Endogenous androgen levels in epithelium and stroma of human benign prostatic hyperplasia and normal prostate.

5 alpha-Dihydrotestosterone (DHT) and 5 alpha-androstane-3 alpha, 18 beta-diol (3 alpha-diol) were extracted from epithelium and stroma of human benign prostatic hyperplasia (BPH) and of normal prostate and quantified by RIA. The main results were: (1) concerning the BPH, DHT is mainly located in the nuclear fraction of epithelium and stroma, whereas 3 alpha-diol was completely of extranuclear origin, (2) in the nuclei derived from BPH stroma the DHT content (7.1 +/- 0.88 pmol/mg DNA, mean +/- SEM [n = 14]) was significantly higher (P less than 0.01) than in the nuclei derived from BPH epithelium (3.8 +/- 0.38 pmol/mg DNA [n = 14]). The DHT content in the nuclear fraction derived from unseparated, i.e. whole tissue, was 5.6 +/- 0.60 pmol/mg DNA [n =14], (3) the biological significance of the overwhelming DHT accumulation in the stromal nuclei for the BPH tissue is reflected by a significant correlation between the DHT values in the nuclei from stroma and whole tissue (rs = 0.710, P less than 0.01) and (4) in four normal prostates the DHT content in the nuclear fraction of epithelium (1.3 +/- 0.37 pmol/mg DNA) and stroma (2.2 +/- 0.93 pmol/mg DNA) was significantly lower (P less than 0.01, each) compared with the respective BPH fraction. These data support earlier findings which indicate that the stroma of BPH is a preferential tissue for androgen metabolism.

A method for evaluating postural hypotension in conscious rabbits as a model to predict effects of drugs in man.

Because of the importance of postural hypotension as a side effect of antihypertensive drugs in man, an experimental model has been developed that permits the investigation of blood pressure response to tilting. Conscious rabbits were placed on a tilting table and tilted rapidly from horizontal to vertical position. Blood pressure was recorded continuously throughout the whole period. The individual orthostatic reaction was expressed as an orthostatic index. The experimental data obtained with guancydine, clonidine, guanethidine, prazosin, and dihydralazine were compared with clinical observations with regard to the impairment of orthostatic reaction; dihydralazine did not agree, but there was a good agreement for guancydine, clonidine, guanethidine, and prazosin. Although complete agreement between the experimental data and clinical observations does not exist, the model seems to be sufficient to differentiate between drugs with low or high potential for postural hypotension.

The influence of epsilon-aminocaproic acid on the mineralocorticoid hypertension of the rat.

The development of hypertension induced by DOCA/salt in rats can be reduced by epsilon-aminocaproic acid. Analogously to the clinical finding, glomerulopathy is also less marked. Recognising on the one hand the interaction between the coagulation system and the vessel wall and on the other hand the fibrinolysis-blocking properties of EACA, it can be assumed that derangements of the coagulation system are aetiologically involved i hypertension induced by salt/DOCA. As a secondary finding, it was observed that the course of an existing lung infection is deleteriously affected by fibrinolysis-blockade with EACA.

Comparative investigation on the cardio-protective action of the beta-blockers carazolol, propranolol and pindolol in rats.

A comparative study of the beta-receptor blockers propranolol, pindolol and carazolol, with respect to their cardio-protective properties against stress (hypoxia or isoprenaline injection) in rats showed the following. The beta-blockers investigated protected the heart against glycogen depletion during hypoxia. Carazolol was active at considerably lower doses than pindolol (about 25 times less) and propranolol (about 100 times later). All three beta-blockers protected against the induction of cardiac necroses by isoprenaline, but only carazolol produced almost complete protection. There is a good correlation between the protection against the development of cardiac necroses and the inhibition of tachycardia induced by isoprenaline (r = 0.96). The results are discussed from the point of view of the possible mechanisms by which the beta-blockers prevent cardiac damage induced by stress situations.

Interrelationships between sex hormone-binding globulin and testosterone, 5 alpha-dihydrotestosterone and oestradiol-17 beta in blood of normal men.

