Tubeless translaryngeal superimposed jet ventilation.

Microsurgical endoscopic interventions of the larynx offer an optimal approach to the surgeon by providing an unrestricted operative field. During such operations, ventilating the patient should in no way be impaired. For this reason we have developed a new type of tubeless jet ventilation which consists of both low-frequency and superimposed high-frequency jet ventilation. In addition, we have integrated two specifically sized jets into a Kleinsasser laryngoscope, placing them at different sites. This technique guarantees adequate ventilation with an oxygen-air blend. Due to the Venturi effect, air and tidal volumes are also enhanced when passing through the external open end of the laryngoscope. This type of tubeless jet ventilation was applied to more than 60 patients, using a prototype jet. Anesthesia consisted of a continuous intravenous administration of propofol, with sufentanil and vecuronium given as needed. Clinical results revealed optimal ventilation of all patients without hypercapnia or other complications. Operative conditions for the surgeon were also very satisfactory. Findings demonstrated that this type of tubeless jet ventilation is also particularly suited for laryngeal laser surgery, thus avoiding flammable tubes and noxious anesthetics.

[Tube-free translaryngeal superposed jet ventilation].

Microsurgical operations on the larynx require sufficient space for the surgeon in order to achieve the best surgical result. After preliminary experimental studies we integrated two jets of a specific size into the Kleinsasser tube. Simultaneously, we developed a "superimposed jet-ventilation system", which consists of a low-frequency jet ventilation and superimposed high-frequency jet ventilation. Respiration was maintained with a mixture of oxygen and air, whereby an additional increase in air and volume via the Kleinsasser tube, which is open on the outside, can be sustained on account of the Venturi effect. We tested this tubeless translaryngeal superimposed jet-ventilation system in 48 patients. Anesthesia was carried out by continuous intravenous administration of Propofol and intermittent doses of Sufentanil and Vecuronium as required. The clinical results showed optimal ventilation without hypercapnia. The arterial pC0(2) levels were below 42 mmHg. The arterial p0(2) levels were above 120 mmHg with a FIO2 of 40%. No complications were observed with regard to respiration during any of the operations. The surgeon had optimal conditions to carry out the operation. Because of the absence of a plastic tube, inhalation anesthetics and nitrous oxide, laryngeal laser surgery is another field of application for which this form of tubeless jet ventilation is excellently suited. We tested it with 12 patients, and no complications due to laser anesthesia were observed. We consider this form of a tubeless superimposed translaryngeal jet ventilation to be a great improvement in microlaryngeal surgery.

[Otorhinolaryngologic emergencies in practice].

In practice, the most serious emergency cases may result from shortness of breath and bleeding. Some typical diseases such as subglottal laryngitis in small children and the edema of the larynx entrance are described in greater detail and emergency measures are discussed. Special attention is drawn to the aspiration of foreign bodies and the complications connected with the desensitization of allergy patients. The dominating role of cortisone, the provision of drugs for shock treatment, and possible emergency steps are discussed. A further subject is bleeding disorders in the field of otolaryngology; above all the treatment of epistaxis is discussed in greater detail. Reference is made to the various local emergency measures which in practice may become inevitable. In this context the limits of therapy outside the hospital are listed.

Emergencies↗

[On atypical partial resections of the larynx (author's transl)].

With reference to the relevant literature the paper reports on three patients in whom a partial resection of the larynx helped to remove the tumour and largely restore the laryngeal functions. The intervention differed somewhat from the usual procedure. Voice, breathing and swallowing functions remained intact despite the radical nature of surgery. The indication for partial resection was based on the extension and differentiation of the tumour (exophytic growth), the localization being less decisive. Among others, the laryngeal fissure proved useful as access path. A practically normal swallowing function was due to an at least partial preservation of the epiglottis. Two of the three patients operated in this manner can already be regarded as permanently cured. This surgical method should be used for selected cases only.

[Surgical therapy of chronic parotitis].

A short survey of the therapeutical possibilities available in cases of chronic parotitis is followed by a discussion of the results achieved after surgical treatment. This discussion is based on the findings obtained in follow-up checks. The - very strict - indication for surgery was not made until all conservative measures had been exhausted and thus it involved only 2.4% of 380 surgical patients suffering from various diseases of the parotis gland. It was possible to achieve a sustained total absence of complaints in all cases, with partial or subtotal resection being sufficient in most instances. Although postoperatively partial loss of the N. facialis function occurred relatively frequently, permanent damage was not observed in any single case.

[On the value of auxiliary findings in parotid surgery (author's transl)].

