[Influence of androgens on cardiovascular system in men].

It is well known fact that incidence of morbidity and mortality due to coronary artery disease is higher in men than in women. For many years experimental and clinical studies have focussed on the role of androgens in cardiovascular pathology. The clinical implications of androgens' deficiency as well as the molecular mechanisms of their action have not been well established yet. However, the increasing number of evidences suggests that androgens may have beneficial effects on atherosclerosis. In this review we discuss the androgen metabolism, their effect on cardiovascular system and the relations with major risk factors of atherosclerosis.

[Management and outcome of pregnancy in women with hypothyroidism--case report].

We analysed the influence of congenital hypothyroidism, diagnosed in the third year of life, on the outcome of pregnancy. We observed the physiological progress of pregnancy and normal development of the foetus. We delivered newborn by caesarean section. We did not observe any disadvantageous influence of congenital hypothyroidism in the mother on newborn status. We analysed the pharmacological substitution and its influence on pregnancy outcome.

[Effect of dihydrotestosterone treatment on exercise induced ischemia in men with stable ischemic heart disease].

Many studies report the negative correlation between the endogenous testosterone (T) level and majority of risk factors for arteriosclerosis, as well as the presence and the extent of coronary artery disease (CAD) in men. Dihydrotestosterone (DHT) as a non-aromatizable androgen produces more favourable changes in the hormone profile than T. A constant level of androgens, independent on liver function, may be achieved by their transdermal administration. The aim of our study was to determine in a double-blind placebo study whether a 12-week treatment with transdermal DHT can decrease exercise-induced ischaemia in males with CAD. All patients underwent symptom-limited treadmill stress testing at the beginning of the study and after DHT treatment. The selected preliminary results are presented in this paper. Until now the study group has comprised ten men with stable CAD and decreased morning total T concentration. No side effects of treatment were noted during the study. Chronic DHT administration increased significantly: total exercise time, time to the onset of angina and time to 1 mm ST depression. Our results are consistent with the previous publications investigating T effects. The changes of heart rate and systolic blood pressure indicate rather direct coronary-relaxing effect of DHT than peripheral one.

[Dihydrotestosterone therapy of men with coronary artery disease and processes of peroxidation].

In 11 men after myocardial infarction (mean age 58.8 years) influence of 3 months transdermal dihydrotestosterone (DHT) treatment on malondialdehyde (MDA), total cholesterol, HDL cholesterol, triglycerides and platelets was estimated. Decrease in MDA concentration after DHT treatment only in men with low (< 13 nmol/l) testosterone (T) level (p = 0.06) was observed. MDA concentration after 3 months DHT treatment was significantly lower in hypotestosteronemic in comparison to normotestosteronemic men (p < 0.05). Plasma lipid levels and platelet count were not affected by DHT administration. Decrease serum MDA concentration in low testosterone men with coronary artery disease may suggest that DHT treatment in this group has beneficial effects on peroxidation processes.

Right ventricular function in systemic hypertension.

The aim of the study was the assessment of right ventricular (RV) structure and diastolic function in hypertensive subjects. The study group consisted of 44 patients with untreated, mild to moderate essential systemic hypertension. All the patients were in sinus rhythm, no symptoms of congestive heart failure, ischaemic or valvular heart disease and lung disorders were found. Twenty-six healthy subjects were the control group. M-mode echocardiographic measurements of the right ventricular wall (RVW) diastolic thickness, right ventricular outflow tract diameter (RVOTD), left ventricular (LV) structure and LV systolic function were performed. Pulsed Doppler echocardiography was used to measure peak early (TE) and peak atrial (TA) right ventricular diastolic filling velocities as well as velocity-time integrals (VTI-TE and VTI-TA). TE:TA and VTI-TE:VTI-TA ratios were calculated. Similar parameters of the left ventricular diastolic filling were recorded at the level of mitral annulus. Mean pulmonary artery pressure (MPAP) was measured non-invasively by the estimation of pulmonary artery systolic flows. We demonstrated in hypertensive patients significantly thicker RVW (3.94 vs 2.8 mm, P < 0.001) and increased LV mass. In the hypertensive, increased TA and VTI-TA and diminished TE:TA and VTI-TE:VTI-TA ratios were recorded, indicating the abnormalities of RV diastolic function. RV diastolic filling parameters correlated positively with corresponding parameters of LV filling. The results of our study demonstrate that impairment of LV diastolic function, the common finding in systemic hypertension, is associated with diastolic disturbances of the right ventricle. RVW thickening and hypertrophy of interventricular septum seem to be major factors influencing RV diastolic function.