Sex hormone-binding globulin (SHBG), testosterone (T), 5 alpha-dihydrotestosterone (DHT) and 17 beta-oestradiol (E2) were measured in 93 men in the age range 36--80 yr. Assuming that the binding of the steroids to blood albumin and SHBG follows the law of mass action, non-SHBG bound steroid concentrations (Tn, DHTn, E2n) were calculated from the SHBG and the respective total hormone values. The main results were: (1) Mean Tn, DHTn, E2n and SHBG remained on the constant level until the age of 65 yr. In men 66--80 yr old Tn and DHTn were found to be significantly lower (P less than 0.001), whereas E2n and SHBG were elevated (P less than 0.05, P less than 0.001, respectively) when compared with men age 36--55 yr. (2) In contrast to total T and DHT, which correlated positively with SHBG (P less than 0.001) in Group I (age 36--55 yr) and also when eliminating the influence of age (P less than 0.05, P less than 0.01, P less than 0.01 for T, DHT and E2) in group II (age 56--80 yr), the correlation between Tn and SHBG was insignificant, whereas a negative correlation between DHTn and SHBG (P less than 0.01) was present in Group I. (3) The age dependent increase of SHBG in Group II can be best explained by the respective alterations of the E2n/DHTn ratio. These results are discussed with respect to the relation between the SHBG, the total steroid and the non-SHBG bound steroid concentrations.

Interrelationships between sex hormone-binding globulin and 17 beta-estradiol, testosterone, 5 alpha-dihydrotestosterone, thyroxine, and triiodothyronine in prepubertal and pubertal girls.

Sex hormone-binding globulin (SHBG), 17 beta-estradiol (E2), testosterone (T), 5 alpha-dihydrotestosterone (DHT), T4, and T3 were measured in prepubertal (age, 1--8 yr; n = 44; group I) and pubertal (age, 9--16 yr; n= 56; group II) girls. The main results were as follows. 1) In group I, a higher mean (+/- SEM) value for SHBG was found (73.5 +/- 2.82 nmol/1) compared to that in Group II (56.8 +/- 3.37; P less than 0.001). 2) Despite a significant increase of the androgens T and DHT with age in group I (rT.age = 0.578; rDHT.age = 0.544; P less than 0.01 for each), no significant alteration of SHBG with age was present. 3) In group II, a significant decrease of SHBG with age occurred (rSHBG.age = -0.438; P less than 0.35) along with the expected increases of E2 (rE2.age = 0.691; P less than 0.001), T (rT.age = 0.765; P less than 0.001), and DHT (rDHT.age = 0.712; P less than 0.001). 4) The analysis of the data by partial correlation coefficients supports the suggested role for androgens in the decline of SHBG during puberty in girls, whereas correlations of SHBG with E2 or the thyroid hormones were insignificant.

Hormone blood levels and their inter-relationships in normal men and men with benign prostatic hyperplasia (BPH).

In 128 non-hospitalized men (age range 36-65 years) rectal palpation revealed in 54 cases an enlargement of the prostate (group II), which was very distinct in 20 cases (group III). The measurement of testosterone (T), 5alpha-dihydrotestosterone (DHT), 5alpha-androstane-3alpha,17beta-diol (3alpha-diol) oestradiol (Oe2), sex-hormone-binding-globulin binding capacity (SHBG), luteinizing hormone (LH), follicle stimulating hormone (FSH) and prolactin (Prl) in the blood of normal men (group I) and those with BPH (group II or III) demonstrated no significant differences between the three groups when respective age ranges were compared. A significant increase of FSH and decrease of 3alpha-diol with age was seen in the normal group (I), which was similar but less pronounced in BPH (groups II and III). A distinct increase of DHT with age was found in BPH (group II), which was not so dominant in normal men (group I). From these data it is concluded that the conversion of DHT to 3alpha-diol might be reduced in older males independent from the occurrence of BPH and that the hyperplastic prostate possibly secretes significant amounts of DHT into the circulation. These results are discussed with respect to their possible role in the pathogenesis of BPH.

Hormone blood levels in patients with prostatic carcinoma and their relation to the type of carcinoma growth differentiation.

Human prolactin (HPr), luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone, 5 alpha-dihydrotestosterone (DHT), oestrone and oestradiol are measured by radioimmunoassay (RIA) and bilogical prolactin activity additionally by pigeon crop sac assay in plasma of patients with prostatic carcinoma and two control groups. Pigeon crop sac assay is more often positive in cancer patients (50%) than in controls (20%) and HPr distinctly elevated in 20% of the carcinoma patients. Patients are classified according to histological type of tumour: carcinomas with cribriform and/or solid growth show significantly elevated pigeon crop sac activity and higher testosterone, DHT and oestrone levels in plasma than tumours without such growth. Values are discussed with respect to age.