The value of the individual examinations and tests that were carried out before and during surgery of tumors within the parotid was evaluated on the basis of a major number of surgical cases. Most indications for surgery were based, however, solely on the clinical examinations if the tumor was isolated in the parenchyma, well-differentiated and mobile. The radiological examinations are of importance only if a differential diagnosis is to be made to determine lithiasis, foreign bodies and sclerotic processes. It is of no relevance for pure tumor diagnosis. Sialography and scintigraphy are of a certain value for diagnostic purposes, both methods, however, are too vague in tumor diagnosis and may lead to wrong results. Still, frozen section and exploratory excision are the most reliable approaches for determining in vague cases the optimal extent of excision in accordance with the respective state of the growth. Needle biopsy was deliberately avoided in tumor patients since the danger of a propagation of tumor cells in certain types of tumor--in particular the pleomorphic adenoma--must not be neglected.

[On the elasticity of the ligamentum anulare. Experimental investigations into human temporal bone preparations with the help of an electromagnetic probe (author's transl)].

An electromagnetic probe was used to investigate the elasticity of the ligamentum anulare in human temporal bone preparations. The static measurement showed with preserved auditory ossicles but without tympanic membrane an average displacement of 37--41 micron at a pressure of 10 mN. Five different points of measurement did not lead to any significant differences; we must assume that the elastic properties of the ligamentum anulare are completely equal. After the removal of the malleus and incus, the displacement increased by only 2.2 micron, by another 4.2 micron if the stapes tendon was severed. Under these circumstances, too, a completely even elongation is proved to occur in all sections of the ligamentum anulare. Different pressures lead to a no-load characteristic which is linear in the 5--20 mN range, suggesting excellent elastic properties of the ligamentum anulare. From these data we may assume that the oscillation behavior of the basis stapedis approaches that of a membrane pump, with a linear transmission of great oscillation amplitudes possible.

[On the surgery of esophago- tracheal fistulae (author's transl)].

Esophago-tracheal fistulae develop to an increasing extent due to the long-term intubation in intensive-care patients. Their treatment is usually problematic, since the general condition of the patient, weakened by his basic disease, very often does not permit any major intervention; in addition, the topographical location of the fistula makes the access to it even more difficult. In one patient two attempts failed to close the fistula from an extended tracheostomy. The narrow access passage between the anaesthetic tube and the tracheal wall did not permit an exact suture; the heavy formation of scars in the fistular environment did not permit either a preparation in layers or the shift of mucosal flaps for coverage. However, a fistula was successfully closed from a collar mediastinotomy. With normal tracheal width, unrestricted esophagus function was restored without damage to the recurrent laryngeal nerve. The advantages of this surgical method are discussed in greater detail.

[On the oscillation form of the stapes as a function of the volume. Experimental investigations into the human temproal bone preparation (author's transl)].

In order to further clarify the change in the oscillation form of the stapes as a function of the volume, measurements were performed in 10 fresh temporal bone preparations at two points at the short and long diameters of the base of the stapes from the labyrinthine side. The oscillations were recorded from two channels, first, by two electro-dynqual stress on the base of the stapes, also optically by modulating a light beam and electronically scanning by means of a phototransistor, and the oscillations were then recorded on a two-beam oscilloscope. The result was that it is not possible to prove any phase shift between the two measurement points at the short or at the long base of the stapes in a volume range within the physiological limits. Thus, any sweep phenomenon at high volumes does not exist. Also, a volume-dependent shift of the oscillation axis on a line within the base was not to be observed.

[Long-term observation in juvenile papillomas of the larynx after surgical removal and ultra-sound treatment (author's transl)].

The paper cover 17 juveniles suffering from papillomas of the larynx. They were treated mainly surgically and with ultrasound. In most cases repeated interventions were necessary over an extended period of time. Because of the -- frequently considerable -- extension of the papillomas, a double spoon was used to remove larger tissue parts before the ultrasound treatment. In six children a tracheotomy was inevitable, in four of those children it was necessary to preserve it over several years. In the course of the treatment a lengthening of the intervals was generally noted. Related to an observation period of up to 10 years, out of 16 patients 13 were in the end permanently without any relapse, the remaining can be expected to be cured definitely because of the significant improvement of the local evidence. No malignant improvement of the local evidence. No malignant degeneration has been observed so far. All the methods of treatment used to date, including ultrasound and cryoscopic probes, are not entirely satisfactory, at least they do not constitute any causal therapy.

On the oscillation behavior of the incus-stapes joint (author's transl).

A two-channel inductive measuring system in the human temporal bone preparation was used to oscillographically record oscillations of incus and stapes as well as to observe the phases between the two auditory ossicles at the same time. In the frequenzies between 300 and 1,200 Hz it was not possible to prove clearly a phase difference, in the higher frequency ranges (approximately from 2,000 Hz upwards), however, a phase difference was observed to occur regularly to an extent of 90 degrees and more.
