Idiotype vaccination against murine B cell lymphoma. Humoral and cellular responses elicited by tumor-derived immunoglobulin M and its molecular subunits.

C3H/HeN mice were immunized with idiotypic immunoglobulin M (IgM) and its molecular subunits from the syngeneic 38C13 lymphoma. Immunization with idiotypic IgM (38C-Id) resulted in idiotype-specific humoral and cellular immunity and protection against a lethal tumor cell challenge. Heavy (H38C) and light (L38C) chains were isolated by electroelution from preparative polyacrylamide gels. Both of these immunogens induced significant resistance to a subsequent tumor challenge. Variable region immunogens, in the form of trpE-fusion proteins, were obtained by cloning heavy and light chain variable region genes into the expression plasmid pATH-11. Of these, only the trpE-VH38C immunogen yielded immune resistance to tumor challenge. Finally, the nucleic acid sequence of 38C-Id light chain was determined and, based on the corresponding amino acid sequence and an analysis of predicted secondary structure, a region of potential antigenicity in complementarity-determining region 3 was chosen for the production of a synthetic peptide. Vaccination with this synthetic peptide resulted in significant suppression of tumor growth. Analysis of the humoral and cellular immunity generated by these vaccines revealed the presence of antibodies reactive with native idiotypic IgM only in 38C-Id, H38C, and trpE-VH38C immune sera, although the latter two were not idiotype-specific. Idiotype-specific lymphocytes, which proliferated in response to native 38C-Id, were observed in all immune animals. With the exception of the fusion protein immunogens, conjugation to an immunogenic carrier protein (keyhole limpet hemocyanin or thyroglobulin) was required for optimal humoral and cellular responses.

Comparison of Biobrane and Scarlet Red dressings for treatment of donor site wounds.

Two methods of autograft donor site management were tested in 31 patients with burn injuries. Two donor sites of equivalent size were respectively covered with Biobrane and Scarlet Red, two commercial dressing materials available for donor wound coverage. Pain, the amount and type of exudate developing underneath the dressing, adherence, and cellulitis were evaluated on a daily basis, and healing time was determined. Biobrane proved superior to Scarlet Red with respect to control of pain, accumulation of exudate, and healing time; the materials were equal with regard to adherence to the wound, the character of exudate, and the appearance of cellulitis.

Sleeping bradycardia during recovery from childhood status asthmaticus.

Seventeen of 67 children recovering from status asthmaticus developed sleeping bradycardia. They were compared to a control population of asthmatic children who did not develop sleeping bradycardia and differed only in the low heart rate. The authors speculate that this phenomenon is due to a combination of withdrawal of sympathomimetic stimulation and catch-up in REM (rapid eye movement) sleep.

Albinism. Recent advances.

There have recently been several fundamental advances in our understanding of albinism, both in animals and in man. These advances have been in the division of oculocutaneous albinism into tyrosinase-negative and tyrosinase-positive forms, the demonstration of a widespread disturbance in the structure of melanosomes in X-linked ocular albinism, the description of the condition now known as autosomal recessive ocular albinism, and a peculair anatomical abnormality of the visual pathways which appears to be present in all forms of oculocutaneous and ocular albinism. Evidence has been presented which confirms that the retinogeniculate anomaly in human albinos is similar to that in animals, and it has recently been suggested that there may be two major patterns of geniculocortical projected in man, as there are in the Siamese cat.

Preexisting psychiatric disorders in burn patients.

The authors studied 51 of 56 consecutive eligible patients admitted to a burn unit. Sixty-nine percent of the patients had a preburn psychiatric diagnosis. Depression alone was present in more than half the sample. Few burns were strictly accidental; 68% involved some degree of complicity on the victim's part, but intentional burns were rare. Patients with depression were more likely to sustain their burns in a setting of risk-enhancing behavior.
