eliciting abuse. This study was intended to clarify the temporal relationship between illnesses and maltreatment. Health data were collected on a sample of 80 children: 11 from abusive families, 31 with nonorganic failure to thrive (NOFT), 14 from neglectful families, and 24 from control families. Hospital records (both inpatient and outpatient) for these children from the time of birth until they were 3 years old were searched by data collectors unaware of the child's classification. Children from abusive families or with NOFT appeared to be ill more often than control children, particularly during the first few months after birth, before abuse had been reported, but not necessarily before NOFT had been discovered. Health records of neglected children were not significantly different from those of controls. In addition to the abused. These 6 children also had more illnesses than control children, again particularly during the first few months after birth. Having ill children is described as a source of stress that may trigger abuse in an already stressed family.

Staphylococcus in toxic shock syndrome and other surgical infections. Development of new bacteriophages.

Recent studies of Staphylococcus aureus isolates from patients with toxic shock syndrome (TSS) have shown the dominance of phage type 29/52 with the capacity to produce pyrogenic exotoxin C and enterotoxin F. They also showed that 29% of the isolates were nontypable and 90% of them had similar toxigenic properties. The existence of unknown and important phages in this disease was postulated. Five new phages were then developed and used for typing three groups of staphylococcal isolates: 236 from patients with TSS, 67 from patients without TSS, and 159 from patients with infected burns. Results showed a high correlation between the lytic action of the new phages and the 29/52 phages, and an additional typing capability in 35% of the previously nontypable TSS isolates, emphasizing further the potential of bacteriophage typing of S aureus in these infections.

Concerns of mothers seeking care in private pediatric offices: opportunities for expanding services.

A surplus of general pediatricians has been predicted for 1990. This surplus could provide both opportunity and need for practitioners to identify areas of maternal concern that might guide expansion of marketable physician services. For this purpose, maternal concerns were assessed by interviewing 207 mothers seeking care in private pediatric offices. Only 30% of mothers were most worried about their child's physical health. The remaining 70% were most concerned about problems falling into six categories of parenting, behavior, and development (psychosocial concerns): personality/social development, discipline, mental development, mother-child interaction time, adjustment to divorce and other life changes, and adolescent transition. Although the majority of these concerns conceivably could be handled in private offices, only 28% of these mothers had discussed their greater psychosocial concern with their pediatrician. A search for explanations of this failure to communicate indicated mothers often were not aware that their pediatrician could help, or they questioned his ability or interest in assisting them. Characteristics that correlated significantly with communication were higher family socioeconomic levels, and greater physician self-perceived ability and interest in these psychosocial problems. Parenting, behavior, and development concerns represent an opportunity for expanding services if some of these obstacles can be overcome.

Antecedents of child abuse.

The qualities of parents who batter their children have been determined by interviewing known abusers. Because most studies lack controls and because family characteristics may be changed by abuse, a prospective study was instituted. Fourteen hundred low-income mothers were interviewed in a prenatal clinic, and the characteristics of 23 reported for abuse within two years were compared to the characteristics of the remaining mothers. The predominant antecedents of child abuse were: unwanted pregnancy, aggressive tendencies, and aberrant childhood nurture in which disturbed family relationships were more important than outright abuse. Abusive mother had slightly less self-esteem. Factors which increased exposure between parents and children seemed to increase risk for abuse. In contrast to retrospective findings, abusive mothers did not differ in support available from others, age, education, isolation, family alcohol or drug problems, and expectations of child development.

Effect of use of a measured dispensing device on oral antibiotic compliance.

The substitution of a syringe in place of a teaspoon for the delivery of oral medications has become routine in some pediatric practices for reasons that are not clear. Although a syringe would measure dosages more accurately, most oral medications have a broad acceptable dose range. A possible advantage of syringes is that parents who receive these may be more impressed with the importance of treatment than those prescribed medications by teaspoon. To test this hypothesis, 72 patients below 24 months of age with otitis media were randomly divided into a group that received a prescription indicating amoxicillin should be given by teaspoon and a group that received a labeled syringe for this purpose. The qualitative presence of the antibiotic in urine at seven-day follow-up visit was 56 per cent among the teaspoon users and 67 per cent among syringe users, while return rates for one week appointments were 51 per cent and 32 per cent, respectively. Neither difference was significant. Use of a measured oral dispensing syringe did not improve compliance in administration of medication or return for follow-up and therefore, was judged not to be cost effective.

Mother-infant interaction and child development after rooming-in: comparison of high-risk and low-risk mothers.

