Modified Seldinger technique for the insertion of standard chest tubes.

Closed tube thoracostomy is a standard procedure for the evacuation of air, blood, or other materials from the pleural space. This paper describes a modification of the Seldinger technique that facilitates chest tube insertion. Either a Nelaton or Thieman catheter is threaded into the side drainage hole and out the tip of a standard Argyle-type chest tube. After using the clamp to insert the catheter into the pleural space through a previously dissected tract, the catheter serves as a guide over which the chest tube is inserted. The technique is simple to use, effective, and safe. It employs standard, inexpensive materials to insert chest tubes in such a way as to minimize the potential traumatic complications inherent in other techniques.

Myocardial heat shock gene expression in pigs is dependent on superoxide anion generated at reperfusion.

The heat shock response is a conserved response to cell injury. We sought to determine if ischemia alone versus events at reperfusion stimulated expression of the major heat shock protein (hsp-72) in a clinically relevant model of global myocardial ischemia in pigs. Pigs were placed on nonpulsatile cardiopulmonary bypass. Serial transmural cardiac biopsies were taken at baseline following 20 min of normothermic global ischemia (induced by crossclamping the aorta) and at 20, 40, and 60 min of reperfusion. Test animals received a bolus and subsequent aortic root infusion of superoxide dismutase (total 7,500 U/kg) beginning just prior to reperfusion. Hsp-72 mRNA abundance was estimated from Northern blots. We found that hsp-72 mRNA was not induced following 20 min of ischemia but accumulated to high levels within 20 min of reperfusion. Intravascular administration of superoxide dismutase at reperfusion eliminated hsp-72 mRNA induction. We conclude that in the postischemic myocardium, hsp-72 gene expression is dependent on superoxide anion generation at reperfusion. In this setting, hsp-72 gene expression may reflect a specific response to oxidative injury rather than a more general response to metabolic stress associated with ischemia.

The antinociceptive and motivational effects of intranigral injection of opioid agonists.

The antinociceptive potency of morphine and the morphine metabolite morphine-6-glucuronide (M6G) was examined after injection into the substantia nigra and periaqueductal gray (PAG) of rats. Both drugs produced antinociception in both sites. The antinociceptive potency of M6G was significantly greater than morphine in the nigra. There was no difference in the antinociceptive potency of M6G in the nigra and PAG. M6G and other opioids were also examined for motivational effects after intranigral injection. A high dose of intranigral morphine (10.0 nmol) produced a conditioned place preference. No significant motivational effects were produced by 1.0 nmol of M6G, D-Ala2, N-Me-Phe4,Gly5-ol-enkephalin (DAGO), D-Pen2,D-Pen5-enkephalin (DPDPE), or U-50,488H. It is concluded that the substantia nigra plays an important role in opioid antinociception. The role of the nigra in opioid reward is questionable.
