The present paper reports our first results after pars plana vitrectomy in patients with diabetic retinopathy and hemodialysis with a follow-up of 6 to 24 months. Between January 1992 and October 1994 we performed vitreoretinal surgery with silicone oil tamponade in nine eyes of seven patients with diabetic nephropathy on hemodialysis. All patients had had type I diabetes for 19-32 years. Over the observation period the retina was completely attached in eight eyes. Final visual acuity of 0.1-0.7 was attained in four eyes, 0.06 two, hand movements in one eye. Two eyes had no useful final visual acuity because of redetachment of the retina or secondary glaucoma with rubeosis iridis. The small number of complications shows that pars plana vitrectomy can be done in diabetic patients with nephropathy on hemodialysis. This significantly improves their quality of life.
Assuming, that deterioration of renal function in the primary glomerulonephritis may also result from the destructive lesions to parenchymal tissue of the renal cortex (with simultaneous lesions to renal tubuli), the assessment of the urinary excretion of beta 2-microglobulin could be approached as an indicator of renal function in this situation. The study involved 85 patients with chronic glomerulonephritis treated with immunosuppressors++. In these subjects beta 2-microglobulin concentration per one unit of the urine (UB2M) together with fractional excretion of this globulin (FEB2M) was studied. Statistically significant differences in mean UB2M and FEB2M values have been noted between patients with remission and with the lack of the remission following therapy. Gradual increase in UB2M values as well as FEB2M values have been noted with deterioration of excretory renal function. The results have shown, that the assays of UB2M, and particularly FEB2M, is a sensitive marker of proximal renal tubuli function and indirectly of the processes which take place in extraglomerular renal cortex structures. It might be there fore of prognostic value.
Ultrasound was used to measure echogenesis, renal cortex thickness and kidney length in 59 patients with various types of the primary glomerulonephritis. The obtained results have shown that the measured parameters correlated with the progress in renal failure. An increase in the intensity of echo from the cortex, thinning of the renal cortex, and the decrease in renal length are typical signs of the advancement of renal failure. As renal function deteriorates in the consequence of destruction of its parenchyma, ultrasound examination might be an illustration of the pathological changes in the kidney. Considering the fact that ultrasound is perfectly safe, it may be used in monitoring of the disease progress and the results of treatment.
