Anorectic behavior, mood, and metabolic and endocrine adaptation to starvation in anorexia nervosa during inpatient treatment.

Twenty-two patients with anorexia nervosa were studied at 2-week intervals during treatment on psychiatric wards. In order to characterize the metabolic situation in starvation, levels of free fatty acids, beta-hydroxybutyric acid, and acetoacetate were measured. The endocrine adaptation to starvation was studied by measuring triiodothyronine, noradrenaline, and cortisol. Anorectic symptoms were assessed by the Anorexia Nervosa Inventory Scale (ANIS) and mood changes on the basis of a "Befindlichkeits" Scale (BF). Only half of the patients showed metabolic and endocrine signs of starvation on admission to the hospital, despite low body weight. This group had significantly more severe anorectic symptoms (ANIS) and gained weight at a lesser rate. Metabolic signs of starvation disappeared during the first 4 weeks of therapy in most of the patients. The endocrine indicators for starvation normalized much more slowly, with noradrenaline having the slowest pace. "Bulimics" and "restricters" could not be distinguished from one another by metabolic or endocrine observations. The "bulimics," however, showed more severe anorectic symptoms (ANIS) and a more depressed mood.

[Swimming pool granuloma--a mycobacteriosis].

We report on a patient suffering from swimming pool granuloma after having handled in fresh-water aquarium. As the causative organism, we could isolate Mycobacterium marinum from a nodular, ulcerous lesion.

[Volon A shake mix--a promising innovation].

Clinical testing of Volon A shake lotion (containing zinc oxide and corticosteroids) showed good results especially with regard to acute dermatoses and, as a surprise, in patients suffering from gram-negative infections of the feet.

[Acquired nevus flammeus (author's transl)].

We report about two patients with acquired nevus-flammeus-like skin changes. In one case the skin alterations spread all over, in the other case the mole - although it became bigger - was restricted to the right foot. The first patient was pregnant, the second in puberty. There were no hints to any traumas before. Biopsies revealed findings as in nevus flammeus.

[Epithelioma cuniculatum - a peculiar type of cutaneous squamous cell carcinoma (author's transl)].

We report about three cases of epithelioma cuniculatum. In two patients the tumour occurred at the auricle, in one patient at the forepart of the foot. One tumour at the ear was removed by local surgical excision, the other was treated by x-rays but without lasting success. In one patient amputation of the foot had been necessary because of intensive local invasion. The differentialdiagnosis especially at the sole of the foot must include viral warts and pseudocarcinomatous tumours as papillomatosis cutis carcinoides. The tumours on the ear could be misinterpreted as keratoacanthomas. Clinically the cauliflowerlike tumours empty on pressure foul smelling keratin. Histologically they show traits of a squamous carcinoma of low grade malignancy and an extreme keratinization beside the typical numerous anastomosing crypts similar to rabbit burrows.

Proliferation and production of hemopoietic cells two stages of disease: preleukemia and overt leukemia.

The kinetics of erythropietic and granulocytopoietic cell proliferation have been investigated in the same patient at two distinct stages: firstly in preleukemia presenting as pancytopenia with ineffective erythropoiesis, and secondly 2 years later in acute myelogenous leukemia. The method investigation is based on determining the DNA synthesis rate of individual cells by means of quantitative 14C-autoradiography after short-term incubation with 14C-thymidine and fluorodeoxyuridine. Erythropoiesis was equally ineffective in the two stages, the rate of proliferation, however, slowed down towards the leukemic state. The production rate of myeloblasts was markedly reduced in preleukemia accompained by a very low labelling index. In leukemia on the other hand the production rate was increased to such a degree that the leukemic myeoblast compartment is to be considered as prevailingly self-reproductive. The proliferation rate of myeloblasts was reduced already in preleukemia, and there was a further decrease in leukemia. From the point of view of cell kinetics the manifestation of leukemia in this patient is explained best by a change in the mode of proliferation: the myeloblasts change from steady state growth to behaving like an exponentially expanding population.

Rate and time of DNA synthesis of individual Chinese hamster cells.

The duration of DNA synthesis of a diploid cell line of Chinese hamster fibroblasts was determined in a comparative study by the FLM technique, and also by a new technique for measuring the rate of DNA synthesis of individual cells. These methods produced comparable results when applied during exponential growth of the cells. The rate of DNA synthesis was measured by means of quantitative autoradiography following a short-term incubation of the cells with 5 X 10(-6) M FUdR and 10(-5) M 14C-TdR. The choice of the medium for this purpose did not seem to be critical. The autoradiographic silver grains over cells and 14C-standard sources are counted by microphotometry using incident light bright-field. The direct measurements of DNA synthesis rate are 'compartment' statistics which have been converted into 'flux' parameters for comparison with the FLM method and applicability in cell-kinetic calculations. Frequency distributions of the rate of DNA synthesis of individual cells thus obtained may resemble normal distributions quite closely. They result from several factors: differences in the rate of synthesis in different parts of the S-phase, the density distribution of cells within the S-phase, the variation in the time of DNA synthesis among individual cells, and the experimental error. In the case of a pronounced partial synchronization as probably has been present in one experiment performed in the lag phase, an incorrect time of DNA synthesis may result from the rate values. Due to the variation in DNA synthesis rate in different parts of the S-phase it is not possible to determine the duration of DNA synthesis of an individual cell. However, the mean values of DNA synthesis time are reliable. The new method will be preferentially applied for determining the duration of DNA synthesis of human cells in as far as difficulties are encountered with the classical methods. In addition, it may be used to advantage for studying cells which make up low percentages in mixed populations. It finally permits a safer morphological classification of the cells under study than is possible with the classical methods.