Atherosclerosis risk factors in the inhabitants of the nascent Coal Basin of Lublin region.

There were examined 222 men and 253 women over 18 years of age, selected at random from among country people living in the area of the nascent Coal Basin of Lublin district. There was determined their height and body weight, degree of overweight, general fat content in the body, arterial systolic and diastolic blood pressure as well as blood serum cholesterol concentration, concentration of triglicerides, glucose and uric acid in the blood. It was found out that the level of studied vascular atherosclerosis risk factors in the population under study was lower than the level found by the Polish Experiment in other populations.

[Reference values of selected parameters of lung functions in the population of Bogdanka coal miners].

The aim of the study was to construct and test new equations for calculating FEV1, ITGV and FVC predictive values. Our examined population consisted of 2058 coal miners working underground in Bogdanka colliery (Lublin Basin). The following persons were excluded from out considerations: 1. smokers, 2. miners with pulmonary or allergic diseases history, 3. patients with chronic cough, sputum production or symptoms of chronic rhinitis, 4. persons with bronchial hyperreactivity. Finally, our standard group consisted of 353 coal miners. On the basis of our standard group data, new equations for calculating predictive values were formulated using the multiple regression model. According to these new equations and to equations elaborated by other authors, we calculated predictive values for FEV1, FVC and ITGV in our standard population and expressed results of measurements in percentages of different predictive values. Our predictive values in the standard population were significantly higher than the other authors' values. Percentages of predicted values were lower in the same comparison. We believe that observed differences are linked to a possible self-selection phenomenon in occupationally exposed population, relatively small maximum age of our standard group, possible differences in anthropometric features among examined populations and excluding hyperreactives. Our results seem to emphasize the need of greater attention to the paid on more restrictive use of the specially formulated equations in separate age compartments.

[Unfavorable blood lipid indicators in obesity].

There were evaluated total serum cholesterol, cholesterol HDL, cholesterol LDL and triglycerides in relation to relative body weight, calculated according to Broca-Broughs formula and changes in body weight during last 5 years in 306 males aged 40-59 from the housing estate "Czechow" in Lublin. Mean total cholesterol, cholesterol LDL and triglycerides concentration were higher and cholesterol HDL was lower in overweight males in comparison with normal weight subjects. Significantly higher mean triglycerides concentration was stated in becoming overweight as compared with stable weight males.

Relation between somatic development and some environmental factors in the male population of vocational mining schools in the Lublin Coal Basin.

Somatic development of a child, although genetically determined, depends also on the influence of biogeographic and socio-economic factors of the environment. The effects of these factors popular in the literature are seen in the differences of somatic development of children brought up in urban and rural environments. In comparison with urban population, country children are characterized by lower growth and body mass, delayed manifestation of sexual maturity, greater amount of incorrect posture features, worse state of nutrition and lower index of mental development. The differences in somatic development of children were also found while comparing the size of agglomeration and socio-professional factors determining parents' level of education and financial situation of the family. One could also observe the improvement of somatic development of children living in regions of quick and intensive urbanization and industrialization.

Evaluation of lung function in male population from vocational mining schools of the Lublin Coal Basin.

Occupational exposure to dust and other environmental factors in coal miners may impair the lung function of workers. Besides the coal workers' pneumoconiosis, the inhaled dust may cause decreased ventilatory capacity as a due to chronic bronchitis (9, 13). The same diseases, especially chronic bronchitis, are common in general population and may arise not only from occupational reasons (11, 12). The involvement of the genetic factors, environmental pollutions and cigarette smoking should be considered.

Evaluation of nutritional status of male population from mining vocational schools in the Lublin coal basin.

The basic nutritional mistake in Polish population at considerably rare deficiency of proteins and calories is incorrect composition of diet with the excess of animal fat and carbohydrates. Social and economic changes which influence living conditions result in the change of diet whose trends are not always correct. The problem is the quality of food products, contamination of pollution due to industrialization and the use of chemicals in agriculture, and inadequate proportion in the essential food components or supply of the indispensable trace elements. The other problem is overnutrition leading to obesity which is one of risk factors in civilization diseases (8, 11). The nutritional status depends on the level of education and economic situation of different social groups. It is expected that among the pupils of vocational mining schools who usually come from numerous peasant and working class families nutritional mistakes may occur very often. It denotes both malnutrition and incorrect proportion in consumption of proteins, animal fats and carbohydrates. On the other hand, the expected changes in social and economic status due to a good job create new conditions for proper nutrition. An additional factor which should be taken into account are nutritional requirements resulting from specific character of underground work.

