Palliative care in the acute hospital setting.

Palliative care, associated with the hospice movement, has achieved wide acceptance in many countries. Its focus has been care of cancer patients and more recently, those with AIDS. Most acute hospitals do not have avenues to care for these patients or others in the end of life stages. One method of providing care and comfort in an acute hospital setting for those who have chosen to discontinue treatment or whom aggressive treatment is no longer beneficial is the palliative care team. This team was developed to ensure that specialized services would be offered to the dying patient in the acute setting.

Confirmation of brain death using 99m Tc HM-PAO.

Because 99m Tc HM-PAO is simple to use, noninvasive and readily available, it appears to satisfy most criteria for the use of radionuclide cerebral imaging in the confirmation of brain death. Waiting periods, which are often included in brain death protocols, delay the diagnosis of brain death. 99m Tc HM-PAO has proven to be a useful test to confirm brain death.

C1-C2 fractures and dislocations.

The management of patients with bony or ligamental injury of C1 or C2 depends on the nature of the fracture or subluxation. Other factors which must be taken into consideration are mechanism of injury, presence of neurological deficit and demonstration of stability. It is the intention of this article to examine bony anatomy of the upper two cervical vertebrae and their relationship to each other. The management of this injury and the role of the nurse in caring for and educating the patient with this type of injury, as well as a case study will be presented.
