[Skin nodules and ulcers of the limbs in a patient with rheumatoid arthritis].

CASE HISTORY: While being treated with corticosteroids and methotrexate for rheumatoid arthritis, a 63-year-old man developed livid nodules on his lower arms, hands and feet, as well as fever, necrotizing skin ulcers and rupture of a finger extensor tendon. INVESTIGATIONS: No vasculitis was found in a biopsy of one of the nodules on the lower arm. Fast growing mycobacteria, classified as M. marinum by PCR, were cultured from wound swabs. TREATMENT AND COURSE: The lesions healed on administration of ciprofloxacin, ethambutol and clarithromycin as well as local treatment. CONCLUSION: Cutaneous lesions of an atypical mycobacterial infection are often misdiagnosed. This is especially so in immunocompromised patients and in the differential diagnosis of vasculitis.

[Human behavior between instinct, feelings and responsibility].

The paper deals with the fundamental problem: How does the brain (in the way it is and the way it functions) influence human experience, behaviour and social interaction? Deriving from this question, there's the matter of how all this influences people as road users. After some epistemological considerations the following subjects are dealt with the article: the human perception and human "reality", the limits of human imagination, the basis of human learning, the complexity of processes within the brain, the reaction to changes and the ability to concentrate as well as McLean's theory of the triune brain. The subjects are complemented by aspects of biological and socio-cultural evolution of man, starting from evolutionary ethics going right up to the problem of responsibility.

[Social psychological and sociocultural aspects of alcohol drinking and alcoholic intoxication behind the wheel].

The paper deals with socio-psychological and socio-cultural problems of drunken driving and of alcohol-consumption in general. It is the intention to make evident how the way to show a certain behavior is imbedded in more comprehensive conditions. The contradictory relations between the role of individual traffic and of alcohol consumption in a society on the one hand and the phenomenon of drunken driving on the other hand are pointed out, as well as the different valuation of social drinking and alcohol dependence. The way of profanation of the drug "alcohol" and the importance of social influence on consumption and effect of the drug are shown, taking into consideration the subjective position of the individual in the society. Finally the role of the state and possibilities of a systemic approach against drunken driving are dealt with.