[Risk factors of coronary artery disease in males during a 3 year follow up based on a cohort study in a selected region of Lublin].

To assess changes in risk factors of coronary artery disease, examinations which had been performed in 1981 and 1982 were repeated in males aged 40-59, inhabitants of the district "Czechów" in Lublin. The study was based on Polish Experiment of Coronary Disease Multifactor Prevention assumptions 112 of 203 males investigated in 1981, 1982 underwent repeated examination (attendance rate - 55%). Statistical analysis was performed only in twice examined subjects. Significant increase of blood pressure, body weight, total serum cholesterol and uric acid concentration was stated in comparison with their respective initial values. Significant decrease of HDL-cholesterol and no changes in serum triglycerides concentration were observed. The only positive stated alteration was a lower percentage of tobacco smokers. Diagnosis of coronary artery disease remained unchanged despite of increased risk factors levels (increase of mean Rose's index score from 5 in initial examination to 6.5 in a control one). This high increase in risk factors occurrence allows an unfavourable prognosis for future incidence of coronary artery disease and also demands to undertake vigorous prevention.

[Prevalence of coronary disease risk factors in the male population 40-59 years of age from the housing complex "Czechów" in Lublin].

There was studied randomly selected approximately 10% sample of population of males aged 40-59 from the housing estate "Czechów" in Lublin. Authors assessed the level of coronary artery disease (CAD) basic risk factors as well as a percentage of persons with a high risk of developing CAD. The study was extended by determinations of plasma HDL--cholesterol, glucose and uric acid concentrations. Mean total cholesterol concentration (220.4 mg/dl) as well as the frequency of hipercholesterolemia occurrence (15.4%) were significantly higher in comparison with their values in male populations of Warsaw and Southern Poland. Mean systolic (134.2 mm Hg) and diastolic (87 mm Hg) blood pressures as well as frequency of hypertension occurrence (22.7%) were comparable with results of two, above mentioned, epidemiological studies. We stated the lower percentage of current tobacco smokers (52.2%). The overweight of 115% or more was stated in 22.2% of persons. Very high percentage of men of low physical activity was stated (51%). Higher level of respective risk factors reflected in individual score of CAD risk coefficient fixed on 5 points and being higher than in other studies of different Poland regions populations.

[Content of overall fat in the body and levels of selected coronary risk factors].

Authors estimated total adipose mass based on skinfold measurements in 205 males aged 40-59, residents of the housing estate "Czechów" in Lublin. They studied the correlations among calculated total adipose mass and arterial blood pressure as well as serum lipids levels. It was stated, that total adipose mass of the examined group was similar with those of other urban males populations. Total adipose mass well correlated with arterial blood pressure and serum triglycerides levels, but poorly with total serum cholesterol concentrations. Greatly significant inverse correlation between total body adipose mass and a concentration as well as a percentage of HDL-cholesterol was proved.

[Effect of physical exertion on respiratory airflow and airway resistance in patients with chronic bronchitis].

In a group of 30 men aged 44-60 years with chronic bronchitis the effect of physical effort was assessed on the value of airways resistance and expired air flow. Physical effort was noted to increase the air flow and decreases airways resistance in most cases of chronic bronchitis. A significant negative correlation was demonstrated between the TGV value and the flow-volume value, and changes of air flow after effort in chronic bronchitis patients depended on the resting TGV value.

[Endocrine ophthalmopathy--etiopathology].

Endocrine ophthalmopathy is regarded as organ specific disease of autoimmunological origin. It occurs most frequently in patients with Graves disease, less frequently in those with Hashimoto thyroiditis. The presence of HLA DR-3 and HLA b-8 antigens, cigarette smoking, treatment with radioiodine, and thyroid function disturbances predispose for the development of the disease. On the other hand, the most important pathogenetic mechanism is fibroblast stimulation and increased collagen and glucosaminoglycans production within the extrabulbar muscle interstitium.
