Malaria treatment practices among mothers in Guinea.

A survey of mothers of young children in Guinea, West Africa (June-July 1988) suggests that individual, sociocultural, and structural factors influence the use of antimalarials for episodes of fever presumed to be malaria. A survey of 243 mothers was carried out in eight different sites (four urban and four rural) in two regions of the country. Among mothers interviewed, use of Western antimalarials for infants and children with fever was acceptable, but access to treatment was problematic. Mothers interviewed were less likely to use antimalarials during pregnancy, partly because of certain misconceptions about their effects and lack of access to the medication. Despite programmatic efforts to promote practices of presumptive treatment of fever among young children and chemoprophylaxis during pregnancy among mothers in towns and villages surveyed, variables associated with low use of antimalarials by mothers and their young children reflect important barriers to the implementation of child survival initiatives.

Perception and management of guinea worm disease among Ghanaian villagers. A framework for differential health education planning.

A survey, resulted from the immediate need to understand the health behaviour of Southern Ghanaian villagers during an epidemic of guinea worm disease. While such problems are usually studied at a time when the epidemic has abated, this study emphasized getting into the field quickly to obtain data before the epidemic passed. If behaviour models are to have value for programme planning in the developing world, they should be amenable to such quick sample approaches. The study involved a survey on attitudes, beliefs and practices related to gent. Results showed distinctive and somewhat differing perceptions of susceptibility, seriousness, curability and preventability for the two ethnic groups within a single geographic area. The analysis of the survey results permitted suggestion of various educational strategies for both of the groups. This quick survey approach must be more refined before it has wide applicability. There appears to be a significant potential for developing a simplified checklist to be used by health workers in carrying a rapid educational diagnosis of a specific health problem for a given population.

Long term ridge augmentation with rib graft.

Results of ten cases are reported to indicate the bone augmentation that may be expected in three to six years using transoral rib grafting. Some augmentation was present in all cases. This is an improvement over the continued resorption that would have taken place in these already thin mandibles. Function was improved in all cases. This modality falls short of the ideal aid to these patients, but is an available holding procedure.

Guinea worm in southern Ghana: its epidemiology and impact on agricultural productivity.

In southern Ghana guinea worm disease was found to occur almost exclusively in villages dependent upon pond water during the dry season. The recent occurrence of guinea worm for the first time in many villages in the survey area suggests that the disease is spreading. The risk of increasing disease in the Accra plains is serious, because almost half of the 159 villages surveyed use pond water, and residents frequently travel to endemic areas. In this study adult male farmers were at greatest risk of becoming infected. The average work loss in untreated adults was more than 5 weeks. Because guinea worm disease is seasonal, conciding with peak agricultural activities, and few alternative labor sources are available for the incapacitated farmer, a marked reduction in agricultural output occurs. Additional research is needed to guide health education programs, to evaluate the effectiveness of chemical control of cyclops in ponds, and to develop low-cost improved rural water supplies.

Failure of thiabendazole and metronidazole in the treatment and suppression of guinea worm disease.

Guinea worm disease is endemic in West Africa. In 1973 a field drug trial was conducted to compare effectiveness, cost, and side-effects of thiabendazole and metronidazole in treating active guinea worm disease and preventing latent worms from emerging. A mass chemotherapy campaign was planned to follow the drug trial. Only 15.5% of the treated patients expelled the worm completely, and in 28.4% of the cases worms continued to appear. Both drugs were equally unsatisfactory in their anti-helminthic effect. Consequently, our efforts to control guinea worm have shifted from chemotherapy to chemical control of cyclops and improvement of rural water supplies.

Data-based planning for educational interventions through hypertension control programs for urban and rural populations in Maryland.

As part of a statewide effort to coordinate existing resources for high blood pressure (HBP) control, a public health HPB control program was planned and implemented in two high-risk communities in Maryland. The selection of the two communities was based on epidemiologic data. The planning of the educational intervention program in these communities (urban and rural) was guided by organizational theory and health education principles. The framework for development, implementation, and evaluation of the program utilizes an educational assessment model which identified factors that predispose, reinforce, and enable individual persons to practice positive health behavior. Multiple data sources were used in assessing the extent of the problem and relevant approaches in the development of the coordinated HPB control program. These include statewide vital statistics and a random statewide household survey to assess the prevalence rates of awareness, treatment, and control of HPB. To supplement these data, telephone surveys were carried out in the two communities to assess knowledge, beliefs, and practices related to HPB control. Medical record reviews provided baseline information on appointment keeping behavior and BP correlates of hypertensive patients. The planning and implementation of the program was carried out under the direction of representatives of the health care systems, community leaders, and residents, and representatives of communitywide organizations involved in HPB control.
