Campylobacter pyloridis gastritis II: Distribution of bacteria and associated inflammation in the gastroduodenal environment.

The distribution of Campylobacter pyloridis in the stomach and duodenal bulb of two series of patients (n = 53) with dyspeptic symptoms was studied. In all patients where a comprehensive search of the stomach was conducted, C. pyloridis and active chronic gastritis were associated. That is, the presence of this organism anywhere in the stomach may be taken as a marker of active chronic gastritis within that stomach, although not necessarily at the site of bacterial detection. Two main patterns of colonization are proposed: C. pyloridis either colonizes normal antra and extends proximally along the lesser curvature and into the body with subsequent development of active chronic gastritis and/or the bacterium colonizes areas of chronic-only gastritis induced by other agents resulting in active chronic gastritis. As C. pyloridis may be limited to the antrum or body of any given stomach, it is necessary to biopsy both areas when looking for the bacterium.

Cimetidine and acute upper gastrointestinal bleeding: a double-blind controlled trial.

Patients presenting with acute upper gastrointestinal bleeding from a variety of lesions were admitted to a prospective double-blind controlled trial to determine if cimetidine reduces the severity of bleeding and/or the incidence of rebleeding. During the first 48 hours, the patients received intravenous cimetidine (200 mg four-hourly) or placebo, and for the following ten days, oral cimetidine (1 g/24 hr) or placebo. Eight-eight patients entered the trial of whom 45 (51%) were in the cimetidine-treated group. Six of the seven patients requiring surgery for life-threatening bleeding and four of the six patients who rebled were in the cimetidine-treated group. This study failed to demonstrate any advantage of using cimetidine routinely in the treatment of acute upper gastrointestinal bleeding.

A comparative study of two dicyclomine preparations in functional bowel disorders.

Using a double-blind crossover technique in patients suffering from maladies associated with gastrointestinal spasm, sustained-release 40 mg dicyclomine hydrochloride tablets (Merbentyl Dospan) have been compared with 20 mg plain dicyclomine hydrochloride tablets (Merbentyl). It has been concluded that these two dicyclomine formulations are equivalent in terms of efficacy and low incidence of side effects.

Chloroquine-resistant falciparum malaria from Papua New Guinea and its implications for Australia.

There is need for effective chemoprophylaxis against chloroquine-resistant falciparum malaria, and for a safe and effective drug to be readily available for the treatment of that condition. Experience with the cases cited in this study indicates that Fansidar should be made available for treatment and it is suggested that Maloprim be made available as a prophylactic agent.