One hundred and seventy-two low-income women were interviewed prenatally to determine risk for later mistreatment of their children. High-risk and low-risk women were then randomly assigned at delivery to either limited or extended postpartum contact with their newborns over the first two days after birth. Mother-infant interaction observations were performed at 48 hours and at one, three, six, twelve and eighteen months postpartum. Infants were tested at nine months with the Bayley Scales of Infant Development. Results indicated that outcome following extended mother-infant postpartum contact varies with maternal risk status and measures employed for evaluation. Low-risk extended-contact mother-infant pairs differed from low-risk controls in observed interaction while high-risk extended-contact and controls did not differ from each other in interaction. High-risk extended-contact infants were more advanced in motor development than control infants at nine months, however, while low-risk extended contact and control infants did not differ in development.

Staphylococcus aureus associated with toxic shock syndrome: phage typing and toxin capability testing.

Phage type 29 Staphylococcus aureus was identified singly or with type 52 in 64.1% of 248 coded isolates from patients with toxic shock syndrome. These phage types also have a high capability of producing pyrogenic exotoxin C and enterotoxin F. The origin and development of these toxigenic strains were explored by studying 25,220 isolates of S. aureus stored in a staphylococcal bank between 1960 and 1979. A small percentage of phage types 29, 52 were found in 1960, but their prevalence increased between 1961 and 1970, and continued at elevated levels through 1979. The toxigenic capabilities of these phage types were apparently acquired about 1971 and increased up to 1975. High levels of prevalence persisted during the following 4 years, and receded in 1980 and 1981. Other evidence during 1980 and 1981 indicates that these strains of S. aureus have become an important pathogen in surgical wounds, burns, and other infections.

Increased child abuse in families with twins.

Large families and inadequate spacing of children increase the risk of abuse. Twin births incorporate both of these factors, yet the association of twinning with subsequent abuse has not been explored. Forty-eight families with twins from St Vincent Hospital and Medical Center and Nashville General Hospital were compared with 124 single-birth families, matched for hospital of delivery, birth date, maternal age, race, and socioeconomic status. Three control (2.4%) and nine twin (18.7%) families were reported for maltreatment (P less than .001). Mothers of twins experienced greater previous parity than did control subjects (P less than .001). Twins also had significantly longer nursery stays (P less than .001), lower birth weights (P less than .001), and lower Apgar scores at one (P less than .01) and five (P less than .05) minutes. A regression analysis incorporating all of these variables, however, showed that twin status was most predictive of subsequent abuse.

Infection of bone by Mycobacterium fortuitum masquerading as Nocardia asteroides.

A case of traumatic osteomyelitis of the leg yielded on culture a branching partially acid-fast organism that failed to respond to therapy directed at Nocardia asteroides. Subsequent laboratory investigation revealed the organism to be Mycobacterium fortuitum. N. asteroides and M. fortuitum can demonstrate similar staining and morphologic patterns microscopically, as well as common colonial and cultural characteristics. Separation can be aided by careful examination of the branching pattern, and can be established by thin-layer chromatography of lipid extracts of the organism. Correct identification of these species in the laboratory is important because of some overlap in their clinical syndromes and because of differences in their susceptibilities to antibiotics.

Reduction in alternative complement pathway mediated C3 conversion following burn injury.

Studies were performed to further investigate an abnormality of the alternative complement pathway associated with burn injury. C3 conversion by inulin and cobra venom factor was measured in serial serum samples from 18 burned patients and in the sera from 25 normal controls. C3 conversion by both activating substances in the patients' sera was significantly decreased during 50 days postburn in comparison to C3 conversion in the normal sera. The most marked reduction in C3 conversion occurred in patients with large full-thickness injuries and infectious complications. However, the abnormality was not more severe in patients with pneumonia and bacteremia in comparison to patients with bacteremia only. It was also not predictive of fatal outcome resulting from infectious complications. Preliminary evidence was presented to suggest that the abnormality is caused by a serum inhibitor, which has an affinity for cellulose and/or is inactivated by it.

Serum-mediated inhibition of polymorphonuclear leukocyte function following burn injury.

Serial serum samples from 12 bacteremic burned patients were tested at a physiologic concentration for their ability to facilitate phagocytosis and intracellular killing of the homologous infecting Staphylococcus aureus strains by normal human polymorphonuclear leukocytes in comparison to pooled normal human serum. Serum-mediated inhibition of leukocyte bactericidal activity was demonstrated in three of the patients during 13 to 56 days after burn. Decreased bacterial activity was related to an inhibitory effect of the burn sera on the phagocytic process, which reduced the number of internalized bacteria available for intracellular killing. The serum-mediated inhibition of phagocytosis was not found to be dependent on bacterial surface properties unique to S. aureus. The inhibitory effect was shown to involve a direct interaction of the burn sera with the leukocytes, which was not associated with cell death and was not reversed by washing of the leukocytes.