Sex hormone binding globulin binding capacity, testosterone, 5alpha-dihydrotestosterone, oestradiol and prolactin in plasma of patients with prostatic carcinoma under various types of hormonal treatment.

Sex hormone binding globulin (SHBG) binding capacity, the concentrations of testosterone (T), of 5alpha-dihydrotestosterone (DHT), of oestradiol-17beta (Oe2), of oestrone (Oe1), of prolactin (hPr) and the percentual specific binding of T to SHBG (%TB) were measured in plasma of patients suffering from prostatic carcinoma and of a control group of similar age. No significant differences in any of the investigated parameters were found between the control group and the carcinoma patients before treatment although 15% of the latter showed distinctly elevated hPr values. Treatment of carcinoma patients with 1) Antiandrogen (cyproterone acetate, Androcur) resulted in a significant decrease of T, Oe2 and SHBG. The DHT/T-ratio increased. n=5. 2) Orchidectomy caused an even more pronounced fall in T, DHT, Oe1 and Oe2 blood levels. SHBG was not altered. DHT/T-ratio increased. n=32. 3) Cyproterone acetate after orchidectomy led to elevated hPr values. n=5. 4) Oestrogen (diethylstiboestrol-diphosphate, Honvan) after orchidectomy increased SHBG and hPr. n=6. 5) Corticosteroid (Prednisone, Decortin) after orchidectomy decreased T and SHBG below the levels found after orchidectomy alone. n=5. 6) Diureticum (Mefruside, Baycaron) (n=5) or 7) a placebo (n=7) did not alter any of the parameters measured. 8) Treatment with HCG (Primogonyl) of patients suffering from oligozoospermia resulted in a significant increase of T, DHT and Oe2. SHBG was not altered. DHT/T-ratio decreased. n=7.

Cardiac action of carazolol and methypranol in comparison with other beta-receptor blockers.

The new beta-blockers 4-(2-hydroxy-3-isopropyl-amino-propoxy)-carbazole (carazolol) and 1-(4-acetoxy-2,3,5-trimethylphenyloxy)-3-isopropylamino-propan-2-ol (methypranol, Disorat) were compared with 14 well-known beta-blocking agents with regard to isoproterenol antagonism (equipotent doses in the rabbit i.v.), acute toxicity (LD50 in mice i.v.) and intrinsic sympathomimetic activity (increase in the heart rate of reserpinized rats i.p.). The following descending order for the equipotent beta-receptor blocking doses was obtained: nifenalol, DCI, pronethalol, practolol, sotalol, alprenolol, bupranolol, toliprolol, propranolol, methypranol, YB 2 pindolol, bunitrolol, oxprenolol, bunolol and carazolol. Carazolol had, in addition, the highest therapeutic index and at beta-receptor blocking doses virtually no intrinsic sympathomimetic activity.

Experiments in animals on the pharmacological effects of metipranolol in comparison with propranolol and pindolol.

The beta-blocking agent 1-(4-acetoxy-2,3,5-trimethylphenyloxy)-3-isopropylamino-propan-2-ol (metipranolol) was compared with propranolol and pindolol. The beta-blocking activity on isoproterenol induced tachycardia was determined in rabbits (ED250bpm). The following doses in microgram/kg i.v. were required to produce the same inhibition: 410 propranolol; 160 metipranolol; 130 pindolol. When intrinsic sympathomimetic activity was determined in reserpinized rats metipranolol was found to produce less than 10% of the increase in heart rate induced by a standard isoproterenol dose (1 mg/kg i.p.), whereas propranolol caused less than 20% and pindolol ca. 60%. The membrane stabilising activity, determined by the elevation of the fibrillation threshold (ED delta100muA) in rabbit hearts, was found to be greatest with propranolol (0.98 mg/kg i.v.) and least with metipranolol (1.68 mg/kg), with pindolol occupying an intermediate position (1.10 mg/kg). The cardioprotective action to hypoxia stress of metipranolol in rats was found to be the best, requiring a dose of only 5 microgram/kg i.p. compared with values of 28 microgram/kg for pindolol and 250 microgram/kg for propranolol. These results show that metipranolol has a high beta-sympatholytic activity which is not accompanied by either marked intrinsic activity or membrane-stabilising properties. The relatively marked cardioprotective action, which is probably due to a metabolic action, is particularly noteworthy.
