Medications for the elderly.

Medications were checked by a trained nurse during the course of her duties in a controlled study of 150 elderly people with cerebrovascular disease. The cost of medications was marginally reduced in a group of people receiving the additional supervision, while it rose by 12.0% in a control group. In the group visited by the nurse, containers were labelled correctly for significantly more people than was the case with the control group.

Pyoderma gangrenosum--a literature review.

Pyoderma gangrenosum is an ulcerative skin disorder with typical clinical characteristics. Histologic and laboratory findings are nonspecific. Pyoderma gangrenosum is associated with internal disorders including inflammatory bowel disease, paraproteinemias, leukemias, and arthritis. The pathogenesis of pyoderma gangrenosum is unknown, although a partial defect of cell-mediated immunity may exist. Treatment includes bedrest, local care, sulfonamides, sulfones, and corticosteroids.

Ichthyosis hystrix.

According to a popular system of classification, the term ichthyosis hystrix denotes an organoid epidermal nevus composed of hyperkeratotic papules with a bilateral distribution. Our patient had dramatic disfiguring epidermal changes of this type covering nearly all of her body. Although this abnormality began at the age of 2, she does not exhibit any of the various neurological or skeletal derangements that have been reported to be associated with the condition.

Diffuse fasciitis with eosinophilia.

The recently described syndrome of diffuse fasciitis with eosinophilia consists of scleroderma-like cutaneous changes in the absence of Raynaud's phenomenon and without visceral involvement. Peripheral blood eosinophilia and hypergammaglobulinemia occur in the majority of patients. The biopsy specimen is characteristic and shows a diffusely thickened fascia with a noticeable inflammatory infiltrate. The etiology and pathogenesis of the entity are unknown. The majority of the cases have been reported to respond to corticosteroids. Pulmonary function abnormalities and rheumatoid factor elevation in one of our patients suggests that this syndrome may be a variant scleroderma or may have previously unrecognized systemic manifestations. The patients' objective responses to corticosteroids were minimal.

Calcifying acne lesions.

Cutaneous calcium deposition can occur in long-term inflammation. The deposition of calcium in long-standing acne may produce either small pigmented nodules or no visible lesions. The former type is or no visible lesions. The former type is much less common, with the nodules containing particulate spheres of mineral which probably represent true osteomas. Both types of calcification can be demonstrated roentgenographically and may occur in as many as half of all cases of severe long-standing acne.

Measurement of cytoplasmic fluorescence depolarization of single cells in a flow system.

We have built a flow system in which we can analyze and sort individual viable cells on the basis of their cytoplasmic microviscosity. The average cytoplasmic microviscosity (or cytoplasmic structuredness) of a cell can be quantitated by exciting fluorescein molecules in the cytoplasm of the cell with a polarized light source and measuring the extent of depolarization of the fluorescent signal. Changes in the state of a cell (e.g., the reception of a signal inducing the cell to differentiate) often appear to be associated with changes in cytoplasmic microviscosity, these changes being detectable within hours of the inducing signal. An example of one such change is presented.

Leishmania tropica infections in travellers.

Oriental sore (acute cutaneous leishmaniasis) is usually a self-limited infection of the skin caused by the protozoan Leishmania tropica. This disease is endemic to the Mediterranean, Asia, Africa, and the Middle East, and is seen in this country among travellers and immigrants. We found four cases at the University of Michigan and diagnosed and treated the disorder. Some of the treatments currently recommended for this disease are potentially toxic, and none have been proved to be of real value. Uncomplicated, acute cutaneous leishmaniasis (L tropica) may best be managed by withholding treatment and observing the patient.

Disseminated granuloma faciale.

Granuloma faciale is a well-defined entity that almost exclusively involves the face, although it has been reported in extrafacial locations. The infrequent reports of extrafacial lesions may reflect the inconspicuous nature of extrafacial lesions or a failure to specifically examine the patient for such lesions.

Modified transit dosementer system for 60Co rotational therapy.

The transit dosemeter has been used in obtaining inhomogeneity corrections for 60Co rotational therapy. A modified approach is presented which features a rapid electronic system for determining the average tissue-air ratio (TAR) for a 360 degree rotation. Experimental results and early clinical experience show that this system is capable of providing a TAR value directly, and more rapidly. It can be readily incorporated in a therapy system which already includes a transit dosemeter.
